Very curious to hear what @simontutt88 have to say about this? A paramedic (not completed ACP training either) has opened a private GP clinic and is labelling themself a "medical expert". Is this not dangerous and unsafe, with no supervision?

@VishwajeetP4t3l Advising doctors (BMA members) on what the limits of scope their assistants should be performing is well within the BMA's remit. PA groups are now frazzled because your ability to play doctor and do whatever you want to pts without going to medical school is now in jeopardy.

@NatashaMDay @simontutt88 Paramedics absolutely do get supervised when working in non-pre hospital settings, ie A&E or GP. This person does not have the training or knowledge to recognise their unknown unknowns. What may seem like minor illnesses can often be very sinister or dangerous.

An anaes dept suggesting that a student AA, on completion of their 2 year course, will have "knowledge and skills roughly equal to Primary FRCA level". I recall 5 years MBBS + House Job + years working as a doctor + the actual FRCA to achieve that. The AA must be a genius?

@ExNHSorthopod @simontutt88 It's interesting as well, because allegedly when registering with the CQC you can only choose options like "NHS GP service" or "independent consulting doctor". This person has registered as a doctor/GP. So seems like fraud and using a protected title to evade the CQC?

If he had done Graduate Entry Medicine, he'd be an SHO chained to the ward, having to beg for clinic time to meet portfolio requirements, and having done a total of 0 endoscopies. But because he took a shortcut he's promoted. Training in this country is a joke.

@AnalysisDrawer Immediate restriction of scope of existing PAs to originally intended role of workload assistants - scribing, bloods, cannulas, prepping paperwork. Ward based only. No theatre or clinics. No new hiring of PAs. Reallocate funding for PAs to JCF and CESR posts.

But equally, 6 months of ICU/Anaesthetics an an ACP does not compare to the same time spent there as a doctor, with the underpinning background.

@parthaskar @gmcuk This would make an excellent QIP/Audit project for some juniors! After a PA led ward round, go around a few hours later and survey the patients as to who they think they saw and what their role is. 100% should be able to say "I saw a PA and not a doctor". Else GMC referral.

@NatashaMDay @simontutt88 Can you read English? I said in non-pre hospital settings. An ambulance is pre hospital medicine and bread and butter for a paramedic. General Practice is bread and butter for a doctor with 10+ years of training (as a doctor) and the MRCGP exams.

Completely agree. A consultant anaesthetist can supervise more than one theatre at one time. But the anaesthetists in those theatres need to be fully trained doctors specialising in anaesthesia, with appropriate exams that builds on their broad base of training. Not assistants.

Just like the RCP have published guidelines for PAs instructing them to introduce themselves as their role and also "I am not a doctor", maybe the Geri's soc could lead the way in doing the same with AHPs? If the RCoA pushed a directive/guidance tomorrow that no consultant should supervise any AAs, the idea is dead in the water. Nothing they can do.

This one is even more egregious, Anaesthesia Assistants working in the hospital's dedicated emergency theatre (CEPOD) where the sickest and highest risk patients are sent.

As I have said for decades, the college does not have your best interest at heart. You cannot simultaneously "want to steer the direction of travel" & also allow trusts to deploy AAs as they see fit as a matter of "local governance". They are contradictory. Stop this immediately.

More work must be done with regulatory bodies (NMC/HCPC) and the police to pursue these cases of fraud with genuine sanctions and prosecutions in the most egregious cases. Only then will people think twice before pretending to be what they're not. GEM is open for the rest.

PAs may have a role, not as surgeons, but as originally planned - physician's assistant that can mop up the ward work allowing surgical doctors to go to theatre/clinic and learn.

They have all, with 0 exceptions, achieved the same quality assurance benchmarks. PAs have none of the above. World of difference.
