The BCG was first developed in 1921...It remains the only vaccine used for TB. COVID-19 has 12 developed in less than one year. If you think the 100 year lag for TB is because it has a complex immunological response rather than it is a disease of poverty, then you'd be wrong.

I wish people would stop talking about ICU capacity based on number of ventilators. Unless critical care has turned AI in 4 years since I was clinical fellow, I remember it being a complex intervention requiring highly skilled nurses, physio, pharmacists & intensivists.

Spiking a temp isn't sepsis. Sepsis is life threatening organ dysfunction secondary to an infection. Spiking a temp is just spiking a fucking temp - all other obs & bloods normal - for the love of Christ stop labelling everything as sepsis.

I'm still surprised how often when I'm asked to see patients with severe infections that they have not had an HIV test performed. Treated HIV has similar life expectancy of someone without HIV. To treat you have to diagnose. Offer testing to everyone. 🤷🏻‍♂️

Seriously, I'm done with being a reg...Three calls about antibiotics, same patient, regarding their allergy which isn't an allergy. There is a special place in Hell for doctors that label patients allergic to penicillin for "nausea/sickness".

A nurse told me I was in the wrong the PPE the other day as went onto the red ward in a surgical mask and needed to be in FFP3. Great we finally got here, but made me reflect on 2019 when we were told to not wear surgical masks outside wards as could frighten patients 🤣

2/3 The PA dispute seems to me to be a symptom of a wider generational chasm that is emerging in medicine (in wider society too). It has been present in the recent industrial action and in other controversies as well. It is real and is worrying as it feels very destructive.
