In the past 6 months or so I have heard similar things from 4 GPs who have worked with PAs and who want to remain anonymous:
- The PAs have been enthusiastic
- However they have been (extremely) over-confident
- Initially there is a GP rostered to supervise and debrief.

As an intern in one of the busiest hospitals in India I used to work between 60-100 hours a week
During my 3 months of medicine I was doing 80-100hrs
I then did foundation training in the NHS. Ended up doing 50-60 hours a week
I never felt tired in india.
NHS exhausted me.
A GP here now replaced by a PA
It means:
1) less appointments available (GPs can do 3-6x work of PAs)
2) worse outcomes due to far less clinical expertise
3) knock on impact to hospital nearby with more patients having to access due to delays in care
4) cost more to the taxโฆ
โWeโre creating PAs to support NOT replace GPsโ IS A COMPLETE LIE!

Just had a call from my GP querying why I wanted to switch abx and painkillers, I said the locum (PA) yesterday had tried to change it but I had disagreed with it due to allergies- GP sounded so exacerbated as she told me that 6 of her morning appts were solving PA errors

2000 more doctors working in Primary Care. Sure, but that includes trainee numbers, who need time and training from qualified GPs
Thereโs actually 920 less GPs since 2019.
So fed up of lack of honesty fromโฆ
I understand PAs being happy and relieved about House of Lords passing through GMC regulation.
For the profession of PA itโs great.
This a key part of enabling the 2 tier health system.

A PA can immediately work in primary care after a 2 year clinical course.

We frequently see PAs and NPs here. There are simply not enough MDs to see patients. Most retired after Covid.
So. I'd rather see a PA /NP rather than no one. That's unfortunately our choice.
The House of Lords talked about bullying that PAs/AAs have faced last night to shut down legitimate concerns that doctors and patients have.
No one talked about the absolute moral injury that doctors have faced whilst they see standards in the NHS tank over the last 15 years -โฆ

This is a PA who is complaining that a patient was complaining about being seen by a student rather than a doctor.

1) blur lines between professionals (doctors and non-doctors)
2) this is completely driven by corporate greed (profit margins are higher with a less skilled workforce and reducingโฆ
Medical associates are working beyond their competence.
Backdoor changes will further confuse the public, put patients in danger and only benefit big business and profits. Our NHS needs more actual doctors, not less.

Weโre seeing 12 PAs being hired at between ยฃ70k-90k
If we look at NHS โproductivityโ alone itโll require 3-4 PAs to replace a GP(with most like worse outcomes)
This is about creating a 2 tier system

I was told this week, for the second time, that the PA at our surgery is exactly the same as a doctor but he just canโt sign a prescription.
I have said this before - I run nursing homes.
I have a named GP on retainer for each home and a geriatrician that helps the GPs.
I do this so we can prevent admissions to the NHS and be proactive with our residents
No way would I put a PA into nursing homes as itโs one ofโฆ

They are using this as a tactic
1) to blame doctors for the wait list
2) pushing people towards private care 3) or letting people die on the wait list

21,000 doctors and public members wrote to MPs about their concerns with regards to PAs/AAs.
To put that into context, thats 10x more than those that responded to the 'extensive public consultation' by the GMC/DoH into PA/AAs.
Yet all we see is words. Not action.
UK NHS healthcare has deteriorated significantly since I went to medical school in 2004.
A key reason why I left in 2015/16 was due to my not being able to deliver the level of care I wanted
What I never thought at that time was that doctors will be involved in criminalโฆ
Just for full clarity, when a healthcare professional STOPs medicationsโฆ itโs also PRESCRIBING.
