➡️ self referral to physio for back pain already exists
➡️ if you have INTERNAL BLEEDING you can’t just go straight for a ‘test’ without being assessed by a doctor
The main reason why the public can’t get timely appointments with GPs is because there are not enough GPs
There is no other way of fixing the problem without recruiting more GPs & stopping GPs leaving
It’s simple maths

President of the Royal College of Emergency Medicine Dr Katherine Henderson clarifies that it is the sickest, most vulnerable, ambulance bound patients requiring admissions that have the most impact on A&Es rather than those unable to get face to face appointments with their GP
When I started as a GP Partner (long-time ago) my Senior Partner always used to say ‘3 strikes and you are in’ -
meaning if you have 3 contacts with the Practice in a short time & not improving - then consider admission
This is always at the back of my mind & the advice has…

After the third contact with a GP surgery about a condition/symptom, a case should be elevated for review. It should be red-flagged & set procedures & guidelines should be followed, including a referral to a specialist. Please RT

- The public has the right to know why they are not being seen
- Referrals are rejected with a few lines of advice
- If GPs wanted advice we would use ‘advice & guidance’
I have been training GP Registrars (aka trainees, ST1,2,3s) for 20 years - but the obsession with high quality ‘reflective learning logs’ & Eportfolio means I will throw in the towel soon!
For over two weeks, patients have been able to get treated for seven common conditions like sinusitis or earache directly from their local pharmacy.
This is freeing up millions of GP appointments and giving patients easier access to the care they need, when they need it.
I kid you not a Consultant Gynaecologist recently slagged off GPs for referring too many patients on the 2ww cancer pathway!
However the pick up rate was 8% - which means GPs are actually under referring

A terrible restriction of freedom of speech
A medical journalist should pick up on this abuse of powers
The government has announced plans to make it easier for patients to contact their GP and end the 8am rush.
They plan to train more receptionists to access patients' needs and speed up the booking process.
Pharmacies have signed up to & started ‘Pharmacy First’
Slight problem is many haven’t had clinical training to assess - sinusitis, sore throats, ear infections, infected insect bites, shingles, impetigo, UTIs 🤷🏽♂️
Renal Consultant sees patient in outpatients clinic - 1 week later letter to patient (GP copied in):
‘on receipt of this letter please ask GP to repeat u&es (blood test) and send me the results’
Don’t dump your work on the patient & GP - deal with your own smoke!
Seeing same GP each visit ‘cuts workloads and improves health’
Yep - that’s why we kept our model of 6 GP Partners for 9000 patients (with ‘usual GP’ lists)
GP Referrals
Looks like the triage of GP referrals by hospital specialists is on the horizon.
Effectively creating a default ‘advice & guidance’ service - with fewer patients actually being seen.
Poorer outcomes for the patient & higher clinical risks by carried by GPs
The whole point of ‘remote GP consulting’ is you can work from anywhere
Not sure why everyone has gone crazy about it now - when the Govt has been pushing Babylon GP at Hand??

The working environment is so toxic many GPs are burning out, retiring early, young Drs are not joining
The solutions will take years & years
12 yrs of a Tory Govt has decimated our NHS
As GP Partners I feel we carry alot of clout compared with hospital Consultants
If I need a patient safety issue raising I won’t be intimidated by anybody
A lot of people asking why the drs never went outside the trust. Not to the police, media, other agencies. It is a legitimate question....but our report shows the culture of the org didn't really encourage this. How true is this of other NHS trusts?

How many clinical hours have been wasted with GPs sitting in PCN meetings across the nation??
Would have been better off seeing pts, improving access at a critical time
After 25 years I have now stopped doing joint & soft tissue injections.
