A PAs having a doctor they can chat to if they want advice is not supervision, it’s working independently. A GP can contact the microbiology consultant for advice but no one thinks the Microbiologist is supervising the GP.

This is exceedingly worrying, this exam question suggests it’s totally fine for PA to see a sick child and the correct course of action is to send them home without a doctor seeing the patient. The plan is to replace doctors with PAs.

I’m flying a plane full of passengers, and have learnt the basics of flying. I’m not sure what to do in an emergency and not sure what weather conditions make it safe/dangerous to fly. However, I have a experienced pilot who I can phone if I think I might need some advice. Safe?

Medicine was a profession I was proud to be a part of and it’s heritage goes back hundreds of years. Despite some quackery we’ve seen standards climb year on year. The legacy, it’s seems, has ended due to the thuggery of politicians who have destroyed our once noble profession.

So seems some GP practices and trusts going rogue and letting /AAs/PAs do identical jobs to doctors, inc GP and middle grade positions. The GMC approach of letting employers determine scope will obviously lead to PAs replacing many doctors.

I find it bizarre people have to prove that PAs have caused harm before there is acceptance their role needs serious reconsidering. We don’t say let’s wait for evidence about car crashes when deciding if people need to pass a driving test or not before getting behind a wheel.

@UMAPsUK Pretending PAs are the sames as doctors!!! PAs only require 2:2 in BSc that has not guarantee on contributing to required knowledge, PA national has very easy 92% pass rate yet to get to FY1 one must excel at school and pass much harder/extensive training/exams.

@DrPastry Staff introducing themselves correctly (as PA or doctor etc) makes little difference to outcomes. Patients aren’t equipped to understand what their different roles entail. I shouldn’t have to ask if the person flying the plane is a trained pilot or not.

@DeanEggitt I beg to differ. It will exist but run for profit by multinationals. This will further erode patient care and the pay/working conditions of the few GPs still within the system. Where continuity of care ends and rationing of care begins. Basic NHS ‘care’ unless one goes private.

@DrNeenaJha It’s also about the privatisation of the NHS. The recent Health Social Care Acts, ICSs/PCNs are to restructure the NHS to allow greater corporate involvement. HCRG have taken over dozens of practices. Central to this is a cheaper more malleable workforce. Hence PAs replacing GPs.

@quackophage @TomStocks1982 FY1 would usually score very highly at school and pass 5 hard MBChB exams. PA entry 2:2-2:1 BSc, PA national exam 93% pass rate. Don’t doubt many excellent PAs but FY1 have considerably harder route. Once post grad medical exams added the difference is huge.

This proves PAs are replacing doctors ⬇️ On the day/urgent care is a core part of a GPs role. If it is being taken away so you can’t see your own GP and done by a hub full of PAs, it means PAs are replacing doctors. I can remember this being on any manifesto!

As a consultant oncologist this is hugely worrying. A 🧵. Somehow the UK is elevating PAs (w/ 2 years basic training) to a level expected of a doctor with 5-10 far more rigorous years of training and exams.

It was so obvious what was coming down the line but GPs, by in large didn’t care, and some so it as an opportunity to profit.

@Dr_BellaR Typical sessional rate is £9,000-12,000. Sessions should be 4hrs 10, but more like 5. Work load considerable so most doing 6 sessions = roughly £60k. 100k for 10 session but burnout central.
