I'm watching a community midwife in tears on Channel 4 News using a foodbank. A BSc-level NHS midwife delivering babies out of hospital...using CHARITY to feed her kids. The NHS staffing and pay crisis is now also a moral one. Shame on the Conservatives.

I was just woken by an F1 friend in tears. Only surgical F1 for 7 wards. An UGIB, fall, NEWS7, no SHO, reg in theatre, noreply consultant. She’s been on surgery for 4 weeks, had a 5 week rotation as a student 3 years ago. £14 an hour. Shame on NHSE, HEE. This isn’t training.

I'm genuinely upset that non-MBBS graduates can now join FY1. Yes, its a trial scheme, but this is a real erosion of the NHS doctor. There is no equivalency, a medical degree is accredited, registered, audited and rigorous. Its 110% from day one for 5-6yrs.

Weeks ago I watched a terrified Jeremy Clarkson in his barn, talking about how he feared COVID in 1st lockdown. The man who would be entirely helped, free by need, by the NHS. This man TODAY wrote in the Times for the abolition of the NHS. Mate, there's no BUPA ICU. 🤷‍♂️

I've been a doctor for 14 months. Ive had 6 jobs, across 3 towns, in primary care, medicine, ED, a hospice & surgery. 45-50 hours a week, 1:3 weekends, earning £14-15 per hour w £100k debt. This is a destabilising, dreadful way to run postgraduate medical education. It sucks.

I worked so hard today in a 12.5hr shift on the eve of my 30th b’day. I made a difference: broke really bad news, caught dangerous things, did procedural skills others couldn’t and taught. £14hr. Its insulting, it’s sad. The vocational, future earning, isn’t enough. I’m sad

There’s something seriously wrong when the second year medical student I’m teaching bedside to has to leave early to do an HCA shift, where her hourly rate is double mine. Her job to fund the qualification to practice medicine pays more than … the doctor she’s studying to be.

All of this is an absolute nightmare for situational factors and medical error, not to mention burn out and just generally dangerous problems. All solvable with actual investment, and real-estate, all of which would be entirely ignored if I made ONE error.

I'm a pushy F1, and I know it. On ward round, I always find myself saying "why are you making that decision ... what does that mean? why why why?" Because without this, I'd be passively writing plans with no insight. The Foundation Programme isn't a training programme.

Let’s talk self-worth & 💰 Pay for a new Dr has gone from 45k to 28k No credible politician/manager can expect such a competent, intelligent and qualified profession to accept this. It’s intentional and deliberate destruction If we don’t say ENOUGH, we will lose the NHS.

13. it took me 25 minutes to prescribe dermol 500 as I had no Doctor's Office to work in, so sat on the desk of the ward clerk 14. the ward clerk didn't like me sitting on the desk, so I kneeled on the floor - for 45 minutes, as a Doctor in 2022.

Anaesthetics: if you get a page for cannula help, pls assume 1) many tried 1A) we are embarrassed to ask 2) the patient needs it 3) I am not trying to make your life hard 4) it’s not a “favour” 5) it doesn’t need “at least a reg to ask” 6) pissed? Lobby for access teams 👍

@XelenX1 Honestly for so many of us it’s: the strikes are the last chance for Britain to keep the NHS. Pay us what many others are trying to pay us abroad, or get a BUPA subscription. We are done being non-competitively remunerated for skilled work. It’s time the UK saw this.

So after two rounds of strikes and 13 years of pay cuts, the government offer FY1s 70p an hour. As someone who just worked 39hr on-call over Fri-Mon with ONE rest day, not even a weekend, before being back to the wards I'm torn. Do I strike or do I just vow to leave the NHS?

I am on the pass list of my MBChB. I’m Dr. Mercer. Graduation in 2 weeks. I couldn’t want to work for the NHS less. It is so abusive & toxic. Annual pay cuts, “training” that is service provision & endless mandatory fees a decent employer would cover. I hate, hate it.

15. the only hospital bistro closed at 2pm, so the necessary coffee was not possible 16. I had to fax a vitamin B12 injection presciption three times to the same district nurse because she was repeatedly unhappy with what was written - each time she told me exactly what to write

I always thought I'd be a physician. Always. Even in my undergraduate degree before medical school. I'm now revising for the MSRA to do GP training with two goals in mind 1) avoid hideous IMT and 2) expedite CCT to flee the NHS. This is a red flag for the NHS' future.

The best thing about this? The salary of MPs in 2008 was £61,820. In 2023 it is £84,144. MPs believe that they are worth more than a 20k rise over the same period, and yet some (not all!) docs being paid 20k more is unconscionable.

inequity is key here. it sucks. by end of F2 I'll have done 100 days in GP at SHO level. im doing LIFT, I co-run the HTN clinic, see undifferentiated patients, provide a useful service, and even do teaching. im not allowed in GP post F2. a PA can, earning 15-20k more than me.

Years of my life will be wasted to have a job that a PA walks into, largely unqualified. My job will be paid at a lower rate. Despite that, my fees & expenses will be higher for memberships/exams/courses. I can’t see how this inequality can allow for a cohesive workforce.

It's worth pointing out that, due to significant historic pay erosion, the incoming FY1s will be at a level of a band five on Agenda for Change. This puts a junior doctor on the £28,400 pay spine level alongside a new ODP, podiatrist and ICT test analysis technician.

Unpopular opinion: intercalated bachelors degrees in medical school should get a new classification. They are <12 months of study (often September-may/June) and gain a full BSc for their students. I am a medical student, and 3-year BSc graduate. It cheapens the BSc.

This isn't an "MDT under attack", its replacing doctors. I was a renal F1, forbidden from "acting up" to SHO. As an SHO today, I cant take a locum where I choose not to prescribe/order CTs. A PA as an SHO is unprofessional. An SHO needs a medical degree, full stop NHS. No.

Doctors need Band 3 Doctor Assistant roles. A driver of burnout/slow progression for medics is the amount of service-provision we do. The NHS is holding doctors back. Nurses have adjunct staff, we need it too. UK: we don't need blood cultures/cannulas from doctors daily.

I think we need GMC, NHSE, HEE etc more readily addressing the animosity growing. Beneath the stress and frustration is sadness and, frankly, disappointment. Working so hard, taking on responsibility, and having clearly inequitable remuneration isnt acceptable in 2023.

Today I learned the NHS is a system where you can be a cardiology consultant and, due to on call commitments, qualify with 2.5 years experience in cardiology. Don’t tell me this rotational, on-call training is in OUR interest.

The fallacy the BMA are “aiming high with pay restoration” so they can negotiate down later is crap. 1) JDs can get FPR by moving 2) a 98% mandate on 77% of the franchise = anger 3) are we so fucked as a country that restoring conditions a simple decade back is greed? Really?

I left Greggs in medical school because it was affecting my grades too much. To know I could’ve applied to Pret and be on as much money as I am now for the work I do hourly at the hospital … is, and let me be clear: absolutely infuriating. I won’t forget this.

One of the saddest thing about the stealth-privitsation of the NHS in PFI (Blair) and Austerity (Cameron et al), is how the baseline of a hospital has fallen: inclusive parking, accommodation, quality of food, staff rooms, places to rest. Frankly ruined.

Anaesthetics MSRA cut off ~565+ this year. Hundreds of doctors unable to get a place whilst this government creates more anaesthesia/physician associates instead, people who have no business around patients. DISGRACEFUL. The U.K. is heading down a very dangerous path.

Don't tell me full pay restoration was unaffordable. Medics financed these pay cuts for 13 years silently, acceptingly. Now the government have spent more than the value of FPR (I'd say heading for double the value v soon) in the strikes. Its about ideology not price.

It is privilege to see medicine as a vocation. It is a highly skilled, non-competitively remunerated , job. If your income is in ANY way familial-supplemented, particularly as a JD/med school, consider your less well off colleagues who have NO shield from these pay cuts.

Frankly, as a consultant, or indeed any other type of doctor, I would feel deeply ashamed to be in a department that pays £33,000 for a post CCT IMG to be working full time. I fear this is the new trend for the NHS.

One of the saddest things about foundation has been realising IMT1-3 is basically F3, 4, 5. The teaching they have looks like ours, the ward day-to-day looks like ours, the on-calls honestly look F2 level. I just get embarrassed when an IMT2 asks if they can do a routine CRP.

Sat here 3 months into my general surgery rotation in the NHS, glancing through some typed notes I made on a gen-surg rotation 2 yr ago in med school ... where my notes covered more than the teaching I have had as a doctor. Stop calling this programme "training". Its a lie.
