First day
@UHMBT
and we’re already being called “trainee doctors”. Some attending the induction today are senior registrars 1 or 2 years away from becoming consultants.
The government are pointing out the low admin spend as a mark of ‘efficiency’- but it’s be interesting to know how much admin is done by clinicians like FY and CT doctors and by our nursing colleagues instead of providing direct care.
@ESHTNHS
It takes at least 9 years (with grad entry medicine) to become a GP who is actually a specialist in general health. But the NHS is selling the idea people with 2 years of training have everything needed to be ‘generalist specialists’
The GMC are proposing having AAs and PAs on the same register as doctors with a number in the same format. This will further act to confuse patients and other NHS staff over professional roles. Urgent action is needed now to stop this
@KeeleyMP
5 years of medical school and 2 years of FY training will result in a professional with the ability to care for patient’s physical and mental health. Leaving physical health in the hands of PAs is dangerous- psychiatrists are medical qualified for a reason.
Today I posted my ballot. After grassroots campaigning with
@Doctors_Vote
and then within the BMA as a member of UK JDC I’m so happy to have the chance to have my vote counted and stand up for our profession for full pay restoration

It’s true- here’s an example from a doctor 2 years after graduating who’s on approx £15.36 an hour. The pay scales for junior doctors are also publicly available on the BMA website.
The gaslighting of our profession by NHS England, Royal Colleges is no longer going unchecked. Our concerns have been ignored for too long and we’re now left with unregulated staff delivering an anaesthetic with minimal supervision
'An introduction to the AA role' from Lancaster University.
We have compiled excerpts from three talks by senior AAs. Names and other sources of identifiable information have been removed.
Homeopathy is based on the concept that diluting something makes it better. Looks like some have applied this to medical education replacing a 5 year degree with just 2.
How is this acceptable
@BucksHealthcare
?
Doctors are NOT worth half a Physician Associate.
FY1s, this is how they value you. Look how they value your replacements.
STRIKE.
@nandwics
If you have an unknown cause of abdominal pain, the true expert at diagnosis (or knowing who to refer to) is the GP with a MINIMUM of 10 years of trainign vs 2 years for a PA. If it’s a sudden abdominal pain with severe symptoms then an emergency doctor is the alternative expert.
It's vital that AA expansion is paused immediately until the effects of the profession on patient safety. The vast majority of anaesthetists are worried about the lack of formal scope of this unregulated role and need to be listened to.
We are calling on UHNM to immediately pause recruitment of trainee AAs, which is contrary to motions passed at the RCoA EGM and the wishes of the majority of anaesthetic doctors at UHNM.
Sign our public petition to make them rethink this decision now.
I would love to see a path for PAs to become doctors- through 4 years of grad entry medicine. It’s a shame that ambitious graduates (who would make great doctors with a proper bursary) are being mis-sold a career that leaves them under equipped to deliver medical care.
"If we want to tackle the crisis in GP numbers, we should establish a pathway for physician associates to go on to train as doctors and for newly qualified doctors to go straight into five years’ training as GPs."
Another case which demonstrates you can’t fit patients into a ‘matrix’ and expect people 5x less qualified to manage them independently. Back pain is not a simple history and expert generalists (GPs) are needed to exclude serious underlying causes in undifferentiated patients.
This is appalling.
Mid 50s, cancer red flags ignored by PAs for 8 months.
This could be your dad.
The UK is failing patients, and planning to fail many more with the dangerous long term workforce plan.
@theCFOM
@drcolinm
@gmcuk
@riaagarwal19
I’d urge you to reconsider the timing of this event- for many doctors 2 days into a new job with mandatory inductions taking place- or is that deliberate to ensure rotating doctors don’t get a voice

I think the days of having a quiet word with trust managent are over. Trusts need to be held publicly accountable for the culture of their organisation, and doctors need to decide what type of trusts they want to work in (if they can).
It’s the oldest trick in the book to gaslight doctors by asking them to “reflect”. Doctors have reflected on years of pay erosion already and will continue striking until a credible path to FPR is achieved
“I very much hope they’ve had time to reflect and they're able to come back with reasonable expectations”

Can you explain why psychiatrists can’t “ensure patients receive holistic and integrated care that addresses their physical and mental health needs” but PAs can with 3 years less training?
@UKGastroDr
This is how DATIXs should work- but when you have mandatory reflections (the doctor equivalent of writing lines in detention) and declarations at ARCP of them being a negative mark on your record, they feel like a punishment rather than a safety tool.
@NHS_HealthEdEng
Failing to lobby the government to provide adequate training posts. Helping create the anaesthetics bottleneck. Driving doctors to other countries or locum work instead of contributing to the NHS.
Last year as a locum doctor I sacrificed paid working days in order to attend meetings and ward walks, and was happy to do this for the good of the profession and my own future. I claimed only the direct expenses needed to get to meetings and get resources for campaigns

Time is running out to safeguard the definition of medical practitioner to doctors who have had rigorous training with tried and trusted content. Don’t let the government water down patient safety.

This is time critical.

It's very odd that PAs are needed when there's a workforce that's spent 4-5 years of undergraduate study then 2 years of foundation training in order to become professionals that can become involved in looking after physical and mental health. Maybe they could help?

Today, we have published new guidance on the scope of responsibilities for Medical Associate Professionals (MAPs).

I have been told constantly by people with a straight face that ARM is the ‘most democratic’ body of the BMA as it’s made up of members. Direct elections are the only way to ensure this as previous methods of deciding who attended made a mockery of democratic ideals
Well, divisions are like the BMA’s version of parliamentary constituencies.
Only, the people who are in them don’t really want you to know about them, or their meetings (think old boys’ club with their mates - in hotels and pubs, on BMA members’ money of course).

How would the public feel about someone less qualified looking after one of the most precious aspects of their health? Already medically qualified staff work alongside others with different skills in the the team. What can a PA do that a doctor can’t? They bring nothing new.

Under any level of scrutiny the pay disparity between doctors and their less qualified assistants cannot be justified. More worryingly,
@VictoriaAtkins
has no coherent plan to ensure patient safety when GP appointments are replaced with PAs.
'They are there to work alongside doctors not instead of doctors.'
Health Secretary Victoria Atkins reiterates 'regulations' on physician associates after the death of 30-year-old Emily Chesterton.
I’m often asked what the BMA did before the ballot. Before this date the BMA had written to the government to try and resolve this dispute. For many months we have had deafening silence from a government on autopilot who doesn’t seem to care about patients or NHS workers.

Illegal prescribing is a serious safety and criminal issue that should be fully investigated. Healthcare workers in any profession should understand the basic principle of if they’re a prescriber or not.
🔴 NEW: IT blunder allowed PAs at Calderdale and Huddersfield to "illegally" prescribe drugs inc opiates and sedatives
PAs prescribed oxycodone, codeine, lorazepam, diazepam and midazolam, despite being "instructed they are not legally able to prescribe”
If you were sat on the fence wondering if you owed it to your NHS employer to help them in a difficult time- it’s clear that this sentiment is not shared by management to doctors.
It’s up to all doctors to strike next week and restore respect in the profession as well as our pay
What would happen if I sent an email to “all managers” showing them any number of daily mail articles hating on managers even more than they hate on doctors 😳

for withdrawing. Consequences must result for the FPD that implemented this discriminatory policy putting staff and patients at risk. It shows serious issues with their ability to look after the well-being of the doctors they’re trusted with.
Update from Dudley Group sent out to doctors today. Full retraction of the email but an attempt to undermine the BMA LNC by seeking to negotiate with the DRC? Given the trust claimed this was policy, more appropriate to discuss with LNC imho.

Grassroots activism is getting issues facing the profession into the public discourse. Of course NHS England claiming AAs allow consultants to “spend more time with patients” is patently absurd given the nature of anaesthesia as a specialty.
