Yesterday we finally said goodbye to my father. I learned my Dad helped 💯s of ppl over the years. Life-changing and saving stuff. Ppl packed into the church. He never told us, just got on with it. In his own quiet way, an activist. I have a legacy to live up to.
I am looking forward to 2024. I hope it is a year where everyone who cares about mental health, regardless of background or belief, will work together to bring about lasting change and lasting improvement for those with mental illness. Wishing all a peaceful and happy 2024.
1/4 In 30 yrs working in psychiatry, I’ve not heard MH talked about so much…ever. Which is great. But this contrasts massively with what is actually being done centrally. Time to narrow the gap - no more inaction in the face of need.
Very sad today. RIP Dad. You taught me wisdom, kindness, scrupulous fairness. Thank you for passing on your principles and your strength, (and the work ethic…I think). I love you.

3/4 Anyhow, to help with this, I’ve decided to stand as President of the RCPsych because there is a lot to do and we need someone who can build on what’s been done and bring about lasting change.
Meeting the most important doctors in mental healthcare - the trainees. The future. They’re a fabulously bright bunch, who care passionately about patients. The future is looking good. We must nurture them (otherwise the Aussies will).

Ouch. If you’re Black, doesn’t matter what your status is; what you’re wearing; how clever or able you are. Expectation is you’re a criminal/up to no good. Historical tropes are so ingrained - structures need changing so individual unconscious bias can’t negatively impact.

Important we know all our history, the good and the bad. Too easy to disappear it. Don’t venerate but don’t destroy. People easily forget and so problems never have to be addressed. MH care has a dodgy past. Change comes from admitting past issues and resolving to do better.

CRED report takes pick and mix approach to evidence, picking the bits it likes, ignoring the bits it doesn’t. This flawed methodology leads to erroneous conclusions, particularly, it seems wrt MH. Sets MH care back 50 years. See our response.
Be aware. Important public health message - Huge ⬆️ risk of schizophrenia-like disorders in ppl who use substances, esplly w cannabis and amphets - males >> females, young >> older.
Study found that individuals with ED visits for substance use with and without psychosis were at 163 and 9.8 times the risk of developing schizophrenia compared to the general population. Cannabis was the highest-risk substance, particularly in young men.
A fish is swimming along one day when another fish comes up and says, “Hey, how’s the water?” The first fish stares back blankly at the second fish and then says “What water?” Structural discrimination (racial, sexual, gender etc). Easy to miss. We’re all immersed in it.

's group. It turns out that MH services are great at getting people out of hospital, but, I think, at the cost of helping them to get properly and meaningfully well.

urges us to move from being “interventionists to being preventionists”. Inequality is a major driver of mental illness - this is now in the curriculum, expected knowledge and understanding for psychiatrists.

100% recruitment but 10% consultant vacancies in MH. We paid £3bn to agencies for staff in 20-21. Ouch! So much cheaper to retain staff. Why not sort out the pension tax penalties, pay junior docs better & improve working conditions.

So 62% of psychiatrists eligible to vote believe SAS doctors (and other affiliates) should have the right to vote. Unfortunately, despite this majority, it is not the 2/3 required to pass. Let’s see how we can get SAS docs the vote asap. Time to right this historic wrong.

This is bad, v bad. As psychiatrists we care for the most deprived and marginalised. Austerity creates deprivation. Deprivation associated with MI. Psych leaders must stand up for the poorest and most discriminated against. No Quality without Equality.
We’ve hit 100% recruitment in psychiatry, but we are losing senior psychs for the same reasons cited for GPs.

1/3 More women now excel as doctors and psychiatrists, but I’m concerned about the ongoing gender pay gap, stagnant career progression for many and sexual harassment at work.
