Things I learnt from helping out at my wife’s GP practice Covid clinic yesterday 1. How enthusiastuc the staff were even though they are knackered 2. How every patient was known to them 3. How respectful the patients were and thankful for the team working extra to deliver this.

GP wife’s first day as a locum yesterday after close to 30 years as a partner. Missed not knowing patients but the lack of running around making things work was a positive. Made her reflect outside of the bubble how much work partners do in General Practice.

So this is a thread about GP finance and why funding issues lead to a crisis. The final wrap up payment to Central Lakes included a £12k deduction for overpayments. A quick twitter check & it seems that all local practices had deductions recently - the highest response was £26k

Just updating bookkeeping for wife’s GP practice . Queried why the oxygen bill was so high ... they need more supplies as ambulance delays mean they need it in surgery as a back up (and use it). No one is funding this though but the GP partners ... that can’t be right

@Keir_Starmer ⁩ . My wife’s former practice was a GP partnership. It provided a good service. They gave up their contract solely because of funding cuts. The new salaried service is unlikely to provide the continuity patients desire. (1)

27 years of advising GP practices - yesterday was a low point. I simply don’t get how you can run a health service which is based on short term funding which commissioners can remove at a whim without consideration of the risks on services. The ICS world looks to excacerbate this

I think you would have retained my GP spouse longer if you’d actually funded her GP practice properly so they could carry on providing a high quality service. But you didn’t so another one lost to the system along with one of her partners. Retention means also resourcing properly

I think Mrs Ps final days of being a GP partner may have peaked too early. I am not sure she will beat yesterdays visit by the health and safety people who asked where the risk assessment form was for the coffee cup washing up liquid at Hawkshead ! She was busy with patients

Just before Xmas notched up my 25th year advising GP practices. Never in that period have I seen the sector under as much pressure as it is now but equally never have I seen the will so high to get things done. The general public won’t see all the effort .. but it’s immense

Annual allowance is a far bigger problem for doctors in impacting work patterns than the lifetime allowance. I’m not sure anyone stops working solely because of the lifetime allowance

Raising the lifetime allowance would be beneficial to NHS doctors and managers retiring early or - reducing their hours due - to worries about breaching the LTA and incurring tax bills.

Surgery phone diverted to mobile so staff can make it home in the light, lunch being eaten at 6pm, staff who were not stranded worked flat out , general practice carries on remotely , let’s hope the rain eases off now.

I do wonder if my spouse is a GP or whether she’s been living a double life for years. Whenever I get hypochondriac issues like today she is very dismissive. Paracetomol , get on with it etc. Wouldn’t even see me on video to discuss 😂

For a senior politician to say that NHS pensions are paid tax free it just shows a big lack of understanding of everything to do with the remuneration package. The NHS pension is taxed in the normal way.

10/ As to the "tax free" bit, get real! Of course pensions are not "tax free". Neither have they been built up tax free - consultants retiring now with a pension anywhere near your figure will have paid a *LOAD* of Annual Allowance Tax

Walking around Sawrey today we didn’t follow maps. My wife simply navigated us by the houses she had visited as a GP with fond stories of the people who lived there. To be rural GP you need to be part of the community - with knowledge passed down to the next generation

So what would help General practice that’s non medical; 1. Change student loan payments - excessively high for GP partners due to pension rules. 2. Reform or abolish annual allowance pension tax - its stopping people working. 3. Change the 60% rate of tax that applied from £100k

What they forget is the massive reduction of experienced people at the moment . It’s not just a case of do some exams and away you go. You need experienced GPs to mentor the next generation … start working on retention urgently

So today is Kaye’s last day as the @HawksheadGP . It’s had its challenges but her time in the village has been the most rewarding & enjoyable part of her career. Sadly she leaves not as Chris or Jane did but will go quietly at lunchtime & will cease being a GP partner tomorrow.

We’re making real progress in getting patients to see their GP quicker. ✅ Almost 2 million more appointments were delivered this February vs. February last year. 🗓️ That’s 100,000 more appointments each working day. 📈 With more GPs and primary care staff to come.

This needs changing and rapidly . ICB’s need to understand funding quickly and the impact cuts have . NHSE needs to understand that without practices you don’t have PCNs … not the other way round. Core funding needs to be moved from PCNs to practices - now without conditions

Assuming 5 day working week then by my maths over 10% of the country has an appointment EVERY WEEK. Why is no one asking why we are so sick as a nation ? Or is the GP system now the only go to for multiple problems in society ?

Anyone who has seen my tweets knows my wife’s partners sadly handed back their contract in Dec 22 as the ICB decided to cut atypical local funding. They were already on the lower earning spectrum for GP’s . On reflection the ICB probably did them a favour

Thanks to the Daily Mail for highlighting that my wife’s practice saw close to half its population last month in appointments. And showing the contribution of nurses , physios & others. It failed to mention that & that they have handed back their contract due to cuts in funding

My wife handed back her contract as that could not afford to run the service with local funding cuts. National imposition of a second year of funding which will leave partners in negative profit positions will lead to the ultimate industrial action - widespread contract handbacks

Heard that the PM of a practice I had close connections to has had to spend a lot of time recently explaining that “no you can’t have an appointment with your preferred GP as they no longer work here”. Didn’t work though as patient insistent because @Keir_Starmer said they could!

If the NHS thinks some practices are not performing deal with those direct .. not by causing the rest of the service which is performing to jump through more hoops, have more unproductive meetings . Not by supporting a view that the service is not open

So next time you see your GP please do thank them for funding the NHS out of their pay. But don’t be suprised if soon they are no longer there. Do remember that staff are mainly low paid when you go to reception. But if there is no one there don’t be suprised.

So in summary your GP gets an average core funding of £95 per year to ; 1. Provide unrestricted access 2. Pay for all the staff 3. Pay for all the premises running costs 4. Pay for everything else you see And the rest they get to keep as there pay. Bonkers isn’t it

So my day in a small general practice with GP spouse ... arrive soon after 7.30. Emails, results. Phone constant for Julie on reception. Face to face arrivals . Break for lunch 1.25. Lunch ends 1.30. Off to visit in nearby village . Back 2.15 for the afternoon (1/2)

I just don’t get why NHSE cannot recognise that practice costs are every month. You cannot therefore fund a service just by putting more and more lump sum “reward” payments at the end of the year which come with more conditions. It needs investment up front.

GPs reporting reduction in vaccination supply and now things like this appear. Hope we don’t do the British thing of snatching defeat from the jaws of victory. GPs doing an excellent job - if they got regular planned supplies like the mass centres they could continue to !

I was told last night that we have over 4000 people on housing waiting lists in the Central Lakes area. Yet many properties stand empty and unused for much of the year. Residential housing should not be a play thing for the few.

It’s also poor economics because well run local practices reduce costs - they reduce admissions to hospital - they reduce the risk of safeguarding issues - they reduce the risk of a poor outcome for a patients as knowledge and knowing people is important

So when you can’t get appointments or find that your local practice you have built a long standing relationship with closes please realise that it’s the funding system which is leading to this . Smaller practices feel the pain hardest & they are often the ones most connected

Midguided policies led to the GP practice which has good patient centred care in the central lakes handing back its contract. Learn from that as well @wesstreeting Reform needed but not this way. Invest not change a system that with investment worked before

This was why my wife’s practice handed back her contract when the ICB removed local funding . The funding per patient meant it was insufficient to cover the costs of running the service once you’d taken into account another years wage rises without funding

I work as a full time GP caring for 1500 patients It's very busy as we're in a deprived area with a large elderly skew Global sum payment last month didn't cover the bills & wages, so I couldn't take a salary Before anyone signs up to become a GP, think again, it's unsustainable

NHS execs don’t pay for the NHS out of their pocket, nor do consultants , nor do politicians . In the business world if demand goes up it generally means you bring more income in so I would not have this issue. So why are we doing this to GPs. It’s poor commissioning.

We have an increasing problem with commissioning of GP services which go out to tender. The process is a tick box exercise which gets scored more around compliance than service delivery. When was the last time a GP contract got awarded in an open tender process to a small team ?

Thanks @timfarron for highlighting this. The resource uplift for 24/25 has been set at 1.9% since this report was written. Many staff now employed are at minimum or close to min wage levels so that’s unlikey to cover the cost rise from wages alone. 3rd year running for this

Morecambe Bay is a big place ! Been on a magical mystery tour of surgeries delivering face shields today as has my son. Quite a logistical exercise. All practices should have a small supply by Monday and hot hubs larger supplies. Thanks to @MFAminGP for sourcing.

So much good happened today in my wife’s GP practice -the daughters of a patient now deceased who dropped in to say thanks as their dad didn’t have a chance to, calling someone who posted something on Twitter about the surgery to check they were ok. Which is why we don’t give up

So when you complain that receptionists are awful - remember they are low paid despite having skills which may save your or my life one day with a quick decision & are doing their job to try and manage a demand that in many cases can’t be met.

In real terms this equates to my family having to pay the NHS back £2.5k for the pleasure of my wife working her backside off for that year before regrettably handing back her contract. It means people give up a job they loved in communities they loved working for

Afternoon .. staff shortage so no reception. Phones covered by other site. GP answers door, sees patients , phone calls . Leave 6.10 to go back to main surgery to pick things up for next day and catch up with team. And people say general practice is closed ... it’s 100% not (2/2)

New GP partners routinely being quoted 9%+ interest rates for unsecured practice buy-ins at the moment. Another big issue in attracting people into partnership at the moment. And proving challenging financing retired partners exits.

Really poor that GP contract terms for 22/23 are only released on 31/3/22. No covering of additional costs to practices of Employer NI increases, rise in core funding not matching inflation ... and all the emphasis on PCNs. GP partners bearing the cost
