Please can medical schools not exclude fy1s from signing off basic procedures but allow nurses / Pas / other staff to who may be less experienced. It's divisive and infantilising. If you think they aren't doing it properly that's a massive probity issue. But presuming they won't?

Disgracefully your case examiners dismissed the case as a legitimate difference of opinion, an outrageous face-saving view rather than realise I had been put through an unnecessary but highly stressful investigation and apologised. Apologies are only for doctors, not the GMC..

Every email was confrontational and distressing, every deadline by the GMC was missed. The impact your appallingly organised and poor quality investigations have cannot be overstated.

My worry is that possibly the default quality of GMC investigations is very bad. What we hear may be the tip of the iceberg. Most who go through this make no fuss possibly because they are too exhausted or worried to. But this process takes doctors lives. We need reform.

As things stand given the quality I saw then if this represents the usual quality of work of our regulator then the GMC is not fit for purpose. It also has no accountability and no interest in reflecting and improving. Feel free to get in touch.

I have complained as the quality was astonishing. No apology, no engagement with the issues. As it stands the GMC doesn't appear to understand clinical reasoning, evidence based medicine or how to do a basic pubmed search.

Update: Firstly noone, including @drcolinm at the GMC is interested in hearing my views. Presumably they feel they have nothing to learn about the investigation process. Secondly this whole thing has been incredibly enlightening, damning and utterly depressing.

This reviewer said my practice was far below expected. The review made basic errors of maths, showed a medical student level of clinical reasoning and advocated non evidence based dangerous actions at odds with the evidence and consensus

I didn't expect this to get this much traction. Legitimately you can argue why make the fuss. Nothing came of it. I suffered less than 1% of the awfulness of the recent well publicised GMC cases they got wrong.

Post fy2 unemployment a thing. This is going to worsen inequality in medicine. People from family money can tread water or even do a paid for masters to CV buff. Excellent recent fy2 asked me for reference for a dead end medical job. They would have made an excellent cons.

Out of hours plumber £80-£200/hr Out of hours mechanic £80-£200/hr Out of hours electrician £80-£200/hr But Sunak thinks these junior doctors with £100k of debt deserve only £22/hr for extra locum shifts after working a 48 hour week paid at £13/hr. No wonder they're striking.

Because inevitably people will point out that they get paid less for their regular in hours job.

@dr_lungs One legendary northwick med reg, now resp consultant was once told doctors couldn't have tea off the trolley during med reg knights. He downed tools (and got team to down tools) until gold command was woken at 2am to approve. Tea for med regs was never limited again.

What next for me: I will continue until I am heard on their clinical and diagnostic reasoning issues and absence of support for experts I am now complaining via my MP I am lucky to be from a family of lawyers, privileged and unafraid of shining a light on the GMC.

Surgery is totally over subscribed, a million doctors want to do it, consultants are limited by theatre space and don't get to do enough cases. What is this supposed to be a solution to?

Chatting to a colleague who's partner killed themself following a (nonsense) GMC investigation (the suicide was after the investigation so doesn't make it into the figures) one thing appears clear to me: The GMC needs external review and change imposed on it. Enough is enough.

@CampaignNhs @drcolinm @GMC I don't want to reduce access for patients to complain. You're right the main issue here is the timeframes. The adversarial nature and quality of investigation also a big issue. Noone wants to limit patient's voices.

@drcolinm @GMC @gmcuk Thanks. I did try to feedback and contact a clinician but never got anywhere. I look forward to the response. It would be great if GMC getting feedback was routine for those going through this process. It could be a lot better in a lot of ways, many very simple.

I have been contacted by a number of people who have been through terrible experiences with the GMC. The themes are consistent. The main issue is terrible timelines and uncertainty with months and years that your life is on hold.

@Dr_BellaR It was easy to make decisions in the 1970s when the treatment for Stroke and MI was bed rest. Medicine is immeasurably more complex, being an F1 is far harder now than when I was.

Lots on an essential presumption of guilt with accusatory and totally unempathetic communications. Many share my anger that when a case is dismissed and there is no case to answer the GMC seems to throw in some final barbs about still thinking in what why you can improve!

As for why too few people speak up There is profound fear that the GMC is a vindictive organisation. This chimes with my MDU lawyer urging me not to complain as the GMC might make things worse for me. Also people are mentally destroyed by these processes and have nothing left.

@ronansetron My recent Aston students. Would you like to go to a haem clinic or anything else you would like me to organise? Nope we have to get sc injection, ECG, multiple GMC mandated sign offs done and learning actual medicine has been relegated. (They were a great group too..)

@KateAndrs Kate the facts are that the pay has gone down by 16% CPI when other professions haven't, debt is enormous and they could be paid better elsewhere. The antagonism towards doctors in this country is nothing like anywhere else. If I was 25 I'd leave for good immediately.

@AliJaneMoore Consultant should have worked the shift, sorry. I had to do this recently. If you can't do this you probably shouldn't be on the OOH rota. If you are a DGH consultant at some point you have to come in and do this, it is impossible to mitigate this entirely.

@Xeon4f145d96s1 Part of the problem is rotational appointments. ACPs stay longer, understand the system so it is easy (not a good thing) to see how trainees get overlooked. We need doctors staying in the same place far longer. Better for life and training.

Delighted to see a lady who I diagnosed with wegeners 3 years ago with a fairly atypical presentation. She didn't remember me, she did however remember the genius rheumatologist who apparently diagnosed her. I am so vexed.

The PA project has been incredibly badly put together. The absence of regulatory body, absence of centrally determined scope has been a disaster. The rapid push to increase numbers without a clear definition of what they can do has been poor.

We need to increase training posts, likely reduce med school output, insist on realistic info given to all IMGs so they are not (more) exploited. But we need to be collecting data on outcomes by demographic etc. We could really alienate this generation. I thought mtas was bad!.
