The year before I applied for HST there were 9 jobs in the region. The year I applied there was just 1. I had just bought a house. I was devastated, sure we had just made the biggest financial mistake. People pushing MAP expansion have no idea what trials trainees go through.

@DrEilidhMaria @jim_crawfurd This for me is the problem with PAs. The medical hierarchy exists for a reason. I’ve been an SPR for >5 years but I still welcome supervision and scrutiny of my decisions. For them it’s all of the autonomy none of the accountability. Just dangerous lack of insight.

@orlasaysthings My 1st week as an f1 I got bleeped by the urology nurses 10x a day saying mr x has got xxmls in his bladder after a TWOC should we recatheterise him? I said I don’t know, in your experience what would you normally do. They said I dont know, you’re the doctor. 1/2

@telswood @medicalmodelbri It is not acceptable for anyone- med student, junior doctor, senior doctor or PA to give a 5 minute run down of the medical histories and management plans of every person in their care. It breaches GMC guidance about social media use and NHS confidentiality rules

@Dr_Done_ This stuff is so tiring. No a PA hasn’t got “far more clinical experience” after a few years. They have done far fewer hours! They just appear knowledgable in department because they haven’t been forced to rotate every 4 months.

@walsh_joe @timricketts_ @HajaraPa I very often do by the f1/f2! having done acute med, cardio+ gerries foundation jobs I DO feel confident to do these things. You cannot be a Reg only trained in your lane or you will miss important things! The patient is a whole, not a single condition/organ

@JaneEMCallaghan I dunno, I’m a woman, regularly get called a nurse despite introducing myself as a surgeon and very close to getting a PhD on top of my medical degree…But I still think the context of using the PhD “dr” AND consultant (without nurse/physio qualifier) as your title is confusing.

Dr Peter Hilton from Haverfordwest says that workplace sexual assault/harassment is FINE ACTUALLY. Very telling that you feel comfortable putting that to paper with your name, location and exact years of work. Shameful.

Response in the Times to the revelation that many female surgeons have been sexually assaulted by their fellow surgeons, including *while operating*. This is probably a TINY minority of male doctors but they are horrific creeps.

Really?? Boring specialty bashing in 2021🙄 my patients get seen every day by a senior decision maker including weekends. My medical SHO jobs were the worst for all of these points, probably why I’m a surgeon now tbh.

@reachimyq 4yrs undergrad + 4yrs medschool means graduating at minimum 26- you spend all your late twenties being pushed around the country on 4 month rotas preventing many from buying homes/having children, disadvantaging women and saddling you with 72k debt for nebulous “life experience”

@LeanneHPatrick ludicrous! If your role as an ACP is to fulfil the role of a doctor then doctors get to decide how autonomous you are. We are asked to supervise and act as a guarantor for your level of skill. You don’t just get to go on an unsupervised rampage. People’s lives are at stake here

@cannula_service I spend a lot of time as a general surgeon explaining that hepatitis is a thing and how to interpret lfts to a lot of EM people for whom RUQ+ any “red” on lfts= cholecystitis. I feel like this is a big reason why. You don’t know what you don’t know.

@nursie6 @timricketts_ This already happens. At UoN we shadowed a nurse for a shift on every rotation - medicine, surgery, paeds, psych, you name it. We also shadowed physio, midwives etc. Never seen any profession shadow an f1 though …

Things in our office that just make sense…Unless you are a doctor, then you don’t have an office, you have to battle for a computer just to do your job and a snack is the only meal of your day that you eat as you do more jobs…

Instead they get shafted with ward round, ward cover and low complexity clerking. Stop giving our training cases away!!!!!!!

@Xeon4f145d96s1 I have to defend Jodie here, when I worked there the SCPs did lumps lists of lipomas+ sebaceous cysts. They didn’t take any opportunities away. They were excellent first assistants, allowing the consultant to step away and the SPRs do hernias and lap choles independently

@dr_doddle Vividly remember “and why are you ordering a CT angio on a ?ischaemic limb and not an MR angio” “because I didn’t think you would do an MR at 5pm on a Friday. Can I have an MR instead in that case?” “Uhhh no” “okay so CT angio it is then” 😇

@Dr_Sarah_Bellum @cannula_service Exactly. When everything is drilled down to protocolised basics, who is going to be left to identify the zebras from the horses? They can and do come up. Secondary care has to be able to deal with it.

@rbarbosa91 @SurgeryBro The appendicolith more than doubles the rate of non-op failure ( @covidharem ). So not ideal candidate for abx management….

@gregcjones @Xeon4f145d96s1 If you can be appropriately trained without med school, SJT, foundation years, core, st3-8 (+fellowship), annual ARCPs, exams, 50 WBAs a year, 2x MSFs a year, 6 monthly rotations, nights and weekends + multiple national selection processes… why did I have to do it?????

@doll_ballet @RoshanaMN Rubbish. Every job someone doesn’t want to take any risk for ends up at the door of the f1. From their very first day. Classic example pt need ct, patient only has central line, whole ward of nurses trained in central lines, no one will go to ct to give contrast even if f1 there

@DrBenLovell Dayteam were late joining PTWR once so I suggested we stop to cons and check if they were here yet. They were all just sat in office, hadn’t realise we were going round. Consultant made a big deal of “Hannah stopped the round to find you, she’s SUPER EAGER to go home.” 🤦‍♀️

@jim_crawfurd @DrEilidhMaria If there are only 3000 in the country then what proportion behaving like this are we calling acceptable? Because we have hit dozens of examples now. That’s an unacceptably high rate of worrying ones in my opinion.

@BestOrthoDr @BMA_JuniorDocs 🤦‍♀️ They aren’t being “woken up” to prescribe paracetamol. They are awake in SAU/MAU/ED slogging through clerking 15+ patients a night, making clinical decisions fatigued. No one gets to sleep through nightshifts anymore.

@acdebeaux But is it the right thing to do? Does the patient want a 40 day hospital stay with multiple complications that results them being moved out of their own home into a 24 hour care nursing home? That is the question.

@PonziniF I’m a 5’2” lefty, never been a problem so far! Be extra nice to the scrub nurses and they make sure you get the best platform in theatre, got to watch out for the ones with wobbly legs and uneven feet 🪜 !!

@bluedoggobook @bad_KATtitude_ But this is the conversation we need to have. You might not be emotionally “ready to go” but if you have a cardiac arrest at 98 your body is “ready to go” and CPR is not appropriate. CPR is not a cure for dying, it buys time for us to fix your body, if there is a fixable problem
