Yet when those ✨️experienced HCPs✨️ can't cannulate, can't get the NG in, can't answer patient/family's questions, don't know what to do, need a decision made, need advice &&&&&
Who do they call for help?
No prizes.
This article uses strong language, but I think it is necessary. I'm a qualified doctor and I've been decieved multiple times recently at work, into thinking a caller is also a doctor when they are not! If i cant tell then our poor patients don't stand a chance.
I think we are calling too many things "consultant". If you need oversight from another consultant to do your consulting, by definition I think you are not a consultant.
Think of a different word instead of devaluing this particular word.

I'm a doctor in London and
@AyoCaesar
is correct, best PPE being rationed for intubations (the most risky), on ward with covid 19 positive pts we have plastic apron gloves and surgical mask only. Bare arms etc. Aren't I doing something positive too?
Pretty egregious statement here - all about regulation and supervision, NOTHING about practice of the PA. A clear sign that when PA's mess up & pts die, the supervising doctor will be blamed, even if they are not even told about the pt and PA acts way outside of competency.

TL:DR:
ReGuLaTiOn will somehow turn a two year crash course into a medical degree, and it was
#teamGP
’s fault.
This would be legit *if it went both ways*
In hospital at work - I say I'm a Dr
In hospital visiting a relative - I'm not
In an event about anaesthesia/medicine - I am
In an academic event that relates to pure science, nothing clinical - I am not
Happy to be disagreed with
If professionals can’t use their PhD “Dr” prefix in clinical settings, should we also be stopping professionals using their medical degree “Dr” prefix in academic settings? 💭

I was called in my first FY1 job to put an NG in because the acute care sister of 20 years experience couldnt do it. ??????
Everyone is out there pretending to be a doctor as if its so cool but the reality of being a UK doctor is I started at a new Trust 5 weeks ago and still have yet to recieve an actual rota. Today is one of the closest days I've ever had to thinking that this job not worth it.
"Only peripherally involved in the care of patients" ... "physiotherapist, occupational therapist, NURSE"
Is this author smoking crack? That is so incredibly rude
@Aleexxies
Genuine Q: how many lates, weekends, nights have you actually done yet? Don't write them off before you've even begun to start! Some of the most intense learning, and the most good you can do for someone happens OOH when very jr like us
@hannahmillana
To be honest probably yes since the cons has ultimate charge for care and we use many meds off license for things. Not being happy to explain why and especially threatening to report u would raise my suspicions tho and make me a lot more reluctant. Hope u have some support.

Thank you for saying that. I am very glad LAS have equipped him so well and I also hope you are right the rest of us will recieve supplies soon. All of us are one team, now more than ever. I hope you both stay well and safe.
Another non physician cosplaying with a "doctorate" from a pay for degree online course. Carefully straddling the line between what is legal and illegal to say. Why spend so much effort pretending to be something you're not? You should be ashamed to mislead patients this way.
@banktoyer
@ladyhaja
What is "it"??? Being told you are arrested by a police officer who has ID, handcuffed and placed into a car? How in the name of God are we supposed to tell an arrest by a lone cop from a kidnapping because they are literally indistinguishable.
He WAS a "real lone officer" FFS.

The Homerton is absolutely fantastic for FY1 - extremely supportive, very well run, learning opportunities, great patient pop
@DrBenLovell
Back when I was med SHO, cons decision whether to d/c home often hinged on CRP trend - So if not avail, err on side of caution and keep PT. Probably worse than just doing the CRP again and we can d/c? Basically the need for daily XYZ is often cons led not FY blood requester led
@farbandish
The most essential I guess is for diagnostic reasons - eg approach to a UTI or acute abdo pain is different based on patient's sex. & Many illnesses present more frequently in men Vs women so it changes the relative likelihood of diagnoses I'm considering. Appreciate ur reply :)
@Alexandra_nidd
Honestly I have min interest in obstetrics/paeds but I found my experience of being a med student with a new mum&dad thru the 2nd and 3rd stages of labour, (inc epidural, foetal decels, PPH...) a seriously profound experience & one that informed my education, growth& outlook ☺️
@chriscraigCCC
@ClareGerada
It might be because we just rotated through 4 other jobs where we kept being told "write for the GP to refer to my clinic/repeat UEs in 36 hours/adjust steroid dose/refer for 2ww/order an OP CT/etc" - we frequently aren't empowered to org any f/u except the above ....
I suspect most people looking at this would still assume Raj is a medically qualified doctor. He should not use the title in a workplace setting.
@drammarahughes
this isn’t going to go away.
Good points raised here. I remember being a medical student and horrified even being called "student doctor" bc I was worried it was misleading
Felt like I clarified that I am not a doctor with every waking breath. My pals who had PhDs as clinical students did the same.
Questions:
Should med students with a PhD introduce themselves as “Dr” on placement?
Should qualified dentists at med school call themselves “Dr” on placements?
Should doctors studying dentistry introduce themselves as “Dr” when seeing pts as a dental student?
If not, why?
@farbandish
If I wanted to know what pronouns to use/how to record their gender I'd ask, what is your gender identity/pronouns/etc. If I was considering diagnoses I'd want their phenotypic sex which obviously can be something other than M or F but I'd still want to know :)
@josh_salisbury
That's fair, just think it's a really irresponsible article 'University failed' for keeping confidentiality - really trivialises adult students as if they're children
@brandontillson2
@danny_mercer
I swapped "how can I help today" for "what would you like to talk about today" when I realised I couldn't ACTUALLY help for the maj of a&e attendances lol.
Or the best "why did you come to the hospital today?"
A lot of money is being wasted by these strikes. It is deeply unfortunate that similar sums will again likely be wasted before doctors are offered a fair settlement. All of this could stop today if only our pay cuts could start to be reversed. The government has the money.
Strikes have cost the NHS around £2bn so far, according to an update from NHS England today. This is through lost income from delayed operations, scans and procedures, as well as through direct costs of providing cover for striking staff. This is a cost the NHS can ill afford.
@pkonline84
@DrLindaDykes
With respect I feel quite sorry for the pt in that. I remember as surg FY1 being instructed by senior: tell the pt they can self certify for up to 7 days and see GP if still needing sick note after that. I had no idea if / how long people needed sick notes after what op...
@ParisahHussain
@MHA_92_
You're doing nothing wrong. Sorry about all the bollocks replies. My only bit of advice is when documenting discussions always document u introduced your role, to safeguard against later "the doctors never spoke to me" 🙄 you are surely doing a great job x
@LukeHurrenpfc
@HarleyShah
Yeah and when all our wards are full of covid patients and there are no nurses left to look after you after your cancer surgery what do we do? Or road traffic accident? Or burst aneurism?
@Lizreally10
@dan26wales
Hi Liz, sadly theres no curative tx for covid, only supportive care while pt fights off virus themselves or doesn't. Ceilings of care are made and reviewed regularly by experienced clinicians. Pall care (rightly) gets involved if pt looking like they may not survive. 1/2
@farbandish
Yes absolutely useful info! I think we agree. I was just confused about your point that sex gives doctors no info,which isn't the case in my practice. Its one piece of the puzzle similar to age/ethnicity - may not matter but needs to be considered as part of the whole patient
@HeyNewDoc
@admit_medics
This is so rude, I'm finishing F2 and I'm pleased when F1s ask for how to do something that I can show them!! The maj of people aren't like this. & u will ultimately have to get used to saying "nope lol I don't know how, show me first pls!"
@hannahmillana
I would definitely NOT email her or head of nursing. Write your own thoughts down in an email draft and wait to see if you hear anything - likely you won't - but threatening a jr Dr for resting head on desk for a moment is bullying & it may escalate so need to keep a record
@kittynouveau
Not that this woman isn't absolutely awful but I read that they told MIL it's OP's engagement party and MIL wore a white dress based on that

Your post made me realise I've never actually seen a nice healthy healed stoma, only formation/reversal/ problems with them. If I had a stoma I would want it to look that great! Thank you for sharing.
