I use #fentanyl daily for patients in ICU while anesthesiologists use it frequently on my patients in OR. I've touched vials, bag, & IV tubing full of it; if a vial broke, it wouldn't bother be one bit if drug touched my skin. You can't OD that way. Really. It's Not a Thing.

Myth:"Covid now only a problem for unvaccinated" Ignores fact that when hospitals are full, people die from delays in Rx heart attacks, strokes, car crashes & surgical emergencies, while "elective" operations for cancer & hip joints are cancelled. It's still everyone's problem.

Rx a large number of incarcerated individuals over my career, many required to stay cuffed entire time in hospital. Not once have I seen, treated or even heard of an amputation required for cuff injuries. This is deliberate torture, not an accidental consequence of detention.

@NxSimon Yet their absorption mechanism is. And if it worked through skin or inhalation nobody would be bothering to inject it. Which is still the dominant mechanism of nearly all proven OD from street fentanyl & derivatives, which are often taken in combination with other kinds of drugs

Dear General Public There is literally no clinical scenario where a surgeon is required to slice a person or organ into 8-20 neat, equal slices. Thus, there is no reason we should be the default pie/cake slicers at EVERY party. Sincerely, Surgeons Everywhere😄🙏

Asphyxiating someone by compressing their airway, when they do not pose an imminent threat to your own safety (ie. they are cuffed and on the ground) is murder. It needs to be prosecuted as such. #GeorgeLloyd #EricGarner #EnoughIsEnough

When HCW forget to don PPE in an emergency, or choose to skip it, that's on them; they are knowingly taking a gamble with their own health. When a respected healthcare organization ASKS them to skip it, that is reckless endangerment IMO.

Resident irritated at $13 parking fee at away-rotation hospital (always free in Lubbock) I understand hospitals in large metroplexes struggle w parking, yet I find it ethically questionable to charge employees to park, so they can do their work w creates income for the company🤷‍♀️

PSA: Surgeons routinely leave their cellphones on a desk in OR, or with the circulating nurse, while scrubbed in. This means any text received will be read (often aloud to the entire room) by their colleague. Proceed accordingly 😁 #MedEd

One dead in #midlandshooting was family of a close friend. Shot in chest at stoplight, he was still alive when she first frantically called me.Then she texted back; he had died. Heartbroken for all whose loved ones didn’t make it in this and every other tragedy ☹️😥

Screening colonoscopy for everyone 45yrs+ shown to save lives, so if you can only do 1 preventive measure, this is the one to choose Got 1st (& last for 10 years) scope today; went great, wasn't uncomfortable at all. If >45, highly recommend you do too, before Delta surge⬇️access

Thread: Last week a pt recognized me; I had cared for her husband after devastating injury. Apparently at lowest point of his ICU stay, when death was discussed, I’d told her “Even bones take weeks to heal. Why should we expect his brain to heal in less? Give it a bit of time”

Texas Medical Center in Houston is the largest medical complex in 🌎 w largest children’s hospital & cancer center; basically a small city filled w ppl in scrubs devoted solely to medical care. They are about to exceed ICU capacity (97%) because of Covid, w new cases still ⬆️

Since being Dx with Covid, I've slept a lot. Haven't exercised at all; no wine, either. My resting heart rate dropped 5points in 2 days, & Fitbit is ecstatic at this rapid ⬆️in my cardiovascular fitness. And that, ladies & gentlemen, is why association does not = causation.

As a trauma surgeon, my default definition of "not severe" is "not likely to kill you in the next 48h". Turns out, the people who said Omicron was "mild" must have been closet trauma surgeons😳 While this isn't unbearable by a long stretch, "mild" seems a bit .. inaccurate.

⬇️appetite ⬇️thirst are normal early stages of natural death. Kidney failure comes next, lungs give out last Tube-feeding/IVF does NOT make pt feel better, instead ⬆️pulmonary edema = ⬆️discomfort at the end. So pls don’t do it, & let families know why #MedEd

When hospitals exceed capacity they close to transfers & then to all new pts, incl critically ill & injured. Pts who need specialized Rx for ruptured aneurysm or car crashes can't get that Rx in time; this is how people can die because of Covid, without actually having Covid☹️

Two dear Palestinians physicians were killed over the last 2 days. Dr Saed Darabeh, a Urology resident and Prof Sereen Attar (my former OBGYN attending during med school). The loss is huge, those were civilians sleeping at home when their houses were indiscriminately bombed #Gaza

Using whipstitch as temporary abdominal closure, takeback w/in 24-48h & accepting almost no reason to leave abdomen open beyond that, we have literally closed every single open abdomen patient w/in a week, all year. Used to have 10-20each year requiring complex closure methods 1/

As a burn surgeon, I cannot imagine caring for 70 major burn pts w almost no resources. Dressing supplies are the least of it. Without ability to resuscitate & do early surgery w blood transfusion, many of these injuries will not be survivable, even in children💔😢

Spent all day shutting OR down to emergencies only, & helping pts affected. We don’t have a single #COVID19 case yet. Every 🇺🇸hospital must 1. Plan for severe supply chain disruption 2. Lock PPE to prevent theft (sadly) 3. Source alternatives now 4. Include surgeons in decisions

When I took over as Chair 5 yrs ago we had 1 Advanced Practice Provider in the dept; now we have 15. They are full, valued members of our team, involved in all activities including interviews! (Which now seems a great way to screen for team spirit in applicants😉)

@Claireific Agree completely. I often step forward & stop someone from cutting clothing off a completely stable trauma patient with no obvious major injury; the few extra seconds to remove them properly are worth it. Should never do it just because “it’s how we always do things”

When selecting a surgeon job, #1 criterion should be future colleagues. Slightly higher salary will make you happy when you sign the contract, supportive colleagues make you happy for life.

Unpopular #MedEd Opinion: While surgeons should know when NOT to operate, it is too often used to camouflage lack of competence➡️low confidence➡️try everything else before operating➡️delay necessary Rx➡️worse outcomes➡️self-fulfilling prophecy☹️ Operate when needed, expediently.

Body-weight applied solely on neck of a person lying down is NOT standard restraint technique; in fact, law enforcement professionals often taught specifically not to do this. So pls stop acting like these are accidents, & perpetrators just didn't know any better🙄 #GeorgeFloyd

Not 1 doctor I know (& I know many) is richer from Covid. President painting physicians as rich, evil money-grubbers profiting off society's woes is eerily similar to how money-lenders were scapegoated in medieval England (eg. Merchant of Venice). As odious now as it was then.

#TipsForNewDocs Never assume re:relationships When I walk in pt room, introduce myself 1st to pt& shake hands Then turn to nearest person, smile & ask “And you are her ..?” They fill in gap I say “Nice to meet you”, shake hands Repeat PRN Saves you from embarrassing faux pas!😳😬

I don't think people in our region realize the tremendous amount of strategy & effort it is taking to provide any surgical & trauma services to our community right now, with Covid filling up every hospital. Doesn't seem to occur to most that these are not guaranteed indefinitely.

Secret to surgery: go fast in the fast parts, and slow in the slow parts. The point of #surgery training is to learn which is which. Operating slowly by itself is not a virtue, since this increases anesthesia time for the patient which isn’t ideal.

Excited Sri Lanka is getting 1st Covid vaccine shipment this week! AstraZeneca, manufactured at largest site in the world: Pune India, & donated freely by our neighbor!🙏 Importantly doesn't require ultra-low freezer, which is a dealbreaker for most LMIC 🌎

Healthcare workers are used to skipping holidays, because illness doesn't take breaks. As trauma surgeons, we take turns so entire team is never off on the same holiday. All we ask is just this 1 year, the rest of 🇺🇸 does the same. You won't even have to work-just pls stay home🙏

#MedEd ProTip: when you decide on rounds to DC nasogastric tube, please take an extra 30 seconds & pull it out yourself then & there. NGT are very uncomfortable, & your patient will appreciate not having to wait ! 😊

Relaxing rules on telemedicine for #COVID19 did not cause sky to fall; patients are getting convenient, safe medical care, & no private details were broadcast on the news from HIPAA violations. Perhaps we should explore which other unnecessary rules should be done away with next?
