I am on my 16th week of COVID ICU duty.
My always working drugs are Dexamethasone & Enoxaparin.
I have no idea why are we making a hue and cry for Plasma & Remdesevir.
IIT KHARAGPUR & IISC BANGALORE both entering medical education in what is expected to better incorporate research and technology into medical education. Exciting stuff!
If your MBBS internship is limited to blood sampling, report collection & fetching food for PG's, your college is a piece of shit irrespective of what NIRF/India Today rankings say or the pride & glamour associated with it.
My story from 2018 floods is waking up stunned to long lines of Tamil lorries which had lined up Kochi's highways with supplies. Most of the drivers had signed up pro bono to help us.
I was sure we would survive any disaster with such a neighbour along our borders.
Thamizhar❤️
So I now have the whole of my youth,
Fought in blood, sweat and tears,
In Eight letters after my name.

I will stay awake to sleep your heart under the scalpel.
Our ICU maasi holds hands and sits aside of patients who are afraid to take on two hours of BIPAP.
Which medical college on earth would teach such humane gestures?
Once a Tamil colleague remembered the pre NEET MBBS admission day in Tamil Nadu where kids would even come even come in bullock carts with happy parents on the joining day.
Tells me a lot why TN will oppose NEET any day and revoke it one day.
Whenever you see a Final yr MBSS kid clueless in COVID duty,
Pls take a moment to appreciate the billionaire capitalist doctor who proposed the same idea masquerading as a socialist.
I love how people have it real easy.
MBBS+MD RADIO+OWN CT/MRI CENTRE from next year of finishing college,
All funded by a rich father and a super rich father in law.
My superspeciality resident quarters provided by the Govt of Gujarat. A 350 sq ft 1BHK apartment with free electricity and maintenance.
Probably, the only state govt to provide such a facility. Many thanks!
First MBBS medicos are either a
Anatomy person or a Physiology person.
They probably grow up as in their final year
To a Scalpel person or a Stethoscope person.
Dear Docs Shetty and Trehan,
Pass on the stage. Every Heath issue in this country is not about you.
Let the pulmonologists, community physicians & intensivists be the face of the battle that essentially is theirs.
UG medical education is now a near extinct entity for students are made to believe it as a mere bridge course to specialisation & teachers considering them as a below dignity nuisance disastrously complementing each other in most of the India's celebrated medical institutions.
Once an intern requested us to let her go home by lunch every day since Anesthesiology wasn't a very imp speciality in her opinion & she could use that time in NEET PG prep.
It was absolutely great to see her join us as a JR1 next year and even better to be her JR3 ever since.
Nobody talks about the role of a Indian govt med college nurse in making a first year resident's life worser than hell.
Half the resident's workload is making up for their terrible work ethics closed to any kind of feedback, let alone reform.
This nurse here is so skilled that he is in high demand by gastro surgeons to help with complex lap cases.
A little more privilege in birth would have made a fine surgeon out of him.
My surgeon can read ECG's ,do echos, put in an arterial line, correct electrolytes and fluid deficits.
Cardiac surgeons are the physicians amongst the surgeons.
If you are lucky, You may end up getting your asshole MD/MS senior as a junior in DM/MCh.
If you are luckier, You might even end up getting your assholer MD/MS Asst professor as a junior in DM/MCh.
If you are the luckiest, both the above happens together!!!
#TrueStory
A private dental college which used to pay BDS JR's a ₹5000 salary was stunned with the nos applying, then stopped the salary and later now charges ₹5000 for an experience certificate.
Just a mirror of affairs to be awaiting MBBS grads in 2035.
Instead of Psychiatry depts organising workshops for incoming residents to help tide over residency stress,
they should try something similar for the faculty to make the residency less stressful, may be even try consider residents as fellow human beings.
NEET PG influencers from Delhi/Mumbai have skewed ideas on which are the best med colleges for a MD/MS candidate.
Some of the finest colleges in this country arent anywhere btw the Delhi to Mumbai belt.
Can't help wonder at the grace with which USMLE aspirants tweet about not being matched & their next plans.
And random strangers encouraging and empathising with them & even offering them help.
Those who matched too celebrate it with such poise & elegance.
Lots to learn here.
A JR1 can very well be a toxic resident.
He can bully the interns, ruin work life of his co-residents and make life hell for a well meaning senior.
In my experience, NOTHING on earth can set right a toxic JR1 who decides to stay least dedicated to his/her work.
'IM'~ General medicine
'MS4'~Medico

'PGY1'~JR1

Indian medicos endorsing USMLE lingo is nothing but craving for white recognition & validation.
In a decade, almost all hospitals in the south of India will have plenty of RMO's speaking Tamil working for 25000 rs, if not less.
TN, you really need to shut down half your medical colleges, let alone starting new ones.
Most of these toppers would have been Surgery/Ortho aspirants who switched 'up' to Medicine/Radio after results for they were told it's easier & quicker money. In a country where skipping clinical classes & internship is the new norm, very few can realise their calling & passion.
Only 2 candidates out of top 100 ( AIR 65& 90) took

No one choosen orthopaedics, ENT & ophthalmology except OBGY!!
The probable reasons are👇
-illegal & legal torture to the surgeons
-Medicolegal issues
-long learning curve
-More struggle and
Dear medico influencers,
Pls do not dissuade your juniors from attending internship sincerely.
You have no idea of the crime you are doing to our profession.
🙏
The only good reason i can think for someone to join MBBS today on merit or otherwise is being an heir to a family run hospital.
It's the only thing that makes it a legit career choice in 2022.
Heyy, everyone who made past the NEET-PG,
Call up that study partner/room mate who didn't make it and talk to him.
Your celebrations aren't important than his distress!
I am sure there are consultants who haven't even noticed that a whole batch of residents is missing.
And the current ones are on the brink of breaking away.
Kerala GMCs are the nation's best kept medico secret. Even Malayalees have no idea of how its so much better here.
All thanks to strong student politics, super strict MBBS internship, accountable healthcare and moreover the democratic values Keralites value in abundance.
Dear patient realtives,
Pls do not request the COVID ICU doctor to have a second conversation with a doctor your family knows.
We just do not have the time or energy for something that serves absolutely no purpose.
TY, exhausted 👨⚕️👩⚕️
Met an 'Arjun Reddy' wannabe surgeon mankid.
Hope people realise Arjun Reddy is a template & caricature of what an absolute loser in real life would look like.
Govt of Gujarat is such a sport!❤️
Upto 40% stipend increase every three years by default for all PG/SS residents and that too with all the arrears
A final year DM/MCh resident would pocket upto 1.1lakh a month after this.
Tomorrow, I will start the 72nd and final month of my Anaesthesiology residency and I absolutely feel on top of my game.
God has been really kind to me!
Medical colleges should be ranked on the basis of how many of its fresh graduates can make out an a/c LVF from a COPD exacerbation or at least rule out an appendicitis from a renal colic
NEET PG results of interns has nothing to do with the worth of a college.
Critical care medicine is probably one speciality that can never be properly parcticed/taught in an Indian govt medical college.
It's essentially a corporate forte!
Counselling for the newly residents to handle the 'Residency stress' is a joke!
Its medical college teachers who have to undergo counseling on how to handle residents in compassion and moreover their mental health should be assessed every year mandatorily.
I just heard of new MS graduate hired by a corporate here on a no pay basis.
Not revealing the speciality, it's only a matter of time it could be yours!
The new NEET SS pattern essentially puts Anaesthesiology residents at a gross disadvantage for DM Critical care seats.
They now have to compete in Medicine group where qns would be limited to med & med SS topics with nothing from Anaesthesiology.
This follows no valid reasoning.
Absolutely ridiculous take by a young doctor.
We as doctors don’t judge our patients. As far as multiple partners are considered, thats an individual choice. As long as they understand the risks / benefits, let them do what they want.
So this ICU RMO one year post MBBS intubates a LVF+PE pt,sets up the ventilator & slips in art+venous lines. Sedates, props up and dries the pt.
And tells me that the Rt U/L pulmonary edema could be due to a eccentric MR.
He can of course do a basic TTE as well.
So this 'Delhi Mallu' guy I know meets a girl at Kannur for a prospective alliance.
She asks her politics and he says he is apolitical.
He still doesn't know why she dumped him outright!
What not to advice medicos?
Courtesy-Medico influencers
"Be a slow intern, Lok kaam dena chod denge"
No surprise here, another one from the so called premier institute of the country.
'I am joining Cardiac Anesthesia this year' because 'My husband joined Cardiac surgery last year' is my spot diagnosis of a prospective horrible junior who is going to hate herself, her life and her choice.
You complement each other in a marriage, not your speciality!
I just got my second child's delivery expenses reimbursed via health insurance from my college. Probably near unbelievable for a govt medico.
Every resident doctor in this country deserves to be provided with health insurance in this pandemic era.
Only 1% of medicos end up in a college & branch of their first choice for MD/MS.
Only 1% of that 1% end up in a college & branch of first choice for DM/MCh.
IISC Bangalore will offer MD-PhD programmes for the first time in the country.
IIT Kharagpur previously famed for offering MMST programmes now starts a med school offering MBBS seats.
Amrita Hospital Faridabad announces its medical College of 150 MBBS seats within 6 months of operations.
Would probably grow to be the finest private medical college north of Mumbai rather effortlessly.
A stethoscope around the neck and tucked in full sleeve shirts can take you almost anywhere inside a Govt medical college.
Speaks volumes of the hospital security in this country!
Indian

routinely celebrating anti reservation propaganda from a high seat of unapologetic & shameless privilege is heart breaking.
My shoutout to all medicos on reseved seats fighting this every single day. You guys are heroes!
So a snr doctor here wants the RMOs to talk to his daughter who wants to pursue MBBS.
Of course to dissuade her!
He has offered a treat if they are successful.
So Dr Rakesh Nair of Marrow now has haters?
As someone who who attended his lectures face to face at Trichur almost a decade back, i have been nothing less than a fanboy.
The interns now carry handbooks of mechanical ventilation ditching the good old Hampton ECG and Golwalla prescriber to COVID duties.
This pandemic could create a whole new breed of passionate intensivists in this country.
Kerala police salutes the intern doctors or house surgeons as known here who are passing out today after a month of extra duty amongst the Corona crisis in Aleppey medical college.
Also to show how a 24*7*365 house surgeoncy is a super sacred part of Kerala's medico culture.
