A "Consultant Practitioner" in EM @NHSGlos A vague, non-title which puts patient safety at risk by not making clear that they are not a doctor. Utter madness. #patientsafety #scopecreep @Dr_Done_ @Xeon4f145d96s1 @DrEilidhMaria

4/4 Yes PAs can intubate, under supervision in their role as AAs, but as a doctor I feel this post suggests that PAs can do these procedures independently and potentially at the detriment of training of doctors who ultimately will be the consultants expected to train PAs...

@theCFOM @drcolinm @gmcuk @riaagarwal19 @drcolinm it's unfortunate that after leaving @warwickmed you have done very little to advance medical education and advocate for those you claim to represent in your GMC role.

@jasaunders90 @MarkCheetham @Alexsahistul ...so every collaboration and supervision is from a higher speciality registrar or consultant. Jamie, this isn't true. There are countless stories of qualified doctors across the trained spectrum who are required to prescribe and work with PAs out of obligation, not desire.

@NHSE_WTE Controversial view. How about more long term thinking and provide increased training places for doctors post-graduation? More sustainable workforce with skills which actually benefit patients.

@Dr_Done_ @drcolinm @drcolinm At @warwickmed you were pretty unapproachable. Now is the time to ensure that this legacy doesn't continue. I'm sure it hasn't. Please engage with doctors regarding this. You have a moral duty in your role at the GMC.

You were lead there at the time. When your origins are a place where you haven't supported students in raising concerns without fear of reprisal, you won't have their support.

@cannula_service @DrEilidhMaria And this circles back to lack of opportunities for many trainees. If AA scope begins to extend beyond theatre, then could this reduce those opportunities (e.g. sedation)?

@cannula_service @DrEilidhMaria Any "out of theatre" Anaesthesia carries special risks and I don't think this has been fully thought through by many departments who utilise, or are considering utilising, AAs for this service.

@cannula_service @DrEilidhMaria One argument is whether AAs can contribute to other anaesthesia-related services e.g. sedation lists. However, this requires prescribing rights and still requires supervision (even if remotely).

I've been a doctor for 5 years and have taken time out to get some management experience in the NHS. I've been frustrated in clinical life but the level of frustration I have now is far greater. People wanting to make change being shot down because of burnout and apathy.

2/4 There's a reason a doctor holds the ward bleep - skills incl. prescribing & ordering imaging. Escalating to a consultant is inappropriate when their role is much wider reaching. Ultimately the patient doesn't receive as timely immediate care, potentially at their detriment.

@gaslad Having worked in a number of ICUs, the quality of training ACCPs receives varies massively. Some are able to contribute to the wider team in a very meaningful way whilst others do not add anything more than a new SHO to a unit.

@DrEilidhMaria @cannula_service Not necessarily. ODPs and Anaesthesia Practitioners have gained added skills that allow them to monitor intraoperatively and AAs have extended the skillset. The post, however, suggests PAs can learn to intubate; far beyond scope of practice.
