I am so saddened by the death of our past President Dr Cliff Mann. His passing is a huge loss to EM and the NHS. Our thoughts are with his family and Taunton colleagues. His charm, integrity and way with a simile will be remembered forever. @RCollEM

@LawrenceDunhill 1. It's really depressing that the focus is on 76%. This will inevitably focus effort on the 'quick wins' and discharged patients. Meanwhile there is deafening silence about the utterly miserable 12 hour (from time of arrival) stays.

@NuffieldTrust @DrLKVaughan 1/n Let's grab a cuppa and chew over this. First off, patients in emergency departments are experiencing very long stays, at a scale that is much worse than we've ever seen. There were more 12 hour (from decision to admit) waits in Sept 22 than ever before.

No ED wants to leave ambulance patients outside. It happens because there is nowhere for the pt. Most often due pts still in the department waiting for a bed-because not enough beds & not enough community capacity for discharges - a whole system failure - exit block @RCollEM

I have not seen the letter. NIAS waits outside ED's is a red flag symptom of a whole system in distress and this cannot be dealt with in isolation. ED's are overcrowded due to prolonged waits of patients waiting admission and not patients who are inappropriately using the ED !

This seems short sighted, pennywise and poundfoolish. I haven't seen the financial arguments and justification, but I have heard that lots of staff have found this invaluable. We recommended tailored pathway for support as part retaining staff in Jan 24.

We agree! There is a serious patient safety concern with ED crowding and exit block causing harm and ambulance offload delays which also cause direct and indirect (the unknown patient at home ) harm. This is a health system problem and needs addressing as such @RCollEM
