On our bloods request system it says you can't request a BNP unless on advice of cardio. On the cardio referral it says they want a BNP prior to referral for heart failure. I've entered an infinite loop.

Small things in my new place of work that are slightly blowing my mind: 1. Free (decent) food for doctors 2. If I have to cross cover because of absence or rota issues I’m paid extra 3. Cheap parking with loads of space 4. Specialties here seem to WANT to takeover patients

Had quite a stressful period on my nights as the only doctor on a floor where three patients needed pretty urgent attention at once. F1s/SHO/reg covering other parts of the hospital came basically straight away when I asked for help. Made me very much appreciate my colleagues.

@archeryjessica It's the way it's so anonymous as well. If I make a decision about care it goes in the notes under my name. I have no idea who makes these decisions about staffing and when I've asked the answer seems to be "someone higher than me" or "I can't tell you".

BREAKING: All hospitals in England told to take "immediate steps" to find extra space for patients so that no ambulance waits longer than 30mins. This must be done despite the extra burden on hospital staff, say NHS chiefs.

@Ken13220657 @hayleymagill @uhbtrust Doctors don’t get protected breaks and are often working all day in scrubs. Forcing them to get changed could be the difference between lunch and not. For most of them it’s no different to anyone else’s uniform (should nurses/physios/radiographers etc have to get changed?)

I started datixing every time there was completely inadequate effort put in to finding cover and in a surprise to nobody the responses were equally inadequate. Offering £18/hour for an understaffed OOH ward cover shift is not effort. There needs to be a consequence to hospitals

Patient safety is out the window here. There needs to be a protocol that mandates a trust to be responsible for minimum staffing. Too often there isn’t even an effort to find a locum/fill a gap.

@BellaRoscetti If I can pass your exams by doing passmed rather than listening to someone talk about their own publications for 45 minutes then that's what I'll do. If you're upset that that doesn't make good doctors then maybe fix your exam.

@timricketts_ Best mate from school is a trainee actuary and thinks I’m mad for complaining about pay because we earn the same p/a. He works 37.5 hours a week, no nights, no weekends, WFH, health insurance, all exams/study materials paid for, 50 study days a year, PC is off at 5pm on the dot.

Watch and then think hard about whether just anyone should be doing interviews when it comes time for doctors. You need people who know how to handle stuff like this and for the vast majority of us without media training or experience that's not us. Redirect to BMA.

Waited at handover for the night F1 and nobody showed. Since found out (from other F1s) that the rota person has known this week’s nights were not covered for MONTHS but made no effort to cover them and didn’t tell any of today’s day or night team. Great.

@msmyth7 @LiMing6859 @bicknaker The fine is not meant to be a transaction for parking like a knob. It's meant to act as a deterrent ideally, and otherwise a punishment. If you earn a million a year a £100 fine is equivalent to about £2 for an average earner.

I’ve changed my commute this week from a 45 minute drive to a 1 hour 15 min walk/train/walk and while it takes longer I think it’s been great for my well-being. 30 mins fresh air and 45 minutes to read a book rather than sitting in traffic.

There was a gap in the rota for the sho I was supposed to be escalating to as well, so special shout out to the guy who was rotad on medical receiving but spent 4 hours on my floor helping with these patients and gave phone advice for the rest of the shift. GREATIX submitted 👍

A ward in my hospital had no doctors on it today because of an annual leave mix up. A doctor who was meant to be off work offered to do it as a locum and consultant agreed, then the budgeting department said no so it was left to the on-call doctor…

1) I think a state funded free at the point of access healthcare system is (when adequately resourced) probably the best for outcomes and equality 2) I can’t see pay and conditions for staff improving meaningfully under a monopoly employer

@lifeoftazza @DrSelvarajah Couldn’t agree more! My gen med consultant last year would introduce me by name to every single patient on every ward round. He would also text his whole team at 5pm and make sure they went home on time. Both are such small things but made such a difference to the general vibe

@RadhikaThakrar My first interaction with a nurse on my first day of F1 was being shouted at by the charge nurse for taking blank paper out of the printer (to write handover notes) because doctors always break the printers. I hadn’t even tried to print anything. I felt so welcomed.

@mrthomasjacques I really think there’s some middle ground here between “juniors should not be doing this” and “juniors should do hundreds/thousands of these a year”. I think I should learn to be good at cannulas, I think I should not be used as a phlebotomist. These can both be true.

@Bossy_nurse1 @KatPaton13 It’s not the HCAs fault it wasn’t done. It is very much the HCAs fault how they responded that it wasn’t done. The pressure on beds/A+E etc etc is also not the F1s fault so don’t put that on them either and tell them they should be working for free to sort it out.

This is exactly the issue as seen from my perspective too. The times I've got to handover at the end of a long day and the night gap has not been filled. Manager has long gone home. It's me staying late and telling the night reg they've got no juniors. Make it make sense.

@juniordrtruths Obviously I’m not fully informed on the subject, but from where I’m standing it does seem like there are no repercussions for management failing to fill a gap. The staff on the ground adapt to the (often unsafe) situation, and management carry on as normal

@BellaRoscetti It’s worth flagging this up to your ES/CS. It would be one thing doing this if it was your second block after a medical rotation but it’s ridiculous to expect you to manage your first ever medical ward cover shift on a weekend without an SHO having never worked in medicine before

I had the perfect storm this weekend: 1. Just changed jobs from GP on Wednesday 2. Never done an ortho job 3. Never taken a referral before 4. On for ortho referrals for Saturday/Sunday 5. Scotland is very icy right now My reg/consultant were extremely helpful and supportive!

@NotoriousB_E_G On my third rotation last year the doctors room was converted to an office for specialist nurses and working on the ward I was constantly having questions shouted at me or worse NEWS/insulin charts just shoved in my face. Everything took longer because I couldn’t focus

If I’m going to the effort of writing a DATIX it’s something I feel is important to raise. Even if the reviewer doesn’t I’d be keen for some feedback on why. I know lots of doctors don’t do them because they think they are a waste of time, which it seems they probably are.

@HealthRegLawyer @molly_mollecule I get paid a salary based on working up to 48 hours a week. I’ve been rotad to work an average of 47 hours and 58 minutes a week. If they want me to work more they can bump me up a band and I’ll crack on.
