Yesterday I engaged in a lively debate about PAs brought about as my dept was in the spotlight. As a clinical leader who works in the “department” side of hospital management structure, rather than the “executive” side (or at least straddles the divide), it was eye opening. 1/11
I’ve chosen to mute the threads from yesterday to avoid distraction in the working week. And I wish anyone reading this far a day that is as positive as it can be. 11/11
…because now is the hardest it’s ever been, and “making a stand” and “not selling out” means watching those under my care suffer, burn out, make errors, feel lonely, and let out their emotions in offices, Zoom calls, coffee rooms and handovers, and that’s so hard to watch. 7/x
What I didn’t expect (prob should have) was to be labelled:
- Management shill
- Sell out
- Facilitator
- Part of the problem
I’m not upset, and don’t need any platitudes, but I think this could give pause for thought to those on both sides. 4/x
For those scrutinising us, I’m not asking for permission, forgiveness or even an easy ride. But I would invite you to consider that we are trying to do what we can in a system that is failing around us. This means taking up resources offered if they might help us now… 6/x

So that may make me a weak leader, because I am not strong enough to tolerate the suffering by making a stand for the greater good. But it doesn’t mean I’m intentionally pulling up the ladder, or actively endorsing any plan to replace doctors with those who aren’t. 8/x

. I have had the privilege to coach them, and learn from them, as they approach this milestone, and I have every confidence in them.
For clinical leaders making decisions now about PAs it’s interesting to think which side of history we/you will end up on. At the moment, it very much feels like being cast as the villain, which is at odds with our drivers, which for me are safety, quality, training and team. 5/x

In a specialist surgical discipline we had prob been insulated from the concerns familiar in gen med when we apptd our PA in 20/21. After yrs of rota pressure, we saw this opportunity to have someone share the workload of our stretched (but safe) SHOs, not to compete. 2/x
I’ll not rehash my points about trying to get the balance right: an evolving process. But I’ll highlight my experience on X as a point of interest. I waded in to this debate, as I generally try hard to “do the right thing”, and I felt my/our motives were being questioned. 3/x
But there are many types of leader. At the top of our profession, we need unity, clarity, direction and purpose. In this matter we may need some Churchillian robustness that we can all get behind. Hopefully the AoMRC will lead the way. And then I can continue to try to lead… 9/x

We have tried to business case before (pre Covid) for more doctors and been unsuccessful. The NHS financial situation is even worse now. I can’t have more doctors because the hospital won’t pay for them, not because I don’t want them…

It’s easy to be combative on here, and it’s a highly emotive subject. I have wondered whether there is a risk that the PA agenda is a way to build a workforce that can’t leave the UK and is being done in a cynical and spiteful way to “get back” at the thousands who’ve left. Sad.

He supports ward rounds and on call (but never on an out of hours rota). He assists in clinics and theatres. He inducts our constant stream of new doctors because he understands our systems and processes and can show them the ropes quickly…

A very, very large amount. New teaching programme. Successfully lobbying exec for return of a specialty ward. Changes to consultant job plans to increase presence. Changes to SHO rota with focus on training/burnout. Even bought them a camp bed.

2. 2mg IV midaz if restless (@ ~3h)
3. Wide field LA for VGs - look tiny but still dilate ok
5. Arterial micro not impaired by adrenaline

Be angry at clinical leaders when they put patients at risk or actively displace doctors, but don’t be angry at us for using resources we’re given to try to make things better for all. We didn’t invent the PA grade, nor do we have control over money so allocated to posts.

Naive is genuinely unfair. You may consider yourself some form of oracle, but I’m sure there are many across the country who took on PAs as I did because they felt their services were under pressure and this was a relief valve, and not in any way a replacement for doctors.

I think you and I probably work in very different systems, so I will not push back against your condescension.
A person who works differently to a doctor but can help those doctors rest, be trained, be less stressed, is in my view worthwhile. It is not a universal solution tho.

I’m sorry, but how dare you make this accusation. I took an opportunity to accept an offer of specific funding for a PA post from our Trust, and we have tried to use our PA to reduce stress and workload for our SHOs…
