Discourse on healthcare in this country is completely wrong. Everyone is arguing about how to funnel more money into a broken delivery system. Ensuring access for all means addressing the elephant in the room: monopoly, cronyism, and anemic productivity growth in care delivery.

Book Review: Where's My Flying Car? โญ๏ธโญ๏ธโญ๏ธโญ๏ธโญ๏ธ At Yale, smart students stayed away from STEM to pad grades. These people now work in law & nonprofits, & because they can't actually solve any problems (w/ science) they simply say "no" to plastic straws.

@nikitabier Parts of the economy are captured by and largely accessible to only an academic elite: law, medicine, consulting, gov, media, nonprofits. Entrepreneurship generally + tech specifically sits outside the "prestige economy". The strength of these areas diminishes that elite class.

@GavinNewsom This policy is bad, standardized tests are one of the few ways for disadvantaged students to prove themselves even if wealth can pay for better test prep. Admissions policy becomes even more arbitrary w/o them. CA despises its ambitious. Positively un-American (see Lowell HS).

AI medical scribes arenโ€™t giving my portfolio companies much contribution margin lift. We can reduce scribe-to-provider staffing ratios a bit (COGS โฌ‡๏ธ) โ€” but it turns out medical scribes do all kinds of jobs beyond basic documentation and the role canโ€™t easily be eliminated.

Legalize birthing centers! Lower cost (21% savings) + higher quality (better pt exp, fewer C-sections) than hospital birthing suites for low complexity pregnancy. 10 states have no licensure pathway & more have such complex requirements that few exist.

@razibkhan This is correct. Medical school is 50% useful and 50% elaborate hazing ritual. So if you throw out the hazing ritual, 8 years should easily go to 4. Unfortunately, it seems like competitive residencies are only inflating the time required for medical training.

A lot of hires are senior but not very good. Other hires are good but not very senior. The former are bad because they misdirect resources and spread bad memes. Seniority is more intuited than taught. But there is huge alpha in training the right people to be senior.

Try starting a nuclear companyโ€ฆ Innovation and entrepreneurship are hard enough. Policy that doesnโ€™t acknowledge the *massive* downside of good ideas never tried is bad policy.

In the US, effective monopsony for our biggest consumers of healthcare (age 65+ population) and excessive regulation slows down innovation and lets Baumol happen. In the absence of Medicare Advantage I wonder if things would be worse.

@_dsam3 Baumolโ€™s doesnโ€™t happen by accident. Why have we been able to automate away elevator operators, librarians, switchboard operators, and string quartets (via mp3) but not other service jobs? The answer is typically regulation.

It was a blunder for policymakers to trash oil & gas over the past ~2 years to drum support for clean energy. Between capital markets and supply chain constraints we can't easily add production. Anecdotally, 12-15 mo lead times currently req'd for drilling & completion supplies.

. @shohinigupta 's idea of a therapy-led primary care seems more and more prescient - you could start a continuous relationship with a therapist and eventually transition them to a PCP when having a longitudinal relationship with the PCP starts mattering

Exciting result in JAMA: Two psilocybin-assisted therapy sessions took patients from 5+ alcoholic drinks a day to ~1 w/ nearly a year follow up. Alcohol use (and many forms of substance use) can be tough to treat. Psychedelics continue to show promise.
