A thread on IV cannulation. I’m obviously biased as an anaesthetist but to me it is a core skill that many allied health professionals and doctors alike should be proficient at, and I think I were doing patients a dis-service by not trying to get better

7) another thing NOT to do: offer me a 100ml cup of water saying I’m dehydrated and this will help my veins pop up. Even if I am dehydrated - a small cup or water will do f**k all to my circulating volume, let alone my venous pressure.

1) firstly vein choice is crucial. Take your time looking at all possible sites. Start with the back of the hands, looking for nice straight veins. Do NOT default to the ACF. It’s uncomfortable for patients and ruins good veins handy for venepuncture. They can also be tortuous…

This is also a story of how broken primary care is. They are under so much strain. It’s probably quite easy to allow PA’s to reel off a load of quick consults and get the numbers under control. But it’s not safe and needs addressing urgently.

I think PA’s have a place in the NHS somewhere, but their rapid expansion is worrying. The autonomy they are given is terrifying. Straight after qualification, they seem to just be given the freedom to act like they have a wealth of experience

5) do NOT say ‘sharp scratch’. This goes for a lot of procedures - it just serves to increase the anxiety. You need to warn them obviously, but you don’t need them to be anticipating pain. I say ‘just popping the cannula in now’ or words to ther affect.

The should also absolutely NOT be allowed to be gateways to specialty referrals. I’m a doctor so know he was wrong and had the onus to follow it up. But many patients would have taken this dismissal of symptoms as meaning it’s all fine.

6) on the theme of what to say or not - do not look and say “ooh I think is going to be hard’ or ‘I’m not confident I’m going to get this’. Wording is important and patients are tuned in to a lot of your feelings and language. Phrases above are not reassuring.

During my chemotherapy 4 years ago, and more recently while having some other treatments and contrast based scans, I’ve had many people struggle to cannulate me. I’m a slim, fairly athletic build with good veins - and people should not be struggling.

2) put the tourniquet on tight. The tighter the better (within reason obviously). It can be a bit uncomfortable for the pt but it’s brief and if it avoids another attempt at cannulation, they’ll appreciate it. The fuller the vein, the easier it will be to cannulate.

With an unpredictable course. On that note - work out the direction the vein travels in so you can stay in line with it. Choose veins that have a good bounce on palpation. Avoid tortuous veins and those which are really mobile when you palpate.

3) tether the vein - use your non-dominant hand to put traction on the skin distal to the cannulation site to tether the vein and stop it moving. This is easier on hand veins - another reason to choose these veins first. Do not release the tether until the cannula is fully in…

The warm glove technique and other warming methods can help to vasodilate. However, cannulate ASAP after doing this. When I was having chemo, my they’d make me bathe my hands in warm water. But then spend >5mins preparing stuff by which point I’d cooler down again.

Two weeks pass and predictably, the lesion hasn’t gone so I make an appointment and finally get to see an actual doctor. They seem a little flabbergasted at my management so far and agree without any debate that a dermatology referral is warranted.

8) Release the tourniquet before fully removing the needle (unless taking bloods from the cannula). This will minimise mess. (This is also important during venepuncture - remove the tourniquet before removing the needle to reduce the bleeding/bruising).

The patient knows it will hurt, but the pain is (usually) brief and you don’t need to make them more tense. If they’re especially needle-phobic, have a discussion with them and ask if they want a warning or not.

It’s not just me. My wife - she had a constant chest infection and asthma flare up between weeks 12 and 29 of pregnancy. She attended the GP multiple times including the aforementioned PA on several occasions and was constantly dismissed.

4) look at the structure of a cannula. You’ll notice at the end, there is a gap between the needle point and the start of the plastic cannula. You need to ensure the plastic cannula is actually in the vein before you start advancing and withdrawing the needle…

When you get flash back, advance the whole cannula another 2mm into the vein. Then using the 1-hand technique in the video above, simultaneously advance the cannula and withdraw the needle.

Every time they would just take a generic history, and offer generic advice and treatment. No consideration of the wider picture. It was just medicine by numbers. I kept seeing the same PA and getting no further.

They clearly hadn’t read my notes, read the letters or considered anything beyond the presenting complaint in front of them. They’d discuss with their supervisor but I’d get a standard course of antibiotics. So for this (and other)reasons, I moved practise.

Fast forward to January 2024, I’ve got a new skin lesion on my forehead. I request an appointment at my new practise. My name is called in the waiting area, and it is the same PA. He’s moved practise around the same time I did!

Rewind two years when my immunodeficiency was beginning to present. Severe tonsil/sinus infections every couple of weeks. Every doctors appt I’d request- I’d get a PA. Each time I’d emphasise my background but would be treated like any other.

He takes another generic history. Clearly hasn’t read my notes/PMHx. Examines and dismissed it as nothing. Takes some photos and says they’ll get reviewed. I politely request a dermatology referral given my history. Again, I’m dismissed. No referral.

Not to divulge too much but I’ve got a primary immunodeficiency, a history of Hodgkin’s lymphoma and a previous basal cell carcinoma. So I’m a fairly high risk patient in many ways.

Later that, my GP record is updated. I’m prescribed a steroid cream and the GP writes “poor quality photos, but given history of previous BCC, come back in two weeks if steroid cream doesn’t work”.

As someone that naively assumed I wouldn’t need income/life insurance, then had lymphoma and now subsequently cannot get it (despite being in remission) - I strongly advise all my medic colleagues to look into this. The fear of recurrence and financial insecurity is very real.

Completely agree with this. Relations between cyclists and motorists feel at an all time low at the moment, and this ‘trespass’ is hardly going to help our cause. Whether the reasons for the closure are solid or not, rules do apply to all road users.

Got to have a go at CPET today and generate my very own 9 panel plot. I’m very unfit at the moment for various reasons but still reassured by the results. VO2peak 31.1ml/min/kg, AT 21.8. Estimated max power and HR achieved. Peak tidal volumes of 2.7 litres!

Fairly sure there’s a significant proportion of junior doctors who do this for many years. My current commute is 22 miles each way, in a years time it will be 35 miles. I’d love to bike but too far. Trains don’t run early enough. No other option

I will vehemently defend the NHS as much as I can, but good god - my recent experiences of the health service, particularly primary care and physician associates has been absolutely horrendous. The scope and independence of PA’s is worryingly high

Just completed the Ilkley Half Marathon. Disappointed in the time but I guess that is the effect of working in @MyiED and having to stay in until 3 AM on Saturday morning due to long waits. Anyway off to bed shortly as have volunteered for a night shift to help colleagues

Remarkably accurate. Especially when remembering the time the Dumbledore-like ex-head of school in my region who, when I was deciding whether to switch from A+E to anaesthetics, advised me - “Have patience - the wand chooses the wizard”. And he was right!

This is horrendous. When I had lymphoma, I was lucky enough to have generous sick pay the NHS offers. I can’t imagine how it feels for people in these positions now. The government simply do not care at all. Utter bastards.

So I’ve been sent a rather large tax bill, which it seems is due to different trusts ‘duplicating my allowances’ when rotating between jobs. Any tax experts on here who can advise and give an idea of what repayment plans are like. Any help much appreciated

📢 STRIKE DATES ANNOUNCEMENT📢 Junior doctors in England to take further #PayRestoration industrial action. The first strike action will take place from 7am on 20 December to 7am on 23 December. The second will take place from 7am on 3 January until 7am on 9 January.
