Actually we use this device all the time and a colleague of mine did had a case where the the back end of the catheter fall off while he was performing an LV Angio and he was filming it! Very impressive! I had to pull it back with a snare afterwards! Video follows!
@yuripridemd
Power injection done through the correct lumen, be careful not to bend the ‘tail’ of the pigtail while taking out the cath from the sterile package. Also caution while entering the LV so that the cath will not be bent at the point where the back end attaches to the shaft of cath

This is no „cosmetic“! This is a superb complete revascularization! If this is not prognostic relevant, then I don’t know what is! From no vessels to brand new vessels with all side branches preserved! Perfect job

as always! One hat to apply common logic as well

1. IVUS! 2. pay attention on how much force u have to apply to open the balloon at desired pressure and stop if it feels high. 3. Small puff after Balloon/Stent/POT 4.Balloon occlusion, ping pong, peric. Punktion if necessary 5.then papyrus.
@DrMauricioCohen
This happened to a very experienced operator, friend and colleague of mine. I was called in to retrieve the broken end. I use the dual lumen catheter quite offen und power inject through the bigger lumen but at lower pressure (800psi). It is validated until 1200psi

Yes but only for now. Check back a few years from now. You still have many years to live und what you eat comes back at you many tears from now. You can’t afford to be so mindless!
@dr_nikhilp
great save!
2 Points: 1. perfect is the enemy of good
2. Why did you have to ping pong since you already placed the pigtail in the pericardium?

I always use 800psi. I am not sure what the other operator used. We use the Acist Pump which is pre set at just over 1000 psi for the LV angio. I always make sure to turn it down a bit
@ErsaTatl
Call the surgeon, Immediate ballon inflation, pericardiocentesis, thrombin injection in the pericardium, order covered stents asap for the next cases!
