Received IDE approval for the ISOSHOCK trial. A RCT Evaluating the effect of SSO2 on infarct size in AMICS. Let’s hope we can not only improve on the short term mortality as demonstrated in

but also long term HF outcomes with unloading & SSO2

AMICS is the deadliest disease we deal with in the cath lab. STEMI mortality <2% STEMI w/ CS mortality 50% for over the past 2 decades.
There is really no good data that inotropes/vasopressers help in shock. It may be that we are hurting pts by demanding more from the heart when it suffers a large MI & is failing. Building upon Finke et al work, vasopressors associated with harm in

Euroshock trial, stopped at 35 pts. 30 day mortality occurred in 43.8% of pts in ECMO arm vs 61.1%. 1 yr mortality was 51.8% in ECMO arm vs 81.5%. Vascular & bleeding complications higher in ECMO arm. Underpowered but headed in the right direction!
My opinion on the use of MCS has been the following since
#NCSI
1. Routine use in AMI-CS (routine doesn’t mean 100%)
2. Selective use in HF-CS
3. Highly selective use in HR-PCI
4. Limited to no role in AMI w/o shock (I have never placed an MCS to “prevent” CS).
Most of us have to beg & plead for a portable c arm in our ICU, Demetri got one on a truck !!! This is the future. Mobile MCS, hospital affiliations to cannulate (with all devices not just ECMO), expertly put in, managed & explanted. Lots of discussion coming up at

And the mobile ECMO unit is ready for patients! Just like that … 3 years later post a pandemic and countless hours of planning! Which EMS will the first to join us?!!

We honestly can get AMI-CS mortality to <10% in the next 10-20 yrs. This should be our generations goal. In the same way our contemporaries have for STEMI and AS/TAVR! 100% believe this is achievable!! We have a reliable way of treating AMI-CS

The ideal MCS will be small but powerful. It will unload the LV, ⬆️ coronary perfusion, ⬆️ organ perfusion. Have low rates of hemolysis, vascular injury, bleeding and stroke. We are not there yet but we will be. (Imagine someone important saying this)

Vascular Access & Managment is the Achilles heel of large bore access. Meticulous access & careful consideration on the type & # of access sites needed is important. AMICS pt single RFA w/ 6F brite tip sheath thru 14F peelaway sheath. Antegrade 6F braided arrow sheath given PAD.
Once the Impella ECP (9F) is available commercially, our threshold for using a small bore MCS for high risk PCI will likely change. Hemodynamics in PCI matter!
Presenting at meetings, abstracts or cases, is under rated. I presented a lot in fellowship, every conference I could. I met so many ppl and made so many connections. I thank our division & program leadership for paying for us & giving us time off.
