I just realised I am in fact the emphysema of medEd. My V/Q ratio is shockingly high. I am the revision personification of over ventilated, and under perfused. When I was dx with ADHD & dyslexia I remember saying I didnt realise noone else worked so hard to be average

If I want an interface between the nursing team and the medical team - that is the nurse and a doctor. If I want some help doing bloods - that is the phlebotomist. If I want some help doing cannulas - that’s a nurse. If I want help chasing things - that’s a doctors

Honestly… I’m going to have to whip these new f1s into shape. It’s 1430, I still have one who hasn’t taken a break and the other two think I’m tapped when I point out they’re meant to take two breaks on a 13hr shift.

Once more for the people at the back…. DISCHARGE SUMMARIES ARE NOT EMERGENCIES. They can, will, and absolutely SHOULD wait until the medical team have eaten/drank some water/been to the loo and taken their contractually valid break.

@timricketts_ It’s the most important part of the role of you ask me. we are the ones who the f1s can ask “hey where do I find….” “How do I do…” - cos we never have to rotate… but we are there to support the foundation docs primarily.

@DrBenLovell They do. I am one in @MidYorkshireNHS - my job as a Doctor's Assistant is to support the junior doctors in whatever they need. I scribe ward rounds, request/take bloods, cannulate, do admin, TTOs, phonecalls, chase scans etc.

@Doc_IonaCollins We already exist. We are called Doctor’s Assistants, we are £25k cheaper than a PA, do all the non medical clinical/admin stuff (drafting TTOs, phlebotomy/cannulation, scribing, ordering tests requested by docs), and we don’t need a masters degree for it. ;-)

@DrJoFranklin @weeinstillag3l @ConkerLover @Dr_Done_ I think that F1s especially are so used to being treated like children, bossed around and having their opinions/wishes/objectives ignored in med school/by UKFPO etc and depend on being “liked” for ARCP that they don’t put their head above the parapet for being treated poorly.

My experience of the relationships we have with our foundation docs especially has been entirely positive,they’re usually incredibly glad to have our experience on systems etc&assistance with bloods.

@Dr_BellaR This is called a Doctors Assistant. We exist. We cost 30k a year less, and we don’t need a masters degree for that. ;-) We are a Band 3 role in @MidYorkshireNHS , but somehow the same role is a Band 4 in @RoyalFreeNHS 🫣

Ooooh I also totally forgot! I discovered today that Actual Fully Qualified Doctors are now so young that they never experienced Netflix as a postal DVD service. My F1 literally googled this because she did not believe that when I was in uni, the internet came in the mail!!🤣

@julesbuddle Just incase any of your other followers are interested,I got a heated electric throw blanket from Argos a few years back for £60 or so. It costs less than 70p to run for 8hrs on my tariff and keeps me TOASTY when I’m working from home.Heat the person,not the room and all that!

@DeakinSue1 As far as I know, @MidYorkshireNHS wanted to improve their performance in trainee satisfaction survey and one of their initiatives was the Doctors Assistant role,to help free up Foundation docs to actually do some medicine rather than ward jobs that don’t really require an MBBS.

@CharSquires @TharushaGunawa4 My aunt is a teacher and lives alone. She had a horrific stroke and was found by the police when her colleagues called as she hadn't arrived to work. The ambulance arrived pre 10am, that's how on the ball her colleagues were in alerting.

@medicalmodelbri I’m confused why they don’t just recruit a JCF and a Doctors Assistant for near enough the same money. Mitigate the risk, double the body count… get the continuity with the DA… and a JCF is usually a year, so that’s not exactly “rotating”

@Cleverclog67596 That’s always my goal too - I often try to start *before* handover, so I can get the lay of who might go home, what bloods didn’t get done by night team, and what way I need to try to “manage” (I use the term loosely) the cons on WR to see the simple TTOs right after high NEWS.

@DrBenLovell Sometimes the best support Juniors (F1 especially!) need is a cuppa tea on ward round and wrangling off the ward to have lunch at a sensible hour. I've threatened to physically carry one off the ward once.She was tiny,I'd have been fine, but I'm sure HR would have been unhappy.

The current med apprenticeship seems poor but the DA role, combined with part time extended time frame med school? Winner.

@DrJSherrington I did support work w/palliative care 5+ years back, and “whatever you fancy” mouth care was standard for us. I’ve seen chocolate milk, whiskey,red wine,pimms,vodka coke, the whole heap on mouth care sponges 😂 I feel like I’ve either had lectures or WR with docs who did similar

Did I pre-bolus for 2 slices of toast and a banana, as I put the baby down for his nap, and go to have a (presumed 30-40min) nap myself? Yes I did. Did he then have a 2.5hr nap causing me to wake to Dexcom having a meltdown about my 3.2⬇️⬇️? Yes indeed I did.

So I’ve been on GLP1 for 10 days now. The results are ASTOUNDING. My 7 day TIR jumped 80% to 97% whilst my average glucose is down 1.8mmol. My 7 day TITR ROCKETED from 46% to 68% -absolutely unreal. I actually need to adjust my ratios tbh, it is THAT good. #gbdoc #type1

Bloody hell, that shift was dire. Why can't people just be NICE to their colleagues? Is it really that difficult? Fed up with how some colleagues speak to my F1 colleagues that I support as a DA. These are our most junior docs, they need support, not tongue lashings.

I can’t even begin to describe what this feels like as a T1D. Insulin Pumps change lives.Ive been pumping for 1year, upgraded to HCL (omni5) 7 months ago,and it is AMAZING. I mean, those numbers. The QOL improvements. Thank you @parthaskar for all you’ve done to make it possible

So I am FINALLY getting sent to Pump Assessment Clinic with the view to getting Omnipod👍🏻the bummer is that the wait list is so long that the assessment clinic isn’t until late Dec w/the earliest possible pump start time frame is MARCH 2023.😭grateful,but a bit gutted #gbdoc

discovered today that when you have a baby, random midwives show up unannounced at your house…and get really quite perplexed when you tell them you’re at the park with baby😂can’t be sitting in all day lounging about the house,this family does ADVENTURES! 😂 #4dayspostcsection

Yeah so this is my 2 day TIR for Christmas Day and Boxing Day… @parthaskar the work you have done getting us not only flash/continuous glucose monitoring but now also hybrid closed looping is the best gift of all.

@HeedDoc @DrSelvarajah Why is it that I can work LTFT from graduation as a disabled doctor, but I can't study part time ANYWHERE in the UK? Why are we still mandating compulsory attendance at 400 person lectures that could be recorded and remote? It's elitist gatekeeping, and nothing more.

@KristaLerista Yip,stop the clock breaks at any point during viva,proof reading service(paid for by the Uni),dedicated office space(not communal) voice to text software.I feel like preemptive questions were also included for the viva but tbh, it was traumatic,I've forgotten the specifics.

@DrEilidhMaria Not a single doubt in my mind. Anyone harms my child, I’ll likely do time so the financial implications are somewhat moot. But also - if married, you’d get a fair/legal settlement anyway, especially if one had “diminished career status” due to childcare etc.

@AlanCarson15 @Dr_BellaR @MidYorkshireNHS @RoyalFreeNHS I’m looking forward to returning after Mat Leave -but I genuinely enjoy supporting the F1 docs on the ward and showing them the(non clinical)ropes,especially the poor ones who get stuck with an on call in AAU the weekend after becoming a doctor 😂I also force them to eat lunch!

@kerrymc05443501 Band 3 means our starting salary is £22,816. We are definitely not out earning Doctors (or many roles tbh, as there’s actually only one band below us) so I think you might have confused our role with another.
