I have been an NHS doctor for almost 34 years . I’ve worked hard and I continue to do so .
But I’m getting to the point where if possible I will take early retirement for my own and my family’s wellbeing . It’s a bit sh!t at the moment isn’t it ?

I am a 55 year old
#nhsconsultant
surgeon.If I do extra clinics and operating lists to help catchup I may get a 5 figure tax bill on a pension I may not live to receive - pay to work . We want to help our patients . Help us do that

Consultants doing ‘junior doctor’ work What have you learnt so far that you will try and remember when our dear colleagues come back ?
#JuniorDoctorsStrike
Consultant and SAS surgeons greeting our Junior doctors on Monday . What you do every day is amazing . Thankyou for what you do . ( and we support
#payrestoration
)
Thankyou
@NHSEngland
We won’t pay you what you deserve
We won’t provide you with the facilities tools and equipment to do your work
We won’t listen to your concerns and suggestions
And now we won’t support you when you burnout . 👏👏👏👏👏👏👏👏👏👏👏👏👏👏👏👏👏
I’ll try and remember
not to interrupt and side swipe them with tasks distracting them from what they were trying to do .
Ensure that window in the JD office gets fixed . Very drafty I didn’t know !
Make 100% sure they are getting breaks and food and drink .
@drlouisenewson
@DrEmmaNash
I’m a surgeon and I knew something wasn’t right , brain fog , anxiety , fatigue irritability , sadness . It was very debilitating . Thought it was post covid/ burnout from work . 6 weeks of HRT and I felt like my old self . We need to talk about menopause more .
@JoBrowningWroe
Still slightly pine for the soft stripy sheets and blanket combination before the continental quilt / duvet became the norm . Just looked for a picture - see they are becoming fashionable again !
Being the medical secretary /PA to a consultant takes a lot of dedication , skill , compassion , kindness I’m really going to miss Sheenagh , colleague , friend ,confidant for 20 years. Thankyou for all your support and loyalty 🙏 shout out to all med secs everywhere
Feeling a bit overwhelmed this week. Moral injury of seeing elderly patients with hip fractures lying in ED for over 24 hours . Children waiting > 1 year to see me in clinic , other fracture patients waiting for a theatre slot . Broken II in theatre delaying trauma list
I’m finding as a surgeon that the inability to see and treat elective patients in a timely fashion is creating excessive admin. Retriaging ,writing letters, chasing test results micromanaging clinic numbers and operating lists -concerned something will be missed .
Is admin taking over consultants lives ? Emails from other desk based workers ,reviewing results and writing to patients ( to minimise follow up appts) triaging and retriaging pts due to long waiting lists . Hustling for facilities and tools to do our jobs? Maybe it’s just me
Fully supportive of junior doctors strikes Mon till weds . Consultants are getting our acts together to ensure that we relearn and update ourselves re what your job currently entails each day to keep our inpatients safe . Thankyou for all you do .
@TheBMA
Excited to be interviewing candidates today for a new role at our trust . A full time Humanfactors practitioner to enhance our journey in improving ot safety and quality of care .
It’s been a long time coming but I sense a change is gonna come
@neilorpen
@trentconsultant
Too much reliance on pathways and protocols that don’t require thinking about the correct diagnosis - the carpal tunnel pathway , the stroke pathway . We need to be careful about employing specialists in pathway delivery when there are other differentials to consider .
I love orthopaedic surgery . I hate not having
Space for clinics , enough multidisciplinary colleagues to provide timely admission for surgery , Patients waiting in ED - never thought I would want to retire early .
Solidarity with those striking. It’s for everyone .
@EmergencyMedDr
Critical incidents
1. always called too late
2.where’s the map of trust in critical incident that no one wants to share ? 3.There is no cavalry arriving to save the day 4. public only notice when it affects them personally
Ward round on a pt this am when needed to check she was rousable. She responded ‘ I’m 100 years of age so I’m entitled to a lie in’ and rolled over . She was so right .
Had a busy but wonderful week this week . Now officially a Nannypod ! To our beautiful granddaughter Rosie Elizabeth After a wonderful day at our sons graduation in Manchester . Love you Georgia and Jack ❤️🎉❤️🎉
Lots of talk about uniforms for nhs staff knocking about . Reminded me of 1990 Royal Victoria infirmary Newcastle upon Tyne - check the ties and white coats ,?house officers . Check the ties long sleeves recognise anyone in there ?
@hannahmillana
Hannah . Well done for doing your best for the patient . So sorry that this happened to you . Perfectly understandable that you would need to get this off your chest . And people wonder why health workers have had enough .
Wow looks like people identify . Its mainly moral injury seeing patients not get the health service they deserve . We don’t mind working hard it’s what we do but current solutions seem to be to ask us to work harder not make it easier to help our patients .
MRI signs of scurvy in a child with dietary refusal and vitamin refusal refusing to weightbear on closer inspection petechia changes in hair and gums - now walking ! Not seen this before # ortho Twitter
Why isnt patient safety improvement more proactive rather than reactive ? Safe staffing , design of systems processes,spaces equipment ,IT, great leadership ,a culture of listening up and caring for healthcare workers ?

Delighted and honoured to have been appointed a trustee

starting in Sept . Constantly working to help heathcare workers perform at their best for their patients
Never underestimate the power of a sincere Thankyou .
The two trainees leaving who sent me lovely messages has absolutely made my week just when I was feeling a bit despondent about working in healthcare . 🙏
“Why keeping colleagues happy is at the heart of my business.” This is an article I wrote in today’s Sunday Times on how we measure happiness in the business, and why this is more important than anything else we do. Happy colleagues means happy customers!
A question for introverts - how can we extroverts minimise how much we affect you? ( I try to talk less when I’m with an introvert ) How can we tell when we are making it hard for you ?
I love trauma and orthopaedics -not loving the daily multiple varying barriers of lack of staff , equipment , poor systems and processes , IT, environment ,lack of listening and bureaucracy stopping us from doing it safely and efficientl.
#orthotwitter
We’re hearing about healthcare staff being labelled difficult when they raise a safety concern . Then leaders claiming that the investigation they pursue into that HCW is unrelated to the previous concerns they raised. It stinks.
@drcmorrison
Oh Christopher . So sorry your shift was so heartbreaking . These moments in medicine you never forget . I hope you have some debrief and peer support . Know that we are all by your side .

board members clinical and non clinical.
Remember why you do your job .
When people raise concerns or have ideas that aren’t yours , listen. They have bravely put their heads above the parapet - do not punish them . It’s the root of most nhs scandals .

I really value our ortho team. We’ve got each others backs even when times are tough -which they are . Difficult case ‘ I’ll help’ , ‘are you ok ? ‘
‘Well done ‘ .
@WestSuffolkNHS
I’m lucky to work in a cohesive diverse ortho multidisciplinary team . Despite intense external factors over which we have no control, we stick together ,look out for each other and do what we can with the facilities we are provided with . Thankyou

Heard about Burt Bacharach on my way home and my husband was playing our wedding song ‘This Guys in Love with you ‘ when I walked in . We danced and remembered what a talent he was and how he touched parts of many peoples lives . 🙏
@rogerkline
If the CQC doesn’t listen to patient safety concerns from within its own employees how can they keep the trust of those who rely on them to ensure safety standards are upheld ?

As a frontline worker trying to improve things I agree with the principles . Translating them into real improvement requires staff involvement . No one’s got the headspace or the energy . Making it easier to do our jobs is what’s needed . Staff, tools working IT etc
Passionate about bones and making it easier for healthcare workers to perform at our best by creating better systems , processes , IT , environments , tools , leadership, teamwork and communication . To provide safe good quality care .
#humanfactorsergonomics
@doctor_oxford
This govt and NHS England obviously DO NOT value doctors .
So many of us have devoted much of our lives to helping the sick, researching new treatments and preventions and easing suffering .
Treated like numbers . Not listened to . It’s insulting .
Omg just bagged this absolute bargain faux fur Karen millen jacket in a jumble sale for 3 quid . If it wasn’t sunny I’d be running around in it ALL day
I don’t know where this is but really ? My understanding is that nurses are not paid during breaks . Whether they nap or not is not anyone else’s business . Give staff adequate rest facilities.
WE DID IT! 🥳❤️
The No Hungry Staff Campaign has achieved it's vision in 1 YEAR to provide 24/7 food to NHS Staff nationally!
Thank you to everyone for your help and support and to all the NHS trusts and staff who have been working hard to provide 24/7 food!
#NoHungryStaff
For every consultant keeping patients safe as inpatients and in ED there are clinics or operating lists or patient administration / procedures not happening . The Government CHOSE this .

After 12 days on in the
#nhs
today is a self care day - lie in , face and hair mask, toenails painted , port and lemon and watching Poirot - bliss . Ignore the bunions I know I’m a foot surgeon and shouldn’t have worn those shoes 👠🤪
@bruceppdl
Yes - juniors have been saying all of these things . I’m ashamed I didn’t know about the drafty window - it’ll be interesting to see if anything gets done about it when I raise it on their behalf .
It’s been a tough 2 years but I’m so grateful for the colleagues in my hospital and the many random acts of kindness , listening , sharing through the many ups and downs we’ve had . Let’s make 2022 a good one . We can do this .
@WestSuffolkNHS
Congratulations to Professor Caroline Hing - now head of research for AO U.K. & I (MSK Trauma charity ) Welcome to the board ! and Thankyou to Will Eardley ( her pedescessor) who has done a fabulous job

I am always in awe of my Emergency dept
@WestSuffolkNHS
They’ve helped my family in our times of need , Always professional and kind . When I’m there these days seeing the ortho pts - I just want to hug them all . I don’t know how they do it .
So proud of my ortho colleague
@WestSuffolkNHS
The amazing Willem Schenk ( not on Twitter ) . It started with witnessing an accident in SriLanka and came to this . Not only a great surgeon he is at the heart of our department .

This is bizarre - why is the HR bureaucracy in the nhs so slow and unresponsive ? Your poor friend and colleague . Constantly amazed by multiple junior doctors being obstructed by archaic HR processes .
Congratulations to

for arganising an amazing conference in Liverpool this week . Wonderful to see so many talented surgeons coming through 👏👏👏👏👏
Finding the challenge of trying to calculate my pension annual allowance tax bill on top of the extra hours working for free and probably paying to work for the
