Additionally, “modern life” is pushing people into crisis through burnout etc and therefore into services and Dx where someone may not have come into contact with services previously as functionally doing okay. That’s not overDx it’s bio-social interactions.
Clinical psychology also has a major class problem, yet almost nobody is willing/wants to talk about it. I’ve struggled to find allies in pushing for better representation. Hoping to be working on something soon.

Stop criticising patients on twitter, what is it people don’t get about *confidential* - don’t care you haven’t put their name etc just stop it. It’s not their responsibility to resolve feelings of unappreciation or inadequacy, get your own support for it. End rant*
So I have now been officially awarded my Professional Doctorate in Clinical Psychology from
@UELPsychLondon
! Now for HCPC registration and off into the world of being a clinical psychologist. It’s been a long.. long.. journey, still doesn’t quite feel real!
#clinicalpsychology
Pleased to announced I shall be joining
@MaudsleyNHS
in September to help set up a brand new service, with the aim of reducing inpatient/long term residential placements for children with learning disability and autism. Exciting times ahead for CYP transforming care!
Why is clinical psychology twitter so fond of the humble brag? Usually something like “I doubted myself the whole way so can’t believe I managed to get the highest mark in class at the youngest age with a national award for super excellence and greatness” 🤔🫣😅
My one piece of advice for trainee clinical psychologists feeling the tension with/within their cohort. Remember, the playground is desperate to recreate itself and your cohort is no different, however it may be conceptualised on the surface.
My advice to DClinPsy applicants who didn’t make it to interview. Mute the hashtags over next few days, your mental health doesn’t need all the tasteless humble brags popping up on your timeline. Take time for the rest of your life, you’re good enough 🙏🏻 ✌🏼
@Shrink_at_Large
That’s quite the headline, Jay. 1) this wasn’t DBT, which is hard to get access to like many psych therapies. It was a watered down skills group in 2) a non-clinical pop (terrible idea). Should we give a 1/5 dose of amoxicillin to all kids and conclude “antibiotics are damaging”.
It’s far more complicated than this, late Dx ASD often comes about because someone happens to ask the right questions when a person is in crisis, it may seem like interpreting trauma as ASD but a proper Fx is telling where ASD and trauma intersect.
I think everyone needs to be paying attention to the amount of women who have been subjected to abuse and trauma suddenly being told they have ADHD and autism.
This is the next wave of pathologisation after BPD and EUPD, mark my words.

Social distancing is a negative term and so doesn’t give much clarity. Tell people what TO DO, don’t tell people what NOT TO DO.

Hertfordshire! Congrats guys :)
311 respondents - Herts took 31, followed by UCL and King's with 28 and UEL with 25.
32 of the courses were represented with at least 1 person.

With all the best intentions (stigma reduction etc, access to support), diagnosis and identity have become too enmeshed, and this has had negative ramifications for some at both a societal and individual level.
@cadoganhealthc1
Clinic space that doesn’t feel like a prison would be nice 😅 the CMHT I was in when a first year trainee was beyond the pale, bars on windows not the most therapeutic space 😩
We’ve done it!!! 🎉 After several years in the making, we are launching PELICAN - all web materials are free to access, a genuinely inclusive resource for young people with additional needs 😁 proud to be a co-creator and watch PELICAN fly!

Good luck to all those selected for the various random tests for DClinPsy courses! Prepare smart not hard, you can’t possibly revise everything there is to know about e.g. stats, and always trust your own (situational) judgement! 😎

Words of wisdom I wish I had heard earlier in training: don’t live and die by a theory, no matter how seductive it is to your lense of the world, or explaining “the questions”. Theories are tools, they are relevant in some contexts and not in others.
Could never imagine (clinical) psychologists taking such action, despite getting absolutely done over by AfC… could learn a thing or two from medical and rail unions.
We’re demanding full pay restoration for junior doctors in England, after a historic real terms pay cut of 22.4%.
If Govt fails to meet this we’ll begin preparations to ballot junior doctors in England for industrial action by early 2023 at the latest
Important question on mental health, response was some numbers and deferring to mental health charities!? No comment from Whitty or Vallance, sorry but that was an appalling answer to a critical issue.

Angry medical twitter with their current war on PAs are turning into areas of healthcare they have no idea about, if random non MH physicians want a battle with clinical psychologists, happy for them to learn something new. The scandal with this advert is that it’s only 8c!!!
Found out that the eminent psychodynamic psychotherapist, David H. Malan has sadly died. Many psychs would have used his triangles of conflict/person in their work, he’s also notable for involvement in ISTDP.

decision seems very unfair, I can see some of the thinking in terms of retaining staff & the whole ‘stepping stone’ stuff but surely this should apply to new applicants, so they can make an informed choice, not those who didn’t sign up for this new rule at the time
“Science can never alone tell people what they must do in response to a threat. Action always requires values. Values come from human beings, and are not given to us when we earn our doctorates.” 👏🏼 - “saints and sinners”, this pandemic has mirrored most issues of the past 5 yrs

Yes, we need pandemic preparedness, but we need something even bigger.....
Scientific preparedness
What does that mean? How did science fail?

This begs the question, why is DBT, an intervention specialised for people with severe emotional dyaregulation often in the form of self-harm, being rolled out in the form of a watered down skills group for a non-clinical population?

Interesting, lots on my timeline about Afghanistan, very little from psychologists, where psych twitter is typically very vocal indeed… what’s different? 🤷🏻♂️
Disgrace, across my career I continuously come across conditions poorer families are living in and the continuous letters of support to housing and whether anything will be actually done. Clearly, this is not an isolated case.
Be wary of therapy or “wellbeing” hot takes that start with never this or always that, life rarely deals in absolutes and these kind of things typically miss a lot of context.
Sorry folks! After a mishap on my behalf, please find THE survey all TCP/CPs want to complete! What course did you train/are training at. Twitter users only!
We need more paediatric neuropsychology competencies in CAMHS LD/ASD services. I’d like to develop this further as an individual... question is whether to go full on and do the only accredited MSc in the country (whenever I can spare the cash) or do variety of smaller courses?
It’s a sorry state of affairs that “learning disability”, a non-degenerative, non-palliative diagnosis with a huge functional and medical heterogeneity, is regarded as reasonable grounds alone, for a DNR. Winterbourne clearly never happened did it? 😔
What does this achieve for the people of Ukraine and those under Russian rule? In the world of social justice social media, is the need to “be seen to be doing something” preferable over sitting with complex and uncomfortable realities that require nuanced action?
Today, we have voted to support the expulsion of Russia from the European Federation of Psychologists' Association
@EFPA_EuroPsy
, as a demonstration of solidarity with Ukraine.

I think greater nuance and clarity is needed here. Spending time with your kids doing interesting and meaningful stuff or hovering over your kids intervening at every opportunity but never actually connecting….
@_DHOTYA
And the taxi driver turned around, said “excuse me fine gentlemen, I used negotiate million dollar contracts before I retired and took up taxi driving, let me speak to your new employer. So I passed the phone over and he got me 50% raise plus they agreed to pay for the taxi” 😎
The irony being is the ones who are least clear about their role/position are medics, expect everyone else to be crystal clear yet “I’m one of the doctors”, “I’m the consultant” is a frequent intro, meaningless even in medicine. Grade? Specialism? Only for the plebs apparently.
If you have a clinical doctorate, you have earned the right to use the prefix "Dr".
If you are working at consultant level as a HCP, you have earned the right to use the title.
Your profession should be clear, particularly to patients, but not a barrier to using earned titles.
One thing seems to be certain, that for every hospital closed for abuse of people with learning disability there is another one doing exactly the same thing. - how long is this going to go on? We’re over 10 years since Winterbourne

The concerning rise of media/celebrity “psychologists” without registration of regulatory bodies, not using a protected title but acting all the same, treading on ethical and professional thin ice. Worrying stuff. How do we act on this?

Relationship to help is often complex for patients under mental health services. The idea we can apply some very neat appointment clinical style is fraught with issues. My team sees our patients wherever they need to be seen, unsurprisingly, DNAs are vanishingly small.
- supporting autistic people and/or with a learning disability has been my career since I was 21, I was a support worker when Winterbourne hit the headlines; now I’m a Dr/CP 11 years later 🤷🏻♂️ I just don’t know what to say anymore in response to these… 😓
@Shrink_at_Large
@becomingethical
It’s not just a resource thing, the neurodevelopmental stuff seems to get seen as some discrete yet all encompassing entity rather than being intimately intertwined with the high levels of distress for a number of biopsychosocial reasons. Autistic mental health poorly understood.
Trauma and ASD can be differentials, co-occurring related issues, or co-occurring unrelated issues. Rising ASD diagnoses does need to be better understood but comparing to PDs I can’t get onboard with.
@PaulEmbery
Straight out the Osbourne/Cameron playbook, they loved banging on the about the “nation’s credit card”. Was nonsense then and is nonsense now.
A strange couple of evenings on this app. Something though is how humbled I am that there are non-psychologists out there who fully understand our training pathway, never thought I’d see nurses and AHPs confront uninformed medics about a CPs right to use their clinical title 🙏🏻
@Ms_Reeny
“I’m wondering if...” = “this is absolutely my thought and you should agree with me”
“Let’s sit with the uncertainty” = “I have no idea what to do”
😉😉

fiasco, maybe it’s worth think that it wasn’t poor families who should have been subject to moral questions and suspicion of misuse of public funds...?
Clinical Psychology twitter is an established community but sometimes a little too connected - most people I click on “followers you know” and I follow all the people they follow who follow me and them - must branch out more from the echo chamber.
@KristinaMathews
@shaylariane
After reading this I can’t help my initial thought being how an autistic child would feel being told they’re only allowed to “smile with their eyes”.
Whether DBT or psychodynamic, working in the moment of coping with suicidal behaviour or exploration of the meaning for it - no established therapy modality states “convincing” as a valid path - so it begs the question, why is this still happening then?
No suicidal person has ever been helped by minimizing their suffering or trying to convince them of meaning & purpose they do not feel. Better to join them in understanding how their life has come to feel as meaningless as it does. This means facing our own struggles with meaning

Social, cultural and of course, economic capital. Something background greatly privileges for some and greatly empoverishes for others. No consolation, but know your experience and struggles will make you a better Clin Psych, when the time comes. Solidarity bro. 👊🏻
Despite the many “new” models, the more evidence based treatments, the exciting new services - the success always hinges on that one constant, the relationship.
3. Relationships were, are, always will be key to recovery. That’s true for us all. Much more with an illness that destroys existing relationships and the ability to make new ones.
A relationship with a health worker, peer support worker or volunteer you trust? Life saving /6
As the psychological profs grow and expand, it’s evident that we need 1) a competent, well managed professional body 2) a new regulator solely for psych professionals and 3) a psychological org that doubles as our union. This is clear in medicine and nursing, we need it as well.
I've argued for a long time that there needs to be statutory regulation of all "psychologists" and therapy professionals providing services to the public, not just practitioner psychologists. However, I've started to wonder if that needs to be separate from the HCPC. 1/6
So seems like academic WC teenagers have been hit hard by this A-Level fiasco. 😔 wondering if clinical psychology’s entry is a blessing in this case by shifting competition to post-grad dr, or will these grades come to affect their chances years down the line?
#classclinpsych
I can’t imagine being blind and then experiencing the utter terror of being dragged across across a room by your ankles. 😳 Ableism, racism: what made them think they had the right to lay their hands on him.
Narrative approaches with parents of children with ASD/LD is so incredibly useful. I’ve found “adapted” (cause little is created for kids with LD 😔) storying / documenting to be some of the most meaningful work I’ve been involved in. It creates something a PBS plan never can.
So sorry to hear this shocking news, I’d got to know Ben over the past year through twitter and class clinpsy webinars, deepest condolences to his friends and family.
This is Ben’s step-dad, Brian Barron. It is with the heaviest, broken heart that I am so sorry to share the sad news that Ben passed away suddenly on the morning of Tuesday 2nd of November.

It’s widely assumed that suicidal men do not ask for help.

Some seem rather too excited about these results. DBT is a life saver for some people who are incredibly unwell who’ve been through other therapies and endless med changes AND there is no basis for a watered down version in a non-clinical pop. Maybe hold the dialectic? 😉

This is something multiple medical and nursing colleges agree with, one can be “pro-vaccine” and “anti-mandate” - the first is about evidence, the second is about ethics and politics.
