I’ve been looking in more detail at the 2023 suicide figures from ONS, published this week. They showed a 6% rise, giving us the highest national rate since 1999. In fact, the detailed picture - by age, sex & where people live - is not simple. Short 🧵 & TW

We need to see an end to that line about choosing a living daughter or dead son. It is not based on evidence. May add to distress in young people & mislead worried parents. Deeply insensitive to 200 families/yr to whom the suicide of a teenager is more than a slogan.

Children with gender distress may face bullying, isolation & family conflict, reasons to be alert to suicide risk. Empathic support is vital but evidence that puberty blockers reduce risk is weak & unreliable. Invoking suicide in this debate is mistaken & potentially harmful.

Unambiguous conclusion on risk assessment tools in today’s NICE guidance on self-harm. Don’t use them. Risk is individual, complex, dynamic & can’t be reduced to a number. Assessment should be personal, collaborative, leading to a practical plan to keep safe.

It’s become common to see an academic dropping off Twitter to escape abuse. It starts with a tweet or media appearance commenting on evidence from their field of study. Someone takes exception to their message, outrage spreads. Their timeline becomes a torrent of hostility.

We don’t always take self-harm seriously but: - 50% of those who die by suicide have previously self-harmed - suicide is up 50-fold in 12m after going to A&E with self-harm - 1 in 50 attending A&E with self-harm have died within a year (My talk in Antrim today)

We know from 1st #lockdown whose #mentalhealth we now need to focus on: young people, those who are anxious, lonely, traumatised or bereaved, at risk from alcohol or dom violence, living with mental ill-health or on wrong end of inequality due to poverty, ethnicity or stigma.

My mother was born 2 months after the Queen, near Balmoral. Left school at 14, ensured her kids went to uni. In WW2 had job photographing PoWs. Family took in child evacuee. Worked in NHS. Cared for 4 generations: 3 younger, 1 older. Lifetime of service but a different kind.

Oversensitive perhaps but could all those debating exactly what represents “political suicide” reflect that for 6000 families/yr the word is more than a useful metaphor.

Every financial crisis is also about #mentalhealth . Rising prices & rents, debts, fears over jobs & homes. Add to that the possibility of cutting public services to reduce borrowing. These figures on our screens are not just numbers, they are people’s lives.

The “out of the blue” suicide phenomenon combines 2 things with different routes to prevention: •people able to hide how low they are feeling •people who experience rapid onset of despair Prevention means tackling stigma, recognising our own vulnerability, crisis services.

Seeing a lot of criticism of the evidence informing the #CassReview . Exclusions, narrow view of methodology, etc. My impression is the opposite, the review was generous. These are vulnerable young people. We can’t lower standard of safety & effectiveness the NHS should require.

Role of algorithms in A level fiasco is no surprise to anyone concerned with risk assessment in #mentalhealth where there are constant claims that AI can outperform clinicians. Problem is it can make choices but lacks judgement.

When people say suicide must be going up in pandemic, I’ve heard of cases, it shows how far from tragic reality we were pre-pandemic. So, as reminder: 1 in 66 men dies by suicide. Think of a crowded train, a stadium, the street where you live. It has always been a vital issue.

Twice in last week I’ve been told of cruel comments by health staff to people who had self-harmed. I really believe this is unusual now but it shows there is something deep-rooted that we have to eradicate.

One of most shameful parts of miserable #Brexit saga has been seeing alarm & bewilderment of people who have made UK their home, some for generations, working in NHS, research, care homes, part of community. Talk of hard borders & backstops doesn’t cover it.

Interviewed this wk about what I’d most like to change about #mentalhealth care. My answer: the way we downplay suicide risk with judgements about mental capacity, “it’s behavioural”, etc. And the way our services reject people because of comorbidity that actually adds to risk.
