Yesterday I was discussing a referral with a medical consultant &summarising my (non-surgical) examination findings &ddx

I have been shortlisted! (Yay!)
I could potentially do the viva before the wedding (tbc) but no other days (boo)
My current options are
1) turn up in wedding dress, rush to wedding
2) decline opportunity for prestigious grant
Today was a standard elective surgery day but
- the list was built by my ES to build my surgical skills
- CT1 anaes did all the anaes bits incl spinal
- I did all 4 open hernias
- med student got to do anaes stuff & scrub in
And it was awesome! We all learned so much
Today I told a possible romantic partner they were *nearly* as much fun as a laparotomy. I will not be answering further questions or giving dating advice at this moment in time 🙈
When I was in FY2 in ED I saw a very pleasant elderly couple with a final yr med student (all of us white). Reassured, discharged, pt turned to me on the way out the door, thanked us &said the line I’ll never forget
“&it was so nice to be treated by someone white”

We cancelled 3 gen surg lists in 1 week recently. Surgeons ✅ anaesthetist ✅ pts ✅ OR ✅ but no theatre staff … because my hospital doesn’t employ enough full time theatre staff to cover without some staff doing extra shifts &people didn’t want to cover extra that week
...& we've been here before. it's not about the physical OR - it's about the pre-op, theatre staff, post-op care, + the management structures to coordinate all of this. It's a whole system, not a room. I could do an operation in my kitchen with the right infrastructure.

2021 has been a mixed bag but this is what I’m proudest of.
I finished my 1st novel
I’ve written fiction since I was young, this is my 1st big completed project & I hope not my last!
2022 is the year I try & get published. But today I’m going to celebrate!

I loved surgery, I don’t regret doing it for a minute, and I wasn’t half bad at it.
I loved the work, the teams, the patients

I’m a surgical SpR & I nearly fainted yesterday. Nothing to be ashamed of!
Experience has only taught me to eat&drink 1st, and how to spot it so you can sit down before collapsing! 😂
I eat well, I sleep well, I walk at least 10k steps/day, I have a gym habit, I have a better relationship
I’m not in pain every day from my hypermobile abused body
My stress level has dropped dramatically & it’s so obvious that people have asked me if I had Botox! 2/
“Muscular back/groin pain” = major EVAR endoleak into psoas
The only reason I spotted it as an ED FY2 was the cracking EVAR/aneurysm teaching I had from med school & as a vascular FY1!
Best way to recognise the value of medical education is to see it in action at fruition.

2years and 2months ago I started work as a dr. I thought I was sure about where my career was going (Paeds - PICU/EM, some MedEd). Turns out I was wrong! Tomorrow is my 1st day of surgical training, and I am terrifcited, hope they like pink as much as I do!
Look surgeons are weird people. I say this as a surgical trainee. We really like operating, if we offer it to a jr it’s as a treat (especially if it’s instead of ward work). If we offer you to scrub into a cool case we like you! 😂
Induction: Wellbeing is important - look after yourself. Have hobbies. Breaks MUST be taken. Exception report!

Today we consented a pt for a big serious op tomorrow. There’s a language barrier& we could have used language line BUT we figured out our FY1 speaks their language& the joy on the pt’s face was overwhelming. Really unhurried consent, diagrams, a delight

I use disposable cups because they last for 12hrs (&sometimes my cases are that long!) but be honest if you don’t feel well! My issue is more mid-cycle nausea, I’ve even thrown up in a bin during a case…look after yourself, rest as much as possible
