What analogies do you commonly use to explain medical concepts to pts? I like comparing osteoporosis to having missing bricks in a wall, which renders it prone to breaking. And having low Ca/vit D is like having poor quality bricks. Hence important to treat both for bone health.

So Psychiatry-UK are not accepting any new NHS Right to Choose referrals for adult ADHD or autism assessments until further notice due to unprecedented demand. Referrals from GPs now being rejected. No sign of locally commissioned services being given more funding. Not good.

I do find these simplistic takes quite grating. Cases are often โ€˜barn doorโ€™ if you strip away the numerous biases and distractors that occur with real patients. An IT worker being able to diagnose a clot based on a list of symptoms does not prove anything in this case.

Re: Barn door... Gave my husband (works in IT), the history of calf pain and shortness of breath, and that the patient died shorty after being seen twice. His response: 'Did she have a clot?' He works with computers folks! Enough said.

This was the worst response Iโ€™ve had from my GPโ€ฆIโ€™ve only just realised it was PA that sent this me shit๐Ÿ‘‡๐Ÿฝ (Iโ€™ve had sleep apnea since I was at least a teen but it was only diagnosed 2yrs ago, which caused me to develop insulin resistance, which then caused me to develop PCOS)

@jakepmann I still remember observing an obstetric SpR suturing a tear during a major obstetric haemorrhage as a medical student and deciding pretty soon that it wasnโ€™t for me ๐Ÿ˜‚ Those who have the fortitude and skill to do it are incredible. Their hourly pay def doesnโ€™t reflect this.

I feel that the follow-up consultation after the โ€˜normal bloodsโ€™ is equally important as the initial one. The patient needs to feel validated that their symptoms are still affecting them and the clinician needs to show ongoing curiosity to find a diagnosis.

I believe all hospital speciality training should include 3-6 months of GP placement. Trainees need to see how primary care work & how issues related to their speciality are dealt with at that level. Maybe they will also develop some sympathy toward GPs.

Another eg: you can do an MRI of the knees/spine of a marathon runner and it would prob show the most horrendous wear-and-tear. Yet they can still run 26 miles. Therefore treating your joint pain from a functional perspective (ie physio) is often more beneficial than a scan.
