I paid £650 out of pocket to attend a 2 day Basic Surgical Skills course as a foundation doc. Why do PAs get 2 weeks F2F fully funded course?
This looks like a great course for surgical FYs, can this be brought in for docs please?
I've been preparing a CST portfolio since 2018... And they tell us a month before applications that we need to do another exam in January? An exam which isn't surgical, but originally designed for GP applications?
It's really exhausting. Maybe time to look at other specialties.
We are making careers in medicine more accessible by launching the first 200 doctor apprenticeships in the NHS.

Huge bottle-necks at CT1 and ST3 selections. So many Doctors who have already completed medical school + years of post grad work who are unable to progress in training.
Let surgical SHOs do these lists. We might even get more consultants in 6-8 years if you keep training them!

I've been offered my ideal medical education F3 job. CST offers being delayed means I either decline the MedEd job and hope for a training post, or accept the MedEd

Why is MRCS part A (£580, 3 good months of study) no longer awarded points, but a Masters (£15-£30k, 1 year full time or 3 years part time) is required for full points in teaching qualifications for CST?

As a med student, an ortho SpR told me that there will be some really tough days in FY- but don't let it put out the enthusiasm I have for surgery, because things will get better.
I've thought about this alot this year. Really held on to it.
Knowing full well I should be studying for MRCS or working on publications or writing an award winning essay prize rather than taking locum assisting shifts in theatres 🤡
But still can't really believe someone will pay me. To assist!! To learn!! 🤩
2 months into gen surg and I've grown to enjoy making The List 😆
It's key for a safe handover and ward round 🤝🏻 It anticipates the information the reg and cons wants at handover 🗣Most importantly, making The List is the perfect opportunity to sit down and have a tea 😎
Do you ever get used to theatre staff +/- anaesthesist asking "and how long more is this going to take?" when you're closing a loooong incision as the most junior member of the surgical team? 😬

Still getting used to reviewing ill patients independently, handling all the bleeps and ringing the med reg for support 😅
At the end received the sweetest card from the final year who's been shadowing 😍 he came for some of the nights (dedication!!)
First week of opthalmology going well so far....
✅ full day clinical induction by consultant
✅ allocated SDT!!

✅ seniors interested to teach (slit lamp, hand held lens etc)

Demoralizing to be valued at 30% less than trainees in 2012. Demoralizing to see senior medical leaders on twitter gaslight trainees who speak up and deny that anything is wrong with the current state of training.
I've discovered I really like teaching, and would really like to have medical education as a part of my future career. Any advice from

Can think of F3 as a teaching fellow, perhaps postgrad qualifications, but seems like there is no clear pathway!
4 months of ophthalmology done 👀
Really sad to leave such a great team... Cons and regs were always so keen to teach! Steep learning curve but finally got the hang of slit lamp examinations, volks lens, subtenons blocks and even some 8/0 suturing. Will miss Monday theatre days!
D-0, post PAO. Lots of insight into being a patient:
- GAs are really quick
- Spinals are a peculiar feeling...
- spent 2hrs snoozing in recovery, still woke up whoozy and asking nurses for their name a few times 😂
- Didnt manage to stand with physios today... hopefully tmr 💪🏻
CST rollercoaster continues... delay in interview invites when it was scheduled for today. It's only 2.5 weeks to interviews and we cant give our clinical teams proper notice of which day we'll need off for interviews!

CST applications evidence uploading is so unclear!!
Are we supposed to upload 1 big document for each of the 4 domains, or upload each piece of evidence separately? Which domain do we tag the index page with?
No AFP offers yet fr Camb or London - but was altogether still an excellent learning experience:
1. Can talk through A-E scenarios in my sleep
2. Learnt to prioritise multiple sick patients (in theory)
3. Can critically appraisal papers
4. Interview practice for ?CST/future jobs!
A 6th former said she wanted to become a surgeon, but felt she wasn't (physically) strong enough after watching a medical TV show.
She was very relieved when I told her about all the amazing female surgeons I've met! I'll count that as a successful talk 🤝🏻

Surgical Skills Club back running this week!
We started this 1.5 yrs ago as we believed in accessible, locally-delivered, regular skills teaching, featuring small group feedback and spaced repetition.
Sadly it seems like things aren't looking to great for post-FY training. Demoralizing to see great colleagues fail at getting training posts due to cut-throat competition ratios. Demoralizing to hear CST trainees say that their work is mostly service provision on the wards.

Last week I've started with light jogging. Now I'm signed off to get back to dancing and ice skating 🥰 the 500m walks between the plastics office and A&E (multiple times every on-call) have been useful rehab!
First STU (supervised - trainer unscrubbed) case in plastics theatres yesterday 🤩
Interestingly - I found the overall flow the challenging part of this. Suddenly forgot the name of instruments and dressings 😂
This department really wants to convert me into an opthalmologist! They all say its the best specialty... Learnt to do a lateral canthotomy and enucleation with one of the SpRs Rob and the Ukrainian medics students on placement with

Fantastic nursing team who did bloods, cannulas needed for ward pts. Means I had time to be clerking and learning in ED!
No matter how many trotters/bananas sutured:
Nothing quite replicates the pressure of closing skin at 5.15pm, under the reg's watchful eye, with anaesthetics hovering and scrub nurses already finished with their count.

Jokes aside- abit nervous that I only have 1 medicine rotation and 3 (!) surgical jobs. Hopefully will have enough time to develop good medicine-ing skills as a FY....
One of the things I find most challenging during on-calls is getting difficult peripheral venous access.
I know there's always help available, but would be so good if I could properly learn how to do ultrasound-guided cannulas! And useful skill for most hospital doctors?

Struggling to rank CST jobs. Combination of pessimism for surgical training in the UK, and uncertainty over specialty choice.
Why is CST still necessary if most posts are "themed" anyway? (Most likely, ST3 bottleneck forces more into trust grade positions)
Low cost conferences, courses and teaching programmes suitable for FYs and med students! Now a spreadsheet. Cost should not be a barrier towards entering surgical training.

This is my monthly full-time (40hr) payslip June 2022. I studied for 6 years at Cambridge Medical School and graduated 2021. I'm lucky not to have any student loans on top of this.
Feeling valued as a trainee today 🤩 Did my first 2 subtenon blocks (under supervision) for today's list. Really supportive consultant and reg who identified the right patients and set out time for this today!

Missing home alot lately, so I made the quintessential Singaporean breakfast- kaya toast with soft boil eggs 😍
Kaya is an egg custard spread flavoured with pandan leaves! Abit of work to make but definitely worth it!

Some ST3 portfolios penalize years out (e.g t&o and plastics)
Yet the CST portfolio gives points for things which seem almost impossible in 15 months of FY, which may mean people taking f3 to build CV for CST (Masters of meded... usually 2-3 part time isn't it!!!!)

Friends have asked if gymnastics made my hips worse. I'm not too sure, does dysplastic hips have better ROM? High level gymnastics certainly accelerated degeneration...
Tinzaparin injections are surprisingly sore. Constipation much improved and pain is manageable.

2 weeks ago! I spoke about fairness and differential attainment CST selections, MRCS and FRCS exams. We need to prioritise this area for research and discussion - People are the future of surgery!
Just because we can make a new event/conference/society, doesn't mean we should.
Think about who your audience is, what they want, and what they need. Organizing events should be more about what others can get out of it, rather than what the organizer can get out of it 🤗

I've been given the go ahead to wean to 1 crutch at my 6/52 appt 😍
It's been easier physically but difficult emotionally. Being dependent and alone for most of the day gets frustrating 😢 Luckily I've had friends visit and a good friend's wedding to celebrate!

- challenging night... struggled to stand up with hypotension so couldn't get to a commode 😢 bedpans need a redesign tbh! I felt so embarassed but the nursing team has been so lovely and understanding 😊
- really pleased to get walking on zimmer frame with PT
Kathryn is a 6th form student and gymnastics jr. While discussing alternatives to work experience, I suggested writing an article in BMJ student, to share these ideas with others.
So proud of her for making this happen! Mentorship can be done at any stage, even as med students🙏🏻
