FWIW The ranking of antihistamines 4. Loratadine - completely inert 3. Cetirizine - works 100% but zonked for next 24 hours 2. Fexofenadine - decent, doesn't always work, need to explore biiig doses 1. Acrivastine - underrated but 1-2 doses never let me down, works quick too! ⭐

Something another doctor said to me recently really struck - "One shift you see lots of patients, get the wait down from 10 to 1 hour and be congratulated. On a busier day you can work harder, see even more patients, but the wait stays at 10 hours and you get no appreciation" 😥

I've digested it and short answer - this offer is terrible I note that a consultant here will nigh on always be stuck in the "60% tax trap" range. There is no incentive to work full time hours - I'd rather keep around £100k as there is no significant break through the other side

"Full time training" is quite inconsistent. One rotation this year had me doing 41 hrs/week. Another is 48 hrs. If I wanted to continue working 41 hrs in the second job, I'd have to switch to 85% LTFT and extend my training... Is this something @BMA_JuniorDocs has looked at?

I think at this point each pre-CCT doctor needs to develop an extra skillset beyond a basic CCT to keep themselves relevant Our USP used to be simply "being a consultant" but if trusts can pay an 8c to do it... For me I hope that dual EM/ICM and PHEM is enough! Open to ideas 😅

This is my rota for next 6 months as ST3- absolutely brutal. I am already regretting being a run through EM trainee. I did this rota as an ST1 and it made me depressed and worn out.

Its hard not to appear daft every time I explain how applying to ACCS works to students/foundation docs - picking a spec at the start makes no sense other than to trap trainees and make HEEs job easier

@BDChadwick @zackferguson @iowmillie Someone still needs to explain to me why you would call something "common stem" but expect trainees to commit to a specialty at the start. I have always considered this a real drawback of ACCS.

@dr_emmacoombe @Alexandra_nidd When I had a long running car parking dispute with my trust, the thing I think finally solved it was writing an email along the line of "I am spending all my time at working writing these emails and going to various offices to solve this, leaving me no time to do my job" 😅

Its strange that everybody can use Twitter, Facebook, etc without being made to go through arduous e-learning, but qualified professionals are forced to go through it for clinical systems Maybe a small hint that these programs have awful design...
