Sadly my NIH R01 renewal did not get funded despite over 42 published papers most of which were in good journals (22nd percentile). I am going to focus on more important things going forward like being able to bench 300lbs. Something I guess none of the reviewers could do.
I have accepted a new role as Editor-in-Chief of pubmed indexed: The Neuroradiology Journal. Looking forward to building a social media team and a new team of INTERNATIONAL associate editors! If interested please email me brinjikji.waleed

(-) MRI does not mean no SIH. Pt with brain fog, orthostatic headaches, tinnitus, imbalance. Dismissed by many docs. Pt advocated 4 themself. Large CSFVF. Embolized. 6 mo f/u Sx resolved. We need to advocate for our patients!

Honored to receive the 2023 No LVO Left Behind Award for my commitment to LVO and coming in on a Lime Scooter for a code stroke because my son was mid haircut at the mall and I didn’t want to leave my family with no car.

Just a diagrammatic representation of all the anastomoses between the ophthalmic artery and the ECA. Yes, I do make the fellow memorize this before any eca embolization.
So happy to see our work featured on
@TheAJNR
cover. First truly impactful paper I have ever written. Glad to see that at least a dozen docs have started tx pts with this. Tx 50th pt yesterday. Anyone who wants help using this technique to Tx a patient call me anytime.
Wake up stroke. NIHSS22. CTP shows 72cc core. ASPECTS 10 though? Went in because ASPECTS>>CTP. JETD. Pt. normal at 24hr. Near nl MRI post op.

faith in CTP now 0.
Clinical and imaging outcomes of cerebrospinal fluid-venous fistula embolization | Super proud of this one. Forty consecutive patients. HIT-6 and Bern score improvement. High rates of clinical efficacy.

This post is bound to offend many. But I want to be a controversial neurorad. Someone asked me yesterday my biggest professional regret. I was honest...it was not going into neurosurgery.
Too much cynicism in medicine leading to too much disillusionment. Take a moment to reflect! What a privilege it is for us to be able to care for others, teach others, and help advance science to potentially touch millions of lives.

The End is Always Near. I poo pooed the talk of pandemics rapidly destroying global economies and societies. We are too modern for that I thought. Well, now I see how it can be true. The end is always near!
Beautiful gift from my mentor and friend

Egas Moniz lecture to British Society of Neurosurgeons signed by Moniz himself! We stand on the shoulders of giants!
Bad things can be prevented if you know your anatomy! Here’s a diagrammatic representation of various anastomoses between the cavernous ICA and ECA branches.
Simple coiling of broad necked acom. Dual operator. A tip I learned from my mentor G Lanzino-park micro at neck when coiling broad necked aneurysms and you might avert the “need” for a ballon/stent/etc
Got the chance to do first in man for our FDA cleared BOSS balloon guide catheter from our startup Marblehead Medical. First pass tici 3 with 6Fr Sofia in M2 and flow arrest! Special thanks to

My research fellow is applying for radiology residency! He has great work ethic, very generous and kind, a great communicator and is very bright! Look him up when you are looking at your ERAS applications.
Lots of ways to cause posterior circulation strokes even when you are embolizing far away from the vert. Keep these dangerous anastomoses in mind! YouTube three part series coming out this week because...why not?
Case-Based Vascular Neuroradiology-This was a labor of love during the past two years. Lots of time reading, writing and putting together cases. I did it to be better in diagnostic while in INR fellowship in Toronto and learned a ton in the process!

Absolutely unnecessary. I am not treating stemis although I am sure I can catheterize a coronary artery and put a stent. It’s disrespectful to the field of neurointervention for other professions to think they can take a weekend course on thrombectomy and become neuroIRs.
Another “classic” CSF venous fistula on digital subtraction myelogram. Thanks to Richard Farb for opening my eyes to this technique!

Large basilar aneurysm. Previously coiled. Marked compaction. Tx options include stent assisted coiling or WEB. Went with 11x6 WEB. Hard to get via 33 to aneurysm neck but once there was very solid.
Great time having

visit this week and see our csf leak, spine and neuroIR practice! We learned a lot from him! Amazing stuff going on in Bern!

Dangerous anastomoses during cSDH embolization. Here is a nice example of the meningolacrimal branch. Always the potential for anastomoses with ophthalmic. We went distal for the embo.

I’m just gonna say it...an unintended consequence of the administrative bureaucracy built around academic research is that it detracts from meaningful research. To put it bluntly, administration kills research and thus progress.

What is low ASPECTS? U r seeing loss of GW differentiation but low ASPECTS≠large core. Here’s an example of a pt I had with ASPECTS of 3-4 in retrospect. One pass Sofia Plus. Final infarct shows loss of non eloquent gray matter but no large core! mRS 1! Topography matters too!
amazing. 3 months ago

were publishing articles framing docs as greedy and now in the midst of pandemic, who is putting their lives and their families’ lives on the line every day to help patients? hope the misrepresentation of physicians stops here.
Americans should cheer for Morocco. On December 20, 1777, the Kingdom of Morocco became the first country in the world to recognize United States independence, only a year and a half after the U.S. Declaration of Independence was issued.
Lecture topics to be posted next week on the YouTube channel: 1. Developmental Anomalies: from variant to disease. 2. Capillary telangiectasias: from variant to disease and 3. Intracranial Dissections!
Going through the charts of the first 100 CSFVF pts we treated for a study on 1yr follow up. Learning a lot from both successes and failures. Every interventionist/surgeon should take time to personally review outcomes. Lots to learn from past experiences.
CSF venous fistula on Cine setting using

fluoro. We are always working to figure out a better way to identify these so patients can get the Dx and Tx they need.
So proud of our CSF leak team

for having enrolled 25 csf leak patients in our clinical trial in just 6 weeks. Teamwork and multidisciplinary care across neurology neurosurgery and neuroradiology pays off.

I’ve never called a sign before but my fellow and now colleague John Benson couldn’t pass this one up. We noticed that Loeys Dietz patients have a very capacious carotid bulb and a wide angle of bifurcation.
One of my biggest regrets in life, other than not becoming a neurosurgeon, is that I never had the patience to learn how to code.

Conclusions
The direct proof of spinal CSF leakage in 25.9% of patients suggests that spinal CSF leaks may be a frequent cause of nongeriatric CSDH.” The inr community needs to start taking SIH more seriously
Sorry it took so long for us to get our act together and get this out there. But finally a nice technical video on our novel csf vf embolization technique
Me: WEB is amazing. It’s amazing that we can finally effectively treat these broad necked aneurysms without stents etc. Boss: Type in Moret and balloon remodeling in pubmed. Me: 😳

Things that make CTP less reliable: old strokes, white matter disease, low cardiac output, arrhythmia, prior contrast, carotid stenosis, bad bolus, patient motion, processing errors...what else?
Thinking about starting a quarterly case conference for neurointerventionalists treating CSFVF. If you are interested please send me a DM and we can work to organize
Excited to talk about csf leaks at the international stroke conference tomorrow. Especially excited to reach out to neurologists and interventional neurologists around the world to talk about how we can touch more lives.
Putin invaded Syria a few years ago. 2021 Airwars documented 23,000 civilian casualties from Russian military actions since 2015. No one cares though because they don’t look like “us” whoever “us” is. No SNL tributes. No celebrity tweets. Etc.
Excited to be speaking tomorrow on treatment of CSF-venous fistula

Another testament to the cross discipline nature of the organization and our field in general.
I’ve been seeing a lot of patients with stone cold normal brain MRI and massive csf venous fistulae lately. I wonder what the mechanism of the lack of brain sag in these patients is.
