🧵Many IMGs seek temporary research🔬opportunities in the US every year. The process can be long, frustrating & unpredictable.
🙋🏻♂️I have experienced it both ways.

IMGs often sound anxious & apologetic when explaining gaps in training or need for visa sponsorship.
These challenges are NOT signs of weakness but rather proofs of resilience.
Be your authentic self
Explain, kindly; don’t apologize

A med student asked: why do we need epicardial coronary arteries? Can’t the ❤️ utilize the abundant oxygenated blood within it? We then talked about transmyocardial revasc (TMR), which apparently has resurfaced some recently. Thought I’ll summarize my read on it in this thread 👇

In the past 7 days, I received 46 emails from IMGs looking for research opportunities. Majority had no prior experience & majority willing to volunteer.

7 months ago I wrote a tweetorial about the non thrombotic functions of the LAA. Great insights from many people followed.

suggested we turn it into a paper. The 📝 was published today. Should have listed

Seekers of knowledge go through 3 stages
First, they attain some knowledge, feel superior & become arrogant
Next, they realize how much they don’t know & become modest
Finally, they master their subject & become aware of & humbled by their deficiencies & limitations

today. Majority of pts (43/50) undergoing TEE guided SHD interv have evidence of esophageal/gastric injury in EGD! & in 20/50, the lesions were complex. Will we see more ICE when we have the right technology?

🚨Landmark BEST-CLI trial published
@NEJM
. Likely the most influential RCT in vascular medicine this decade
Among 1803 randomized pts with CLI, the incidence of major adverse limb event or death was significantly lower with surgery vs catheter Tx (when suitable SVG is available)
Total occlusion is actually present in 25% of all NSTEMI & is associated with 40-60% increase in MACE & mortality.
The STEMI/NSTEMI classification certainly doesn’t tell the whole story & it needs a long overdue reconsideration.

Future of valve disease classification? Machine learning algorithms that incorporate features from all available data (echo, Cath, ECG, labs, etc.).

Its quite intriguing how much TR can be tolerated before florid clinical decompensation. This indeed is chronic torrential + TR in an ambulatory setting.

do not agree on quantifying MR severity (again!)
💢152 pts underwent echo & CMR
💢In pts w ‘definitely severe’ MR on echo, only 52% had severe MR by CMR
💢 Severe MR by CMR (but not by echo) was a predictor of post MV intervention LV reverse remodeling
