For 5 yrs I have taught medical students about childhood obesity, pointing out that it is complex, and strongly associated with socioeconomic deprivation. That simplistic fixes and blaming children and their families won’t work. That we need to change society. 1/2

Reading the discussion about PAs and juniors, as a former trainee and current consultant (paeds) makes me feel the major problem is how we are treating g junior drs. I really think the time has come to acknowledge that 4-6 monthly rotation is no good for anyone.

The lack of non-clinical staff is exactly what means I, a paediatric consultant, regularly end up changing beds, cleaning floors, sorting out discharge letters and re-stocking equipment trolleys. Because until those things are done, I can’t do the job I’m supposed to do.

I often see little girls with vulval issues and the parents can’t/won’t even use correct terminology when I explain it to them. Today I went on a bit of a rant saying vulva and vagina repeatedly. If we don’t know the proper words we can’t look after our bodies
3. No one should feel guilty about not breastfeeding.
4. We should have an unbranded affordable unadvertised formula milk available at cost or for free for families who want to or have to use it.
5. We should train our HCPs to support breastfeeding effectively.

NHS worker here - it doesn’t have to be either/or. The clapping was lovely to hear, but I just want people to remember. What we are doing, we are doing on a background of a decade of criminal underfunding of health and social care. There must be a reckoning at the ballot box.

Because when a HCP is sitting with a distressed mother with a screaming baby and shredded nipples, telling her that breastfeeding will help her baby get A grades at GCSE is not helpful. But helping her breastfeed that baby without pain or distress for either of them is.
I’ll finish with my summary:
1. Research on the health impacts of breastfeeding needs to be addressed at population not individual level.
2. Knowing that breastfeeding is associated with particular outcomes does not help anyone do it.

I don’t expect huge amounts of sympathy but my actual earnings as a dr are 25% lower than predicted 23 years after I started medical school. But that’s because of the actions of this government, so…
6. Look at how people live their lives: how easy is it for families with food insecurity, uncertain employment, poor accommodation, unsafe physical environments, health issues and limited time to cook healthy food from scratch and exercise? (Spoiler alert: it isn’t).
I am deeply worried that families will be (even more) frightened of their child’s death after recent news coverage. I have had the immense honour of providing end of life care to many children and it has always been a dignified and compassionate process.
Withdrawal of life sustaining treatment in ICU is conducted with the continuous presence of a trained ICU nurse providing 1:1 care, with medical help if needed. Supporting family is central to this process, as is preventing pain/distress in the patient. It is not barbaric 1/

Secondly, this stuff really really matters. Feeling embarrassed to bf in public is one of the top reasons mothers give for stopping bfing. The media traction these stories get makes women anxious. It turns bfing into a niche and unusual pursuit. 2/n
The baby may well be normal, but the times are not. The experience of parenting is not. I can’t give worried parents a hug, a biscuit and a cup of tea as a peer support group can. But I can use my ears and knowledge to help them through this strange time. /end
For governments to invest in population level strategies. This includes better training for healthcare professionals. It’s easy to be caught up in the WHY of breastfeeding, but if we can’t help with the HOW to, we just set everyone up to fail. Parents and professionals.
6. We need to look at societal levers around how we understand infant behaviour and feeding, and work to support parents with normal baby behaviour with honesty and compassion.
