I have had 3 cases of monkey pox in my clinic this week. None that looked classical. One that when I curbsided Derm, they swore was molluscum. Just a matter of time before spreads through the country. If is a lesion clustered monkey pox until proven otherwise

Patient tells me today went to a large ED clinic advertising in NYC. They put an ultrasound probe on my penis, said I had very bad flow and I needed shockwave ASAP. No artificial erection, ultrasound by MA. Can we call this malpractice? It's just predatory medicine.

Peer to peer is the biggest joke. The latest nonsense for a patient with a venous leak to get an ipp. Needs a cortisol, TSH and prolactin. I asked what kind of cortisol, random is worthless? Don't know. You going to get an MRI for a prolactin of 25? Don't know.

It is remarkable the amount of BPH treatment options we have now. I think it is important to offer as many as possible because every prostate and patient is different. I offer Rezum, urolift, greenlight, aquablation and bipolar turp. I think they all play a role.

Had to do a peer to peer for an ipp exchange, spent 10 minutes explaining why the patient did not need a trial of pde5 or ICI. Now I have an additional call back tom for another family medicine physician who is an "expert" in ED who will explain why the surgery is denied.

@SeanDreyer Algorithm: hold penis and stretch like you mean it. Try regular catheter first. If resistant at prostate switch to18 French coude. If there is distal resistance try a 12 French. Lastly never inflate till catheter is all the way in! You can get return of urine in the prostate.

If there are problems': clot retention etc. we know what happened. If a pae has issues we are the ones taking care of it. From dysuria calls to clot evacuations.
