Y’all - I prescribed vaginal estrogen twice today on patients with a history of breast cancer. My heart did not race in fear. I explained to the patients the risk (no data showing actual risk) and the benefits (no urinary or pelvic pain) Patients understood all of it.

Much of the data is on ssri induced sexual dysfunction and female arousal disorders. It doesn’t fix libido unless your libido is low because your genitals don’t feel activated.

Nobody tries and fails vaginal estrogen. It is a must for women >50. Many different ways to do it and creams can be irritating. My go to when cost/insurance fails: estradiol 10mcg: 1 insert daily for 2 weeks then twice a week until death do they part. Use goodrx coupon. #sexmed

Side effects: it’s a muscle relaxer so you may get a headache, facial flushing, indigestion. Don’t take it if you take nitrates for chest pain. Otherwise they are very safe and used at much higher doses for men with erectile dysfunction.

Did you know alcohol 🍷 consumption increases your risk of breast cancer? Should we 🛑 using alcohol wipes to clean your skin before a blood draw? Systemic estrogen does not cause breast cancer. Local vaginal estrogen DEFINITELY doesn’t cause breast cancer. Ted talked.

New patient list today and I am very disheartened to say 4 patients on my census are 90+ Fs with mild dementia admitted for delirium. All started on abx in ER and continued by my colleagues on admission. None of the 4 had GU symptoms, fever, or even "appeared ill" Worse yet:
