I can't look at his little face and imagine what he would look like anymore.

The hardest part to accept is, due to the fact I was preterm I should have been offered antibiotics IV prophylactically pre policy- they were forgotten. He was symptomatic shortly after birth, no one came when it was escalated by a midwife. He was saveable for many hours 😔
Watching my children's hearts break as a result of such careless actions of the hospital. It adds a layer of anger and frustration, their innocence has been stolen at such a young age. How do you explain to children that babies die, it's something | don't even understand myself.
This is the devastating impact the hospitals actions have had. The coroner ruled “neglect”, only the reverberations of his loss travel so much deeper. Watching our other children suffer his loss pains us to the core. The birth of a sibling should be a happy time, filled with love

This is Zachary, he lived for 14 hours before I had to watch him die in my arms. He died due to a number of hospital failings at UHDB trust. Maternity care is not safe, take a look around you and not with political blinkers on!
There is something uniquely wrong when the order of life is interrupted.
To outlive our children, to miss out on every first, it’s a type of torture that only ones who walk this path can understand.
I died with Zac that day, only his heart stopped beating and mine carried on.

NEW: I asked the Prime Minister today if he would commit to a public inquiry into maternity services.
It’s what many bereaved & harmed families have called for - in Nottingham and beyond
Here’s what he told me:
"the Trust’s midwives showed no acceptance of any wrongdoing or that they would have done anything differently"
Clearly, not everyone involved in the family’s care was deeply affected then.

Even when Zac was minutes from collapse the midwife was unable to recognise he was septic.

I don’t understand how that’s even fair, for all the families without representation it must be so daunting. The lack of control would make me spontaneously combust. Having a legal team enables families to get the answers they need, not just the coroner.

I went to an inquest recently where the parents had no legal representatives but the trust had a legal team and three midwives also had a separate legal team. 2 Vs 0 😥 so unfair. There should be legal support available for all families at inquests.

I can’t believe it’s been over a week since we listened to the coroners conclusion.
I’m not sure I will ever truly compartmentalise how my son’s death was contributed to by “neglect”.
It’s hard to understand how many errors were made in such a short space of time. 😔💔
When I read Martha’s law, I just burst into tears. It resonates with our story, and for many other loss families I’ve spoken to.
Parents know their children better than anyone, yet they are often fobbed off as “parents worrying over nothing” until, it’s often too late!
The death of Martha Mills had a real impact right across the country.
Parents, patients, and NHS staff could all sympathise with her parents’ call for a single, easy process to escalate concerns when they can see their loved one’s condition worsening.
The inquest into the death of my clients’ baby son, Zac Taylor-Smith, starts today in Derby. It will be an incredibly hard few days for Hannah & Tim but we hope to learn why & how the failures in care before Zac’s death happened.

Cuddle cots are invaluable resource for so many grieving families 💗 If it wasn’t for cuddle cots my other children would never of met their baby brother, I wouldn’t of got to sleep next to him over night.
This is why a public inquiry into maternity services needs to happen and it needs to happen now!!

Not one doctor, consultant or midwife picked up that I should be given antibiotics. As you know the policy is prophylactic intrapartrum antibiotics should be offered for a preterm delivery. One action that we know through evidence, would have changed everything.

It’s something that stays with you, our son passed away 14 hours after birth. Our husband raised concerns and was asked “are you medically trained” as he used medical terminology. It’s heartbreaking to see this overarching theme that staff dismiss parents, then tragedy happens.

I don’t understand how two neglect verdicts can happen in one week, two separate hospitals. Baby Orlando should still be here with his parents, the frustrating part is the staff will go back to work and their lives go on. Ours will never be the same again!!

I’m fairly convinced that most practices done back then- do not correlate to modern day medicine. I’m unsure why this is causing confusion to professionals at all. They should know their local/ national guidance, not be following and copying a TV show based in the 50’s/60’s.

an amazing charity that needs more awareness! My lovely friend lost her boy “Harrison” to undiagnosed CHD, it’s such a heartbreaking story, he’s so very loved and missed.

Harrison struggled with feeding throughout his short life. Although Katie repeatedly took him to see different health professionals, his heart defect went undetected & he passed away at 32 days old. Katie has very bravely shared his story: ❤️

Tools require clinicians to actually use them properly and/or know how to use them. The “NEWTT2” as we heard in Zachary’s case, people were aware of it, filled them in (then did nothing) or didn’t follow them at all/ or did escalate and no one came. 😩🙈

Do you know the threshold for a “neglect” conclusion by the coroner? It has to be “basic” care, not clinical decisions but fundamental “basic care” if you think that this is ok in healthcare, you’re deluded.
