I have exceeded the annual allowance in 4 out of my first 5 years as a consultant. This weekend I could be doing extra hours reporting our backlog of cancer scans. Instead I’ll be trying to work out if I have a AA tax charge and looking to see if I should reduce my sessions.

As a Head of School I’m deeply uncomfortable with the @RCRadiologists raising their exam fees by 22% this year. I know inflation is high, costs have gone up, and college exams don’t make a profit but 22%, really?!? Doctors are genuinely struggling to afford their exams!

Last year I did ~40 extra reporting sessions. This year I’m doing 0. If I do 1 extra my tax bill goes up £3062, a marginal rate of 765%. That’s 100s of patients waiting longer for their results and costs NHS more++ to send to a private company for reporting. #scrapthetaper

12 years ago today, a radiologist told me I had testicular cancer. Now a radiologist myself, I’ve seen so much cancer that it’s easy to become desensitised to the impact our words (usually indirectly) have on patients lives. Today I remember those less fortunate than me.

Interesting debate on titles- The Medical Act (1983) almost always refers to ‘medical practitioners’ not doctors (or medical professionals). #GoodMedicalPractice2024 means AAs and PAs once in UK regulation will have to follow the same high professional standards as doctors.

There are >3000 applicants for ~350 radiology training posts this year. Currently shortlisting based on MSRA only (🤢). Approx 700 shortlisted. Final rank is based on 40% MSRA, 30% portfolio and 30% interview. We can do better. How should we recruit radiologists?!

@AlexNevard I almost never decline scans, but as you say, frequently suggest different modalities or degree of urgency. It’s about discussion. That said, imaging isn’t without cost, both financial and risk. Otherwise we’d just replace the hospital doors with CT scanners and scan everyone.

Top 5 #tipsfornewconsultants from my talk at @RCRadiologists New Consultants Day: 1. Don't be afraid to ask for help 2. Learn how to say "no" (delaying is a good tactic) but say "yes" sometimes 3. Never "take a quick look" in an MDT 4. Find a mentor (or 2) 5. Don't email in anger

@tivaisgreener @rupert_pearse @TheBMA I find it incredibly discourteous that many of those on their high horse about this are sitting on national CEA awards with a careers worth of final salary pension nearing the end of their career but seemingly happy to pull the ladder up from younger and future consultants.
