Last week, 37 yo aortic dissection walked in to the ED, collapses from tamponnade. Awake cannulation for bypass, full repair. Now out of the hospital & great. When I learned his daughter had just blown the candle on her 1st birthday cake 10 days before:

@dr_benoy_n_shah It’s not about the possibility of conversion to surgery - it’s the heart valve clinic / heart team discussions leading to choice of treatment (OMT / TEER / surgery). If you can’t discuss with a full heart team, indication creep for what you can offer is a real risk.

#EXCEL : It is mandatory to publish all results, outcomes and statistical analyses of clinical trials under FDAAA 801 and “Final rule”. Including secondary outcomes. Failure to do so carries consequences for future government funding, as well as civil and penal liability

So to summarize, if the data had been available to the guideline committee: 1. EXCEL (CABG superior) 2. NOBLE (CABG superior) 3. Lancet meta-analysis: underpowered, possible slight advantage for CABG The #HeartTeam needs to sit down and revise the guidelines @escardio @EACTS
