A loved one was discharged from Tallaght hospital 6 yrs ago. In and out for the guts of 2yrs after multiple suicide attempts and suicidal ideation. We were never informed by Tallaght hospital that he was released. We found him in the Liffey. Still waiting for the Inquiry date.
