@PhysioDOH @JackAChew It’s a two way street! My practice is nearly all second opinion. Many patients seek me out and self pay as they were just given an exs sheet in Nhs. In all walks of life in all settings you get good and bad professionals. I’m so done with this debate of Nhs v private.

@AdamMeakins Absolutely agree Adam. Academia not always in touch with clinicians needs. My crepitus research perfect eg. Rejected by 3 journals on grounds of insufficient interest to readership. Video on Facebook about it now in excess of 1.5 million views.

Tip of the day! When u palpate Hoffa’s fat pad remember it attaches onto the goes far superiorly than people often realise. It’s often the lateral superior corner that’s tender too. interested in the fat pad? In depth webinar coming v soon.

@RogerKerry1 @bmj_latest It's ridiculous! I'd like to see someone demonstrate their superhuman powers by stretching the ITB. This pic shows how massive it is. Just to be clear we can stretch the proximal contractile origins but never the ITB itself.

People ask me how I’m coping being the main Carer for my dad with Parkinson’s and mum with Alzheimer’s and two small kids. It just brings home to me the importance of kindness and compassion. I think I’m a better clinician for that even if now and again I miss an article..

It’s been interesting to see how many old patients of mine have contacted me in the last 2 weeks with return of their PFP. Common thread: no gym and long enough has now elapsed for significant loss of strength effecting PFJ.

If you own a dog please be aware of the following. Last night our gorgeous 14 month old retriever Olive ate a ball of raw pizza dough. If your dog eats dough it’s an emergency as it will expand inside them and also ferment. Luckily swift action by the vet meant she’s fine today.

It drives me mad! I spend a lot of time unravelling catastrophic structural inaccurate beliefs that lead to v fear avoidant behaviour over this term. Always good to remember cartilage is aneural.

@honest_physio Can we not integrate? That’s what I truly aspire to on my PFP course. So yes I talk biomechanics and yes horror a bit of taping, but I also talk about health beliefs and how to take a history without your own biases interfering etc. IMO the prob is it’s seen as one or other.

After a 30 year gap I took back up learning the cello.This has been a really wonderful experience and such an antidote to my busy life.I thoroughly recommend it!! I felt like the oldest player by a mile at the exam centre but have just got my certificate for my grade 6- so happy!

Beliefs and expectations influence pain perception in a diagnostic test. Pain was closer aligned with expectations than with the pathobiological process. Anticipation of pain should be considered in diagnostic tests ⁦ @Melaniielp ⁩ ⁦ @michelcoppie

My 1st tweet for #mentalhealthawarenessweek2019 as a physio just treating patients w knee pain it’s amazing how devastating the loss of exercise is.The stress relief but also effects on body image. The loss of exercise can decrease mood, effect sleep & lower confidence. @tomgoom

A moment of real joy this am. My 11 year old performing a surprise concert to my father on his 87 birthday through the window of the care home. Turned out it was to all the other residents and staff too! No hugs but real joy. Rounded off by happy b’day! ⁦ @shrewsburyhouse

Two patients today @WClinics with raging fat pad problems. Both can not tolerate standing even for 10 mins. It’s a v useful clue in differentiation between fat pad and pfp problems. Pfp imo rarely aggravated by standing.

Absolutely. Just been reviewing this literature for my session on adolescent knee pain on the MSc Paeds at UCL & it’s very clear that we should NOT see this as self limiting. @jvsimmonds01

[ADOLESCENT KNEE PAIN] w almost 50% of adolescents who had knee pain continuing to experience persistent pain into adulthood @MichaelRathleff et al concluded that when clinicians presented w adolescent knee pain 2 not assume it will resolve itself. IMO we need 2 do better here 🦵
