So it’s reinforced for me about how it’s worth talking to your patient when they’re unconscious, because as well as looking after their fluid balance or their ventilation, you might also be standing side by side with them at the prow of some ship, sailing through their dreams.

A while ago I started a new run of on-call shifts and went to briefly chat to an intensive care patient I hadn’t seen since he’d been critically ill and since he’d come off the ventilator. At the end of the conversation he said he’d been waiting to meet me. (1/7)

I am on strike because I want more money for doing my job. And that’s ok. There’s no gotcha in that, I’d like more money. It’s not shameful to ask for more money if you deserve more. And given its dropped 26% in real terms since I signed up to the profession, I do deserve more.

I was strangely affected by the whole thing. Like most people, I half-heartedly speak to unconscious patients in ICU when examining them or doing procedures, but have never met anyone that actually remembered.

He said that he’d been waiting to meet the source of the voice that had been in his dreams. That I was the voice of the doctor that had accompanied him on his bizarre visions, sailing ships across turbulent oceans, cutting through forests. Now he knew who that voice belonged to.

My hospital is finally (well, in a few months) ditching paper notes for a multimillion pound EPR system. I asked today how you search through notes. It turns out don’t, you just click through and read them all until you find the information you’re looking for. Sounds groovy.

I thought maybe he was confused, a lot of patients end up being stepped down from the ICU before they’re strong or well enough to have a meaningful conversation, a lot have delirium or tracheostomies or something. I asked though why he’d been waiting to see me.

I’ve started asking after putting in labour epidurals what the patients can remember of the risks we “discussed” whilst they were in agony, loads of people remember nothing. It’s pantomime legal consent, a little show I put on every night for the midwives.

This is great and I'm glad, but it's hard to look at his lovely office and not contrast it with how this usually happens, with a slow laptop on wheels with an impossible to use wipe-clean keyboard whilst you sit on a bin in the middle of the ward being interrupted every 2 minutes

A bleep I got today needs a new AA battery, but I can’t get it from theatres coz they’ve got a stationary budget freeze. It’s a switchboard item so I should get it from there, which means stopping work for 20 mins so I can change, walk through the rain and get one. For a battery.

Why is it that NHS staff are not allowed to work in spaces that look or feel pleasant? Clinical areas have to look clinical. But every office wall has to have 27 angry signs, the keyboards have to be unusable wipe-clean monstrosities, even the lifts scream propaganda at you.

Probably my worst #TeamNights weekend in terms of lack of breaks and surplus of stress. “Could this get any worse” I thought at 3am, and then all power to our half of the hospital died. “The generator will kick in, give it a minute”. Reader, it did not kick in.

The weirdest part of anaesthesia is how I’ll come in and not know what the consultant looks like, they don’t know what I look like, we just start seeing patients in pre-op until we randomly meet and figure it out. Half the time this happens in front of a patient.

Currently topping up this medicine year as an ST5 for ICM, we’ve tried to make it as useful and palatable as possible but still feels like a bit of a waste of a year. Would kill for 3m of echo and bronchoscopy. That would be training, what I’m doing is box ticking.

I think what’s important to remember is that the exchequer has saved billions of pounds over the last 15 years by screwing doctors out of a decent pay award, it’s only a big number now to restore it because of the scandalously large pay cut levied upon us over those years.

Even trying to be balanced though, in the run up to a massive exam the amount of knowledge you need to consume and the fact you consume essentially all of it in addition to a 48hr/week job means you have to give up your social life, your rest time. There's no way around it.

@NeddyFlanders11 Did I ask for a 24 hr service? I’ve been working 13 hour shifts back to back since Xmas Eve, I had half an hour yesterday to open presents and wolf down cold turkey before I went to bed. Half the staff we need are off sick. Why should I not be able to buy a hot meal for lunch?

Another shift, another equipment/drug shortage. Patient care affected again. It’s so insidious it never makes a newspaper headline but supply chain has been a continuous healthcare problem since a certain recent political event, I’m not sure if the general public know that.

Or if you’re in an acute hospital, put out a crash call, have an NP shoved down for some reason, ABG taken, ten people around the bed until someone has a good read of the notes and hopefully de-escalates everything. Repeat until discharged.

@stu4rtdavid I think a lot of doctors aren't tuned into this, but those of us who are agree with you and I can only imagine what those particularly screwed over are feeling right now. Medical training just seems to be getting progressively worse.

A few years ago now but the worst LGBTQ+ treatment I ever saw was of a trans patient in an ICU, persistently misgendered, trans status always mentioned at the start of every round, lots of raised eyebrows and snarky jokes. Didn't feel as an F3 I could call it out. I should have.
