The elevator fell from the 9th floor with her inside while planning to see her patients on the ward. This could have been avoided cos we have complained so much about this elevator and nothing was done. She was yet to get any hazard allowance like everyone of us.

LASIAMA claimed former MD Dr. Gani- Kale signed off but there was no written official documentation to that effect. 13. On the day of commissioning, there was issue of lifting of the elevator due to a power outage necessitating the need to be connected. THE LIFT DIDN’T WORK.

Let me put it out straight that we didn’t shut down the emergency unit of these centers. The labor ward and theater at LIMH were not tampered with. Patients on ward admission aren’t disturbed. There’s a pathway to walk into the hospital.

- The investigations found that the condition with which the elevator could’ve crashed from the 9th floor to the pit without the emergency bolt stopping include: - Suspension ropes had been damaged and this caused a snap of the ropes - 2 out 4 ropes snapped during the free fall

Overspeed governor mechanism failed (this is a safety device that acts as a stopping mechanism when an elevator runs beyond its greatest speed. It activates when the elevator exceeds its limit and contract speed, it triggers breaking system to slow down & stops d elevator safely
