My clinic's at a hospital where we screen for housing & food insecurity. Had a parent say "I'm frustrated that y'all keep asking bc whenever I say I need help you don't help me." 💔 They're right. Too often we screen for things we can't act on & that can do more harm than good.

I really had to beg repeatedly for my Black male patient, writhing in pain, to get pain meds. Then I proposed an IV Tylenol order (couldn’t take PO) & was told “we don’t do IV tylenol on the floor” only to see another patient get it a 2 days later. Healthcare is racist as hell.

Worked in the ED of a major Children’s Hospital today. We had TWO nurses my entire shift and they had stayed on from night shift. Idk if folks truly realize that shit is hitting the fan. Most ppl in this country are alive because of nurses.

In medicine, we compartmentalize. Yesterday, I was notified that a COVID+ family member was unresponsive & taken to the hospital. I broke down in front of my team. I ran out of compartments. Full from inpatient work & trauma from Wednesday's violence. This is not sustainable.

Also something abt resident clinic as the answer to healthcare disparities doesn’t sit right with me either. Residents are obviously trained drs but the limited availability/true continuity for ppl more likely to have complex healthcare & social needs feels like a mismatch.

Welp I avoided COVID for 2.5 years. Masking indoors with an N95 and outdoors in crowded spaces. Unmasked indoors with negative RAT only. Vaxxed x3. Didn’t want to share bc I feel like I have failed, but I need to reframe for myself that getting COVID is not a moral failing.

I got to see a baby I caught & resuscitated at delivery, cared for (for weeks!) in the NICU, for their well baby check and it was one of the most rewarding moments I've had in residency so far. It was nice to finally be reminded why I chose pediatrics. ❤️

It was my last day in the NICU and I got to hold one of my babies. Baby's mom took pictures and cried and I teared up. Baby medicine really is for me. I'm gonna miss my buddies 😩.

It's $332/mo for residents at my institution to park at our jobs. Doesn't include all the hospitals we rotate at. Several specialities, esp surgical have to go to multiple hospitals in 1 day & pay every time. We are unionized & couldn't get free parking in the newest contract.

It’s kind of wild that many PCPs won’t refill a chronic dx Rx they didn’t originally write for. Even if specialist retires or pt loses access. Even if pt schedules w/ new specialist but it’s after they’ll run out of their meds. It feels so dangerous.

Also maybe I’m doing this wrong but I don’t think we should refuse to give pain medicine to patients, visibly suffering, on the basis that they’re “not asking for anything.” Esp when we know what’s causing their pain. It really sucks to speak up for your patient and be shut down.

Want to stress that we give a list of resources but despite being at a public hospital the clinic doesn't even have social workers. I don't know that we should stop screening but don't we scrutinize ordering medical tests we don't intend to act on? Why not do the same here?

Please check with trainees/colleagues abt how we introduce ourselves to patients/families. Maybe it's bc I'm from the east coast but I'm shocked when attendings/fellows introduce me as Dr. Lawren IRL. As a young Black woman, I always use my last name and IMO that's the default.
