End of my shift today and the whole ward is Covid +ve / the nursing & therapy staff I worked with must be finding it tough, but remain positive & served the patients with such high professionalism and care - awesome
First face to face clinic today for months-looking forward to talking to a person-in person! I think I am slightly virtual weary. I love the changes we r seeing-so many positives but I have realised much of the enjoyment I get in this job is that direct interaction with people.
The Roadmap supporting FCP competency to Adv practice in primary care will be available next month - it’s multi professional and I am so proud it will be received this way
Just pulling together advanced leadership MSK role, 50% FCP, 30% rehab in a brand new rehab unit, 20% leadership- opportunities to contribute to physio and sport courses- FCP training on site as PGCert - a blended supportive role with that and more to come, - watch this space
Just amazing how teams come together, we as others have within 24 hrs converted all physio and MSK triage to phone consultations, staff offering extra hours, getting ready to c how we support frailty, community and ward staff, I lead 80 staff but definitely ONE team
Slightly different cohort of patients from MSK 😷 to all the MSK Physios getting involved with the brilliant ward and community colleagues- massive respect and well done!!!

4 pillars of advanced practice - worth thinking how they integrate rather than separate your work. Build every learning,new experience into a reflective model that allows you to link all four as a whole - every new element to practice can link to these in different depths.
Started mentoring new in post FCPs yesterday - it once again reaffirmed the complexity of the role, the need for mentorship a programme, validated programmes of study & not placing inexperienced staff in these roles

Observation 2: When you go for a FCP role make sure you ask; 1) who holds the governance for the service and how does that look? 2) who is providing my supervision and do they have the capability? 3) how do you see my clinical development being supported?

Had a good day on the wards today , working with some great staff, still trying to keep an urgent MSK service , staff showing great resilience and flexibility and support. We are getting ready for next week - tough time’s ahead but sticking together.
Discussed adv practice with gr8 MSc students today adv practice is not (IMO) requesting MRI etc it’s quickly establishing what’s needed,building a plan, delivering person-centred care,clearing scary stuff maintaining responsibility&de-medicalising effectively
Planned FCP modules via e-learning will be-intro to primary care, identifying illness/frailty,complex reasoning, public health,mental health,persistent pain, pharmacology governance,serious&inflammatory pathology via person centred language-coming soon!
Observation: If you are a FCP not employed by the local MSK service perhaps employed by a private company - make it your first job to make contact with the local service,get clear pathway guidance, build working relationships, and integrate across the system - leadership pillar
just read this. To all that read stuff like this-is this really what we want UK physio to look like? Irrespective of differences of opinion-this isn’t right is it? It can’t be,or am I an out of touch dinosaur? We can do so much better than this, can’t we?

Learnt about the “rotunda” “Sara steady “ and got my “Pat slides” sorted - thank you the team at Romsey hospital for helping me as not been clinical on a ward for 24 years! Enjoying the new environment
Going through my MSK list on the phone and patients are very understanding- self managing , explaining natural history , appropriate triage - closing lists down so can get ready to re deploy
Delighted to have contributed to this amazing text . A fantastic book which really covers so much contemporary evidence and immediately will inform practice !
Got back to work half days this week, increasing next week,possible re deployed the following week,family member also NHS for 25 years hitting 8 weeks post Covid & fighting to get back.I keep seeing the inspirational stories across all sectors & it is these that keep us going 👍
For any integrated MSK service - primary care, physio, ortho triage, pain management etc this editorial encapsulates perfectly the “what now” in how we need to reconsider and redefine our service provision / awesome work and commentary

So, finished first out of hours FCP clinic, back pain, knee pain, shoulder symptoms, hand and wrist, loads of advice, lots of worry so lots of education, broad skill set needed,sensible advice key, next door to adv nurse and GP,corridor discussions valuable !

With the advent of long waits 4 secondary care in orthopaedics- time mayb 2 consider sum possibilities 1.Natural History becomes a big factor 2.NHS MSK stops being part of a pathway & becomes a model of long term care 3.Self management becomes more acceptable 4. Pts r never d/c
So, new clinic arranged starting next Tuesday - out of hours, 17 practices can directly book, FCP in local hospital providing access for primary care. Full access to GP systems, PACs,and diagnostics. Exciting development
Just back in New Forest after Consultant meeting / good discussions on Consultant Capabilities framework, work going on nicely, look out for the ACP developments as well - wish I was starting out again as the career map will be a lot clearer
Just started new role in current post , looking to link ACP governance across the multi disciplines and professions in my organisation whilst linking it to work force planning - now I have to work out where to start !
SIG groups- there should not be a "special" bit, get MSK groups linked by commonality, quality standards, drop the philosophies and drive evidence,stop describing care based around single clinical models-integrate not specialise.
Just beginning some scoping work as part of work for HEE looking at how nationally we operationalise the ACP/FCP frameworks into a pathway through practice 4 MSK. Keen to discuss with anyone who has looked at the frameworks and wanting 2 build them into practice .

IMO Advanced practice is not about requesting scans, referring to surgeons etc it’s integrated understanding and reasoning with good underpinned evidence providing person centred care locally and at scale -this means across the whole MSK pathway

Communication skills , reasoning, good rehab advice and the ability to reassure based on knowledge - mix it up with being nice and supportive and that should do just fine
More and more Adv practice frameworks emerging - to really build a mobile multi prof workforce it will be key to ensure that clinicians can map CPD not only to one framework of current interest but across multiple ones to help broaden scope as well as work specialise

I have 2 roles one lead and one other - it’s a split FCP and rehab role in a new rehab centre-to be opened later this year -this is our MSK role - lead role in next few days.We also can supply a PG Cert MSK FCP on site at the uni - . + Opportunity 2 lecture
Just taken a new FCP through MSK capabilities document and was able to use PCA modules and MSK modules to help portfolio evidence large chunks of it - really pleased to see it in action . it works in many on the domains!
It’s day 9 of self isolating - 3 teenagers handling it brilliantly, dad going stir crazy - thankfully mum slowly recovering - even when you are fit and well it hits you very hard.
Just pulling together presentation 4 NHSE/I regarding building CPD in day 2 day practice that maps 2 ACP\ MSK frameworks. 2 have a “CPD passport/road map”that transfers intra/inter prof and drives 2 ACP in terms of language I Hope is what is needed - big year ahead .
Tough times ahead.Plans in place 2 enhance MSK practice via phone call,online support & triage underpinned by safety model. Moving staff where needed .Re-defining what we mean by physio contact. Cancelled gym sessions & with my two sons setting up our home work out for future.
The one sport I had left - my son is quicker, stronger, lifts more, - I still had golf - club championship yesterday/ beats me by a shot - I thought it was all over; it is now 🤨
Nearly done - a few more patients to see. I want to wish my friends and colleagues a great Xmas and 2019, to also the individuals I have had the pleasure to chat to on here, thanks - it’s been incredibly helpful and I appreciate the time taken, all the best
Not sure is I am looking better or seeing more but clinics certainly seem to have greater complexity dropping into them as compared to pre Covid - just shows adv practice should be across all specialties and not limited to escalation pathways
Proud day , been coaching a youth football team for 7 seasons and we make our first Bournemouth cup final after playing great in our semi final today - most importantly the referee commended our lads on their behaviour and attitude, game played competitively in the right way.
I wait to see how our work around the MSK RoadMap for primary Care is received next week. It’s been fascinating 2 c how different organisations have so far responded - what’s more important - Setting and standard or ensuring AFC banding? Seems to be a spectrum of opinion on this

Been involved in youth football for 10 years. Why do some youth managers think it’s ok to shout at young referees - being “ passionate” is no excuse as there is no excusing being aggressive at a kids football match- so sad it still occurs -
Reviewing interface MSK service and my thoughts are currently around my service is to think away from being the pathway model. Orthopaedic waits and good conversion to surgery figures do not reflect a contemporary MSK care offer. LTC in complex environments needed 1/3
