I performed cesareans of two covid positive patients yesterday. Both stable and asymptomatic. Both mothers and babies doing well. Cesareans take 45 min to 1 hour max. Even then we almost collapsed in the PPE. I wonder how longer duration surgeries are being performed with PPE.

100% of pregnant Indian women delivering in public (govt) hospitals do not have access to pain relief options in labor - many require it for medical reasons like epilepsy, but the system is not in place to provide it. Many suffer traumatic births, PTSD due to it. #painfreelabor

If I counsel 100 Indian women with completed families, I can successfully persuade 80-90 of them to choose tubectomy. If I were to counsel 100 Indian men with completed families, I wouldn't be able to convince even 5 for a vasectomy. Takes 2 people to make a a baby.

My father has been on high flow nasal oxygen since 18th Nov. He has tested negative but is still dependent on the machine for his oxygen requirements. This virus causes respiratory failure. It badly injures the lungs. Please do not take it lightly.

@Kamal_t Please leave this country. 😃 Normal delivery takes upto 3 to 4 weeks to heal. It took 3.5 for my mother. Whereas it took 7 weeks for my aunt who went through C-Section with loads of medicines. We can see what profit making game going on!

Firstly no one should be made to bear labor pains in this day and age - EPIDURALS FOR ALL! Secondly, no, cesarean is not done just to save lives - cesareans can be done on maternal request too given the horrible and unforeseen consequences of so many vaginal deliveries.

Vasectomy rates in India remain woefully low compared to tubectomy (female sterilization) inspite of it being a much simpler and cheaper procedure. Female reproductive system is intra-abdominal, whereas males have their testes hanging outside - vasectomy can be done under LA.

I often come across cases of post-coital bleeding in my practice. Newly married women- first wedding night - big deep lateral vaginal wall tear. I think there is a lot of work to be done to educate India youth that porn is not sex, sex should not hurt, use lubrication, technique.

Maybe the neurosurgeon should try to repair a complete perineal tear once to see what women are left with after traumatic vaginal deliveries. Anyway, pain is not something that women have to bear EVER - whether in deliveries, sex, menstruation, or any chronic pain.

The headlines in India are misleading. It is not pvt labs that are "charging" fees for these tests. The tests have a cost and the lab technicians doing tests need to be paid. The govt has refused to "bear" the costs of the tests done in pvt labs. That should be the headline.

@dr_somyagupta Evn lady doc who looked after my mother & kept insisting for a C section, said it’s a normal delivery so 2 to 3 wks of rest should be fine. It would be long if it was an operation to let the stitches heal. You really need to cross check what postpartum recovery period is. 🙂

Went to a village health camp today. Things we take for granted - essential medicines, pathology labs, ultrasound, X ray, eye tests - no such facility for miles around. Not even a simple cream to treat scabies. Not even a urine pregnancy test. This is where 70% of India lives.

Adequate pain relief during labor can even improve utero-placental perfusion and decrease the incidence of postpartum depression and persistent postpartum pain. The pain relief also blunts adverse cardiac, respiratory and GI side effects of catecholamines.

Good universities, teachers and Delhi medical council made me a doctor. No rest is recommended after vaginal deliveries, Mr "Avid Reader". Women can and should move about immediately after vaginal deliveries and 6-12 hours after cesarean sections.

The new India-made #cervavac vaccine can revolutionise the prevention of cervical cancer in India. It is expected to be a much cheaper (Rs 200 per dose) alternative to the currently available Gardasil and Cervarix which can cost upto Rs 3000 per dose.

A very fine needle is used to put a thin tube into your spinal cord, without entering any bone - as the vertebrae in the your backbone have holes through which this needle can pass, and the pain-relieving medicine is delivered via this tube into the “epidural space”.

The spinal cord has 3 layers - dura, arachnoid and pia. The space above the dura is known as “the epidural space”. Contrary to popular belief - receiving an epidural during childbirth doesn’t predispose you to develop osteroporosis or back pain later in life.

Almost all of those delivering in small nursing homes and delivering at home will also come under this group. Women delivering in big corporate hospitals have access to good analgesia - a very small minority of women. There is a big unmet need for labor pain-relief in India.

Cervical cancer screening is started at age 30 in India. Pap smear testing and HPV testing are both methods for screening for cervical cancer. Women are advised to get papa done every 3 years till 60-65 years starting at age 30.

Ideally, every girl & boy should get 3 doses of HPV vaccine by age 14. And, cancer screening started by paps by age 30, every 3 years till age 60. However, recent studies say, even a single HPV vaccine dose till age 45 and a single lifetime paps is better than none at all.

We need to disconnect medical treatment and costs. At point of care - discussions should be about diagnosis, evidence-based medicine and not ICU charges, tests charges, medicine charges. Costs should be borne by the state - we want healthcare not temples, statues.

Patients come with heavy bleeding sometimes- lose upto 500 mL blood, need blood transfusion even. A little education can prevent a lot of regret, shame, anxiety, morbidity. Vaginal walls are extremely vascular- not to be toyed with!

As for cancer, emergency contraception is meant for emergency use only. It is better to use more effective contraception like OCP and condom. I pill usage doesn't cause cancer in long term as it is usually used in emergencies only. OCP use DECREASES risk of ovarian cancer

Some General Surgeons in India - who want to do private practice - try to master cesarean deliveries and hysterectomies as it is much easier to get Indian women on an OT table than men. Indians are very sceptical of surgeries and avoid surgeries unless life-saving. Discuss!

The way progesterone is prescribed for first trimester bleed in India - oral, per vaginal, intramuscular -soon we will have progesterone eye drops, progesterone patch, progesterone body spray!
