Saw a 19 yr old girl with hyperthyroidism. She's had it for over 2 years now. Asked them what was done. Told me they tried acupuncture for over a year. Asked them what was involved🤔 Was pricked weekly. Cost? 50 K😬 Then tried homeo x 1.5 yrs. Finally homeo doc said to see endo!

Seeing 4 patients per day At 1000 Rs per consultation Working around 6 days a week That's 24 days a month. Translates to 96K per month revenue generated in simple math. 80% of that translates into roughly 77k per month. Enough to break even with my expenses to not use savings

Lady in her 50s. Was seeing a diabetologist. Was on glibenclamide 10 mg/metformin 1 gm, sitagliptin 100 mg and dapagliflozin 100 mg daily RBS few months ago was 198. To save cost went to govt hospital. Seen in a busy OPD by someone. Put on metformin 500 mg once daily. Now👇

Uncontrolled blood sugars in 200-300s will cause gradual worsening of creatinine. Certain BP medications can cause slight elevation of creatinine. Those on SGLT2 I may have transient bump in creatinine but gets better over time. Good in long term. Metformin needs renal dosing.

@AdamMGrant @kshockles @ProfASGabriel @nitya_chawla @BroadPathHCS Think having the autonomy to decide when to turn the camera on and off may make a huge difference to the degree of fatigue one experiences. It would have been interesting to see a third group which had that autonomy and the effect on fatigue/ engagement 🤔

80 yr old man brought to casualty with severe hypoglycemia despite IV dextrose. Not a known diabetic. RBS 45. Insulin and C-peptide sent during episode of hypoglycemia. Cortisol level sent at the time 19 Thoughts?

Back in the US, was used to changing my own flat tyre, fueling my own car. Painting my own walls. Filing my taxes on their own, renting my own place and buying my own car when I was 25 :) And now at 47, it seems like having moved to Kerala, an overprotective society cut my wings

Being raised as polite, obedient boys and girls who struggle to speak up for themselves can be injurious to health when they grow up. They will suffer in silence. End up in depression or burnt out... I'd rather savour what's left of my life. Enjoy each moment to the fullest.. ❤️

Unfortunately diabetes in India is NOT a problem of the elite alone. The increasing rates of DM, cirrhosis, PCOD, infertility, HTN, depression, anxiety, stress, in non-smoking non-alcoholic women who work hard is concerning. Just supplementing micronutrients solves the issues?

The amount of time we spend in med school on learning stuff we’ve no practical relevance for, in clinical practice, which majority will eventually take up🙄. Far smaller proportion become academics & researchers. We pride in retaining information and don’t value sense making🤦🏻‍♂️😒

A TSH of 6.5 in an obese patient may be a thought of as a part of metabolic dysfunction. There is a possibility of it normalising when the person looses weight. A TSH of 150 in any patient is NOT a lifestyle disorder and no diet of any nature will reverse this. Needs thyroxine!

Asked her how come she didn’t see her diabetologist? Said she was scared to go see him as she was sure he’d yell at her. Asked her, u do realize that when u are so many medications to keep sugars 180, cutting that down would raise sugars unless u cut down what u ate too 🤔

To be fair to most doctors they don't realise how powerful an intervention nutrition can be. If we looked at just diet interventions in a drug naive patient in say 2 weeks, it would beat most drugs we have today by a very wide margin. Add to that the benefit on BP, fatty liver!

A carb heavy, protein poor diet lacking in B12 in mothers a generation ago may have sowed the seeds of IR, metabolic disease, visceral obesity and cardiac generation for the present generation in our clinics. Hope we can do better with the next generation of mothers. 😏❤️

The unfortunate misconception most parents have in India is that too much of non veg is bad. Even eating > 1 egg a day is scary. I don't blame them. That's what they've been traditionally led to believe. Sad they believing having a large plate of rice at all meals is benign. 🤔

In the past 2 months since I became joint secretary of my apartment association, I’ve noticed -Increased Hair loss -Reduced sleep -Rise in my HbA1c from 5.7% in July to 6.0% despite eating a lot lesser and working out a lot more. -My SGOT and SGPT SGOT up to the 60s.

Increased fat in the liver—-> Increased insulin resistance. Increased fat in the pancreas —-> Decreased insulin secretion. The double whammy of visceral obesity. 🤔 And unfortunately neither a BMI nor a WHR may reveal the problem. Don’t wait to get disease to live healthy.

Dad handed mom a bag with a glass dishes. But in the process of transfer the bag fell & the glass dish broke. Mom was upset. Junior noticed and in an effort to save his grandfather from his grandmother's wrath, said, "Is the glass dish more important or the relationship?" :) :)

@amitonline74 I am an endocrinologist but with a personal learning of nurition as Diabetes and Metabolism is an integral part of Endocrinology :). Can’t outsource everything as patients get such mixed and conflicting instructions. My patients are often lower middle class and cost matters :)

Today it's been 9 yrs that I wheeled him out of the hospital after his cardiac bypass... It's his birthday today and this morning on my way to work I stopped by to wish him. All I could manage was a deep long hug. No words would come out. But guess the feelings understood ❤️

Known diabetic for 15 years. Largely well controlled. HbA1c in March 2023 6.2% Creatinine 1.2 Creatinine now 4.4! Point to be understood? Every high creatinine in a diabetic is not diabetic nephropathy. Usually retinopathy precedes nephropathy. Sudden creat ⬆️?

The one thing I found the most useful in managing a difficult patient, something that I learnt during my residency days was to pick up the phone and talk to the specific consultants. It was a best learning experience. Something many may not feel comfortable in India, but so key!

Explosive parent? Breeds either children who are just as explosive or ones who become people pleasers, the ones who are constantly walking on egg shells, or ones who are passive aggressive or manipulators. Dysregulated nervous systems that are corrupted are bloody hard to debug😞

Junior asked me why I looked upset. Told him so much of our mental health struggles come from not being able to speak up and have uncomfortable conversations. We often would like to avoid them, or end up getting mad. He said, "Can u please teach me how to have them." Hugged him❤️

The biggest misunderstanding about nutrition is that ‘fat’ makes u fat and ‘protein’ will wreck ur kidneys. Why? Because the first kind of sounds obvious. But as for the second, it’s the thought that if protein is in ur urine it must be from eating too much:) #Unlearning 🤔

When I see a 12th grader who’s over 130 kgs,referred to me by the psychiatrist, for a TSH of 6.3, I realize that something is majorly wrong with how we are going about addressing the obesity problem. As an endocrinologist, there is so many things that’s going through my mind😔

In any relationship, be it marriage or friendship, when one person starts to grow and the other doesn’t grow alongside and stays stuck, the emotional distance in the relationship will be very quick. And that will lead to either conflict or resentment depending on who’s affected.

@baxirahul Tell my patients that I am also sitting in my chair most of the day. Occupational hazard. :) Why I decided to move to 1-2 meals per day and no snacking. It will be impossible for me to outrun my mouth :). Also our personal fat thresholds are so unique. Then they start thinking :)

48 yr old lady with diabetes. Husband refuses to give her money to buy her medications for diabetes. Ended up with a UTI. Got son to secretly buy her medicines. HbA1c 7. Weight 5 kgs down on her effort. What makes some people this way. How do people survive such marriage? 😒😏

Discussing the concept of why protein before carbs with senior. :) He told me if he ate the eggs first he’d feel full & wouldn’t be able to eat breakfast:) Told him that was the point! :) Also proteins kind of blunt the sugar spike from the carbs & so reduces insulin spike :)

Topic Personal Fat Threshold. Higher fat in liver causes high fasting blood glucose. (Insulin resistance) Higher fat deposits in pancreas causes reduced insulin secretion after a meal (relative insulin deficiency) And reducing the extra Visceral fat can help Diabetes remission

@abeejuice Unfortunately most pts feel afraid to discuss costs with the doctor. Personally it's the first thing that is on my mind. Cost of care.

No patient ever reversed diabetes eating the same amount of carbohydrates they did when they were a diabetic. That being said it cutting out carbohydrates completely may not reverse diabetes in ‘everyone’ either. No 2 patients are the same. #RandomMusings

My OPD staff. She looks out for me. So grateful. Quietly got me coffee from her own money. Asked her why? Told me because she didn’t see me leave the room to get one :) Little gestures of kindness❤️🫂 What more can I ask for❤️

Best way I can describe Type 2 DM is a process of slow burnout. Onset depends on the reserves of beta cells we have to start with. Genetic to some extent. Maybe made worse by fatty infiltration of the pancreas as we now understand. Burnout hastened by diet/stress,inactivity.

Asking inpatients simple questions like.. -Did u sleep okay last night? -Were u in pain of any sort? -Did u eat okay? How's the taste in ur mouth? Ur appetite? What did u eat? Seems to make many faces light up, and has been a deeply enlightening experience for me too :)

I didn’t get into MBBS in my first try. Well I did get engineering without trying for it but got the 2nd and at the Manipal entrance. The fees were too prohibitive though as my parents were living hand for mouth. Moreover all of 11th and 12 th grade I had all house chores too :)

Just a regular day. Called to see a pt post surgery. RBS was 300 before emergency surgery. The now super proactive surgery team :) I'm so proud of :) sent the HbA1c. It's over 13%... #RealLife :)

Waiting for everyone else to get here :) The boys unpacking… Without even being told to.. Junior started and I guess senior then felt conscious :) Hmmm… Observing how influence can be positive too without having to say things out specifically… Not complaining😂

The presence of someone who cares deeply is under appreciated in mental health struggles. Just someone, anyone. Who’s there to listen, to reach out to, and we are struggling, to help us hear ourselves is invaluable. I’ve learnt to be my own therapist. Listening to myself :)

March 2020 When CoVid hit. And technology made huge strides . When I used the time to create a new telemedicine platform :) Was very different from the everyday grind but I absolutely enjoyed every moment spent creating the product and testing it :) #CloudClinic /Beato
