The RPA is a specialty medical assoc. with over 4K members serving the interests of nephrology practitioners in pursuit & delivery of quality renal health care.
So the ASC rule came out on a Friday MORNING, and the rule states that CMS will not implement the proposal to cut the two high volume dialysis circuit cuts, so by all appearances the deep cuts for 36902 and 36905 will not occur for 2019. This is a big victory.

On the advocacy front, Dr. Schmidt had a LOT to do with the bills that allowed outpatient AKI care and that will promote use of telehealth in dialysis in 2019.
Nephrology was added to the list of specialties with access to the complexity adjuster when (if?) it’s implemented in 2021. Nephrology was excluded from this list in the proposed rule.
Congratulations to Dr. Alan Kliger, a Clinical Professor of Medicine at the Yale School of Medicine, on receiving the 2022 Jenny Kitsen Patient Safety Award. Dr. Kliger was a previous board member of RPA as well as past president of RPA. Job well done.

RPA PAC met with House Speaker Nancy Pelosi (D-CA) this AM, discussed surprise billing & prior authorization legislation, as well as women's health issues. Reps. Ami Bera (D-CA), Kim Schrier, M.D. (D-WA, the only woman physician in Congress) & Brad Schneider (D-IL) also there.

When CMS posted the Hierarchical Condition Category (HCC) risk scores in the 2018 QPP prpsed rule, nephrology was the only specialty w/a score >3.00 (3.05). 5 specialties had scores of 2.00 or >(ID next highest at 2.35); all other specialties listed (66) had scores of 1.95 or <.

So, the three Medicare final rules of consequence to the kidney community (doc fee schedule, ASC payment, ESRD PPS) are all now at OMB, as of Monday. Major issues to watch include compression of E&M codes in the fee schedule, and whether changes in documentation requirements (1)
On a serious note, RPA will be vigorously pursuing advocacy efforts to reverse CMS’ proposed definition for 2023 of ‘substantive portion’ of split/shared services so it can be based on medical decision making and not solely on time.

The final rule on the 2019 Medicare Fee Schedule will be out around 11/1, & that will answer some questions on dialysis & telemedicine, with more regulatory guidance expected later in the year. RPA will be reporting on these issues in real time as they develop. (2/end)
Until 1/1/2019, it's hard, b/c the patient's home and dialysis facility are not approved originating sites for telemedicine in Medicare. Will be much easier after 1/1, home and DF will be approved, with no geographic restrictions. (1)
