Pharmacy schools should put a phone at the students desks during exams and just have it ring constantly. The proctor should also ask random questions every couple of minutes. That's real world prep.
Being a retail pharmacist has made me a better customer outside of pharmacy. I'm not impatient or rude if I have to wait at a restaurant or checkout lane etc., or if something goes wrong. Anybody else feel that way?
Imagine being a doctor and running the whole clinic by yourself. The phones, billing, patients all fall on you. As a bonus you have to ring up groceries.
That's what retail pharmacy is like many times.
They should do pharmacy like the hibachi restaurants. We count the pills in front of people while doing tricks with the spatula. We sling xanax samples in people's mouths. Would be awesome.
If pharmacies want pharmacists to be clinical, companies need cashier's for all hours of operation. I can't spend 2 hours straight on the register because of budgets, then be asked where my MTMs and compliance calls at.
You can't be a clinical pharmacist in a retail setting filling 300+ a day with one pharmacist. But that's what is expected of pharmacists. It has to change.
Why the fuck does a mail order pharmacy call for a transfer then put me on hold to wait for a pharmacist? Fuck that. I'll keep hanging up on them. Have the pharmacist call and be ready for a transfer if you want it.
Had an escript for 300mg bid lovenox. Called physician to verify. With a snarky attitude she tells me it's MG per KG do the math. I asked how much the pt. Weighed. There was a long pause. Then she apologized and said she would resend.
If every pharmacy paid the exact same to get medications and were reimbursed exactly the same, and people could choose where they want to go....how long would it take CVS and Walgreens to go out of business?
Me two months ago: No!!! You can't have the vaccine without an appointment! Make one online, we are booked.
Me now: please show up for your appointment.BTW do you have any family members that want it? Has your cat or dog been vaccinated?
When I call for transfers, I can tell by the way the other RPH is breathing what kind of day they are having. CVS RPhs always out of breath. Makes me sad.
Started wearing a t-shirt and jeans to work because I figured there is zero chance corporate would actually leave their house to visit the stores. It's too uncomfortable to wear dress clothes and PPE.
The $8 we pay goodrx for every rx is why nobody answers the phones in the pharmacy. It's why it takes so long for your prescription to be ready. That $8 per Rx times 30 is 20 hours of tech help daily I don't have anymore. Glad people are saving money though.
Retail pharmacy is like simultaneously taking 7 midterms. Except you can't miss any questions. And people are yelling at you. And phones are ringing that you have to answer. There is no way school can prepare you for this career.
This is clinical pharmacy in the retail setting in a nutshell:
Dear prescriber. Everything you're writing for this patient is wrong. Can you change it so we can get $5 from outcomes and so I won't get written up? No? Ok thanks.
One day, when I have enough money saved,I'm going to open an independent pharmacy. Then I'm going to do a reality TV show so people can see all these crazy stupid stories are true.
I don't care what PBM / insurance it is, no pharmacy should be paid under $10 profit for any drug that's at least a 30 days supply. Pay us for our labor. Paying me 15 cents for Meloxicam and 5 cents for amlodipine is bullcrap.
A month into owning an independent pharmacy and DIR fees are absolute bullshit. You think you made $20 on a script only to find a month later you lost $180 after DIR fees. What dumb mother trucker made this legal?
Giving technicians more responsibility is a good thing. However somebody else(likely the pharmacist) will have to step in to do what the technician was doing beforehand. So really nothing changes without increased labor budget.
Corporate just admitted they can't profit with good Rx, i.e. they finally did the math. You mean paying an average $10 transaction fee on a discounted close to cost transaction isn't profitable?
How are pharmacists going to have time to give vaccines, and then wipe all surfaces down between each vaccine? I don't think corps realize what they are asking. Getting anxiety.
I'm not making this shit up. You can't. For the last 30 days the same woman has come by daily to ask me or other RPh if they use fetuses to make the J&J vaccine. Can I say yes next time just to see her reaction?
Customer: I don't want you to notify me when my Rx is ready because I'm a teacher and I'm not supposed to look at my phone at work.
Me:ok I'll take notifications off
Customer: So how will I know when my medication is ready?
Me: We will use a carrier pigeon.
Why do people tell me what their medication is for? And they are almost always wrong.
P-Atorvastatin is my calcium supplement
M- No that's for cholesterol
P- Then why does it have calcium in it???
Checkmate
So half the people for vaccine appointments are showing up on the wrong dates. Then arguing about the dates. I don't know why this is hard. Pick a day, know the day.
Get rid of bloated upper management and increase labor budget. Then pharmacies will meet the budget. Paying lots of money for "managers" to bitch about numbers, while cutting tech help, doesn't seem to be working.
When me and my partner took over 3 months ago, pharmacy was averaging 30 a day and 60k sales/month. We are now @ a little over 100 a day and 300k sales and still growing. In two years we will be a household name in our area. I won't settle for less.
ARNP complained about being on hold for 10 minutes to call Rx in.
1. Was giving a covid vaccine.
2. We have a got dam mother trucking voicemail for a reason.
3. Do I get to talk to prescribers when I call?
Yes I said those things, politely.
This Karen Lynch, CEO of CVS makes 20mil+ a year. Meanwhile independent pharmacy owners can't even take a paycheck because of all the DIR fees they are paying. Must be nice to make the rules and pillage off of the people doing all the work.
I chose pharmacy so that I could be paid because of my education and knowledge. I wanted to avoid the strenuous labor I saw my parents endure. Didn't turn out how I planned.
This is why pharmacies are closing. Why hours are cut. Why customer service sucks. This is donepezil. Our cost is 80¢ and the insurance pays us 70¢ with no dispensing fee. Shouldn't be legal. Pharmacists matter.
Don't open boxes of insulin pens.
Open the boxes of insulin to give exact days supply.
Don't open boxes of insulin pens but call the insurance every time for days supply override.
To be continued
The greatest myth in pharmacy is that pharmacists can increase patient compliance. It's just a convenient way to shift blame and make more money for insurance companies.
Cardinal Health stock up after great earnings. CVS earnings per share up too. How is your local independent pharmacy doing? How is your CVS pharmacist doing? See the problem?
The hardest thing in the world is getting patients to understand that February is a short month, therefore the date their narcotics is due is different. It's impossible for them to comprehend.
My favorite thing is when a new customer brings me a prescription and I turn it down because I'm losing $200(ozempic). Then I get a call from the ins. company and they have the balls to tell me I have to fill it. There is no law that says I have to lose $
I'm not scared of getting sick with the Corona virus. But I'm terrified I might bring it home to my wife and children. We need to protect retail workers.
Retail wouldn't be so bad if every single freaking customer didn't think they were the only customer. Even after telling them 5 people are ahead they proceed to explain why their situation should make them first.
So goodrx now tells patients that want a transfer it will only take the pharmacy an hour to transfer a med and have it ready. They are trying really hard to be the most hated thing by pharmacist.
Pt: one of my pills looks nothing like the rest, should I take it?
M: That's terrible. What does it look like and no you shouldn't take it
Pt. It's a big cylinder shaped pill and it says do not eat.
M. Sir that's not a tablet it's the desiccant throw it away.
Pharms(retail) when laws grant the right to perform an additional healthcare service: "We don't have time, this will just be another metric"
Pharms(academics)when doctors say we can't do it: "How dare you! We certainly can! We need more responsibilities."
I like how impatient most people are when they have a question for the pharmacist. If you don't run right over there they march off. That tells me they wouldn't respect my opinion anyway.
There is something psychological that happens to me as a business owner when I am continually taken advantage of by rich evil people. It makes me so angry. All I want to do is help people and make a little doing it. Not lose my ass trying to help.
Fill a migraine medicine ordered 1QD prn for 30 days
#20
. Goes through insurance no problem. 6 months later insurance demands documentation of every migraine the patient had signed off by the doctor or we owe them.
How can we get doctors to stop sending patients straight to the pharmacy after they send an escript? It's fucking irritating. It doesn't work that way.
There is no wholesaler in America that will sell you brands at WAC -15%. So in essence, express scripts has made it mathematically impossible to profit on a brand for 90 days. Why would a PBM that owns a mail order pharmacy do that?
Patient wants Norco 3 days early.
Me: Are you taking 4 a day and no extra?
Pt: I never take extra! That's offensive. I take it how it says.
Me: Then you should have enough until it's due.
Pt: Sometimes I take an extra one.
True story.
It's important that patients understand that I really really, from the bottom of my heart, don't give a fuck whether or not their vaccine appointment time is convenient for them. They chose that time not me.
Patient picked up an Advair HFA inhaler. Comes back 30 days later.
"I've been using this every day but it's broke now, nothing will come out! Can you fix it?"
God bless America.
The depressing part about pharmacy reimbursement is all we are asking is to not LOSE money on brands. What business model in the world would just be happy to break even? Just show me
@ftc
what they pay themselves. That's all I need to know.
Corporate says leave big thing of hand sanitizer on counter so customers can use it. It's stolen within the hour. Now we have no God damn sanitizer either. Cashier has no idea when it happened. Lol
If you're having a hard time finding

and

don't bother calling independent pharmacies. We aren't going to order it because every insurance pays us $100 under our cost. If your independent ordered it for you they must really like you.
So when flu season is here we will basically be doing a minimum of 50 vaccines a day. Other chains will probably do even more. When are we going to verify?
Any family members harassing pharmacists about getting masks? If they are sold out, I don't keep a box hidden for family members. If I had hidden boxes I would be selling them on eBay, not giving them to relatives I haven't heard from in years.
I get 100s of calls every day for wegovy ozempic etc. The potential for rapid growth is there but I'm not going to order something I lose lots of money on.
I can't even make money on azithromycin lmao. Insurances reimbursing 80 cents. I'm still a greenhorn independent owner so this crap is still shocking to me. Why don't all the owners have pitchforks?
I kicked out so many patients today. Showed them how much we were losing because of their insurance and they can't believe it. They cried because they didn't want to go back to CVS or Walgreens. Tried to get other Indy's to take them but they refused(cuz ESI)
Copies by phone from pharmacy to pharmacy needs to be outlawed. It takes too fucking long on hold and there are too many opportunities to fuck it up by hearing things/reading things wrong. Have your doctor call your shit in to the right place the first time.
Need 15-30 minutes per child covid vaccine. First I have to calm them down. Then I have to get them to sit down. Then wait for them to stop getting up every time I get close. I refuse to rush it, regardless of the all the other shit I have to do.
