In a previous discussion about endoscopic necrosectomy I mentioned I tell my pts to drink plenty of papaya or🍍 juice. Here is why. Big chunk of necrosum removed with no resistance. Enzymes soften the tissue and make removal easier. What are your tricks?

Here a very patriotic 13 cm rectal LST G w/ 3 dominant nodules(notice resemblance to the 🇺🇸 map). Interestingly only HGD was identified. We didn’t have a big enough styrofoam piece so we had to tape a few together.
Let’s stop calling it “advanced” endoscopy & ourselves “advanced endoscopists”.
It brings a false sense of superiority over our gen GI colleagues to our patients.
We are “interventional” endoscopists. We are not superior to gen GI MDs, we just do a different kind of job.
Why do nurses, PA’s, AA’s get mandatory breaks throughout the day?
The answer I always get is: “to reduce burnout and ensure pt safety.”
Yet somehow, MDs breaks don’t exist. From an admin perspective, MDs are machines who simply never burnout or make mistakes.

EVERY interventional endoscopist should be comfortable with 💉 abdominal decompression.
Last week I did an ESD in hep flexure & had minute perf.
However, pt developed tense capnoperitoneum. 💉 decompression allowed us to decompress abdomen & complete the case.
Received several PM saying this is not a professional pic. Agree. It was meant to be funny.
One has to be able to take things lightly and laugh at the simple things.
Life is too short to take EVERYTHING seriously. Even ourselves.

Why are there so few Hispanic/Latinx IMGs in academic GI careers?
1/ Immigration issues
-Most come with J1 visa which limits their program pool. Recent survey of IM program directors 👉🏼 41% seldomly rank IMGs and 20% never do so.
In our recent publication we report for the first time how patients exposed to NSAIDs prior to their acute pancreatitis episode had a significantly lower rate of organ failure and in-patient mortality in a large retrospective cohort.
