Taken to the OR by @HaniMufti today for evacuation of Hematoma. Minimally invasive Thoracoscopy approach (VATS) through two 1.5 cm incisions. Almost all of the hematoma was evacuated and large drains placed. We could see the ADOII device outer disk outside the aortic wall!

١١ نجاح الجهاز وشفاء المريض يعتمد، بعد مشيئة الله، على معايير ومقاييس منها الحالة الصحية العامة للمريض قبل تدهور حالته ووجود امراض مزمنة تؤثر على حياته، او وجود خلل في أكثر من عضو حيوي غير قابل للإصلاح او وجود موانع لاستخدام مسيلات الدم، او وجود مرض لا يرجى برؤه.

٨ بعد ذلك يتم ارجاع الدم للمريض عن طريق مضخة (القلب الصناعي) لإعادة الدم لجسم المريض عن طريق أنبوب كبير (يصل قطره ما بين ٤ الى ٨ مم). وريدي في حالة استخدام الجهاز فقط لمساعدة الرئة او شرياني في حالة استخدام الجهاز لمساعدة القلب.

٧ يستدعي وضع المريض على جهاز الايكمو سحب الدم من المريض عن طريق وضع أنبوب كبير في وريد مركزي (يصل قطره ما بين ٦ الي ١٠ مم) لسحب الدم من المريض وتمريره على الغشاء المنقي (الرئه الصناعية) لاستخراج غاز ثاني أكسيد الكربون ووضع غاز الاكسجين.

١٧ كما ذكرنا في السابق، لابد من وجود كفأت طبية لديها التدريب المتقدم والقدرة على التعامل مع هذه الحالات والجهاز والمضاعفات المصاحبة للحالة وجهاز الايكمو (في الاغلب يكون جراح قلب او طبيب متخصص في العناية مركزه حاصل على تدريب خاص في الايكمو)

١٢ يجب التنبيه ان جهاز الايكمو هو حل مؤقت وله فتره زمنية محدده لاستخدامه (تقريبا ما بين ٤ أيام ١٤ يوم وقد تصل الى ٢٨ يوم. قد تزيد او تنقص على حسب حالة المريض العامة واستجابته للعلاج وعدم حدوث مضاعفات من العلاج او المرض او الجهاز).

قمنا بعمل ٣٠ عملية جراحية معقده بين جراحين اثنين في خلال ستة ايام بفضل الله ثم بدعم حكومتنا الرشيده و الفريق الرائع من تمريض واستشاريين عناية مركزه وطب القلب والتخدير واخصائيين التنفس واخصائيين تروية القلب والفريق الاداري من @KSRelief وسفارة المملكة العربية السعودية في تنزانيا.

١٣ اثبتت عدة دراسات ان الإطالة في استخدام الجهاز (تقريبا اكثر من ٢١ يوم) يؤدي الى زيادة كبيره في نسبة حدوث المضاعفات (كالنزيف او الالتهابات الشديدة او الجلطات او ضمور العضلات غيرها) وهو مؤشر لشدة المرض وصعوبة علاجه وعدم قدرة الجسم على اصلاح او التخلص من الخلل

٣ يمكن تخيل القلب كمضخة والرئتين كمصفاه او منقي للهواء. إذا كان هناك ضعف شديد في احدى هذه الوظائف او كلاهما لدرجة تؤثر على الوظائف الحيوية يتم الاستعانة بجهاز الايكمو كبديل لهذه الوظائف الأساسية للمحافظة على حياة المريض حتى يتم السيطرة على السبب الأساسي لاعتطاب القلب او الرئتين.

٤ من الأسباب الشائعة لاعتطاب القلب والذي قد يستدعي استخدام جهز الايكمو بعد عمليات القلب المعقدة خصوصا في الأطفال الخدج او الالتهاب الفيروسي لعضلة القلب (Viral Myocarditis) او بعد زراعة القلب او جلطة كبيرة في الشريان الرئوي الأساسي او جلطة في القلب تؤدي الى ضعف شديد مفاجئ للعضلة

١٦ قد اثبتت عدة دراسات ان وضع المرضى في مثل هذه الحالات يقلل من نسبة الوفاة من ٩٠٪ الى ما بين ٥٠-٦٠٪ حسب معطيات محدده يتم اخذها في الحسبان (مثلا كحالة المريض العامة، نوع المرض، فشل أعضاء أخرى، الحاجة لاستخدام الجهاز لمساعدة الرئتين او القلب او كلاهما وغيرها).

١٤ في حال كتب الله للمريض الشفاء من المرض، يتم إزالة الجهاز وإعادة وظيفة الضخ او التنقية للقلب والرئتين بعد تعافيهما. هذا جزئ بسيط من رحلة العلاج الطويلة للمريض الذي كان على مشارف الموت والذي بفضل الله تم تعافيه.

١٥ للتوضيح، فإن اغلب المرضى الذي تستدعي حالتهم الصحية وضعهم على جهاز الايكمو هم مرضى نسبة تدهور حالتهم او الخطورة على حياتهم كبيره جدا (أكثر من ٩٠٪) وقد يتوفون من دون هذا الدخل

Post Procedure on Peak Airway Pressure went down from 36 to 29 & Peak Plateau from 32 to 27 on VCAC mode with PEEP of 8 Vt 500. X-Ray showed dramatic improvement of left lung aeration and off nor-epinephrine. Post Op Lung U/S showed almost complete resolution of the Hematoma

٩ يستدعي وضع المريض على جهاز الايكمو استخدام ادوية تزيد سيولة الدم بشكل عالي لأن خروج الدم واحتكاكه بالأنابيب والجهاز يؤدي الى تجلط في الدم. استخدام هذه المسيلات ضروري جدا لكن قد يزيد من نسبة النزيف او الحاجة لنقل الدم ومشتقاته.

Patient went home at day 2 after surgery. No pain, very satisfied, small scar and no residual ASD. We managed to avoid a sternotomy for this young patient. More to come! MIS cardiac surgery is a great option but requires a team work, support and perseverance.

@jojoksa1991 @SurgMBA كأي تدخل طبي (سواء كان علاج او جراحة او حتى الامتناع عن طعام معين) له فوائد واضرار. الفريق المعالج يضع في الحسبان انه لابد ان تكون الفائدة كبيره او ان التدخل الطبي ضروري لانقاذ حياة المريض (كالإنعاش للقلب او غسيل الكلى) لكي يصفوه او يقومو به.

Mini-ASD closure and Minimally Invasive Cardiac Surgery can be offered to some patients (children and adults) but the most important thing to remember is that "When there is will, there is way" and "You will never progress if you never try"

Patient is recovering but still have high airway pressures (Peak Airway Pressure= 36 & Peak Plateau= 32 on VCAC mode with PEEP of 7 Vt 450 on FiO2 45%). Good hemodynamics on Nor-epinephrine 0.05 and started to develop fever!!.

@naderjo @jedicath I do disagree; this concept of saving CABG for later is not correct at all. Operating on a 60 years old with stents 5-7 years ago is not similar and have higher risk. I argue that doing the CABG first and then when it fails in 7-15 years, then you can place the stents.
